An assessment of capitation in the new General Dental Service contract.

A 2-year study has been completed to examine the dental health and patterns of treatment of samples of children registered under capitation in three areas of England. The opportunity was taken to compare some of these results with those reported in the same areas in 1989 when the dentists were remunerated under fee-for-service. In addition, the validity of the dental health information supplied by general dental practitioners on the FP17C form was assessed. The dental health of 14-15-year-old patients registered for more than a year was compared with that of similar patients in the same areas

Clinical evaluation of a modified silver fluoride application technique designed to facilitate lesion assessment in outreach programs.

BACKGROUND: An advantage of using silver fluoride treatments for carious primary teeth in outreach programs especially where dental resources are limited is that the treatments can be carried out by dental auxiliaries. One limitation to date is that assessments of lesion status have been based on a tactile test where a sharp probe or explorer is drawn across the surface of a lesion to assess its hardness. This is a technique-sensitive step and has the potential for iatrogenic damage, especially when a lesion is deep. This study was undertaken to determine whether an alternative, non-invasive,

Localized idiopathic apical root resorption: a report of five cases with emphasis on differential diagnosis.

Seeking to describe the clinical and radiologic features of localized idiopathic apical root resorption (LIAR), the authors conducted a dental literature review of idiopathic external root resorption and performed a retrospective study of LIAR among the identified cases retrieved from a private general practice. Cases involved five patients--one female and four males--ranging in age from 15 to 48 years. The authors concluded that LIAR appears to be a distinct but uncommon clinicopathologic condition that is self-limiting, and therefore warrants no definitive treatment. However, the condition,

Evaluation of spontaneous space closure and development of permanent dentition after extraction of hypomineralized permanent first molars.

AIM: The aim of this study was to evaluate spontaneous space closure, development of the permanent dentition, and need for orthodontic treatment after extraction of permanent first molars due to severe molar-incisor hypomineralization (MIH). SUBJECTS: Twenty-seven children aged 5.6-12.7 (median 8.2) years had one to four permanent first molars extracted due to severe MIH. Each case was followed up on individual indications 3.8-8.3 (median 5.7) years after extractions. The eruption of the permanent dentition, and space closure were documented by orthopantomograms, casts, photographs, and/or bi

Segmental odontomaxillary dysplasia: report of a series of 5 cases with long-term follow-up.

We report a series of 5 cases of segmental odontomaxillary dysplasia (SOMD) with follow-up periods ranging from 8 to 21 years, bringing the total number of reported cases to 45. SOMD is a sporadic, mesoectodermal dysplasia that presents early in life, possibly as early as in utero and exhibits male gender predominance (1.7:1.0). Its features include enlargement of the soft tissue and/or bone of 1 hemimaxilla that may produce mild facial asymmetry. Subsequent growth of the affected area is proportional to that of the unaffected hemimaxilla. Sclerotic radiographic bone changes and dental develop

Treatment of an avulsed maxillary permanent central incisor replaced by autotransplantation of a mandibular premolar: 14-year follow-up.

AIM: To present the 14 year follow-up of a case in which an avulsed permanent maxillary central incisor was replaced by autotransplantation of a mandibular premolar. SUMMARY: A mandibular premolar (Moorrees' stage 3) was transplanted into the space left by an avulsed permanent maxillary central incisor after a delay of 3 months. Recall examination 14 years after transplantation revealed a normal periodontal architecture with absence of infection, ankylosis or progressive resorption. The transplantation of a premolar is seen as a promising method to replace a lost permanent tooth and to restor

External cervical resorption: an analysis using cone beam and microfocus computed tomography and scanning electron microscopy.

AIM: To provide a three-dimensional representation of external cervical resorption (ECR) with microscopy, stereo microscopy, cone beam computed tomography (CT), microfocus CT and scanning electron microscopy (SEM). SUMMARY: External cervical resorption is an aggressive form of root resorption, leading to a loss of dental hard tissues. This is due to clastic action, activated by a damage of the covering cementum and stimulated probably by infection. Clinically, it is a challenging situation as it is characterized by a late symptomatology. This is due to the pericanalar protection from a resorp

Fractal dimension on dental radiographs.

OBJECTIVES: (1) To compare fractal dimension (FD) from periapical radiographs with FD from panoramic radiographs; (2) to correlate FD with cortical thickness and morphology; and (3) to correlate FD with a reported history of osteoporotic fractures. METHODS: Information on fracture and smoking history was obtained by a telephone interview with 281 elderly (>60 years of age) patients who had periapical and panoramic radiographs exposed on the same date. FD was measured in several locations on both types of radiograph. Mandibular cortical thickness and morphology were obtained from the panoramic

Cone-Beam Computed Tomography Study Of Crown Dilaceration With a Talon Cusp in an Unerupted Permanent Maxillary Tooth.

This article describes a rare case of crown dilaceration with a talon cusp in an unerupted permanent maxillary central incisor. Our patient was a 7-year-old boy with a history of trauma to his primary maxillary teeth (#51 and 52), at 3 years of age complaining of failure of eruption of tooth #11. Periapical radiography showed incomplete formation of tooth root #11 and more superior position of tooth bud #11 relative to tooth bud #12. A cone-beam computed tomography was ordered, which revealed crown dilaceration with a talon cusp in tooth bud #11. The patient was scheduled for follow-up at 6 mo

Endodontic management of maxillary supplementary premolar using cone-beam computed tomography.

UNLABELLED: Extra teeth in dentition is termed as 'supernumerary teeth' and is a very well-known dental developmental anomaly. Supernumerary premolars are a rare anomaly in the maxillofacial complex. Its rarity and complex characteristics often makes it difficult to treat. Radiographs play an important role in assessment of both the location and the typing of supernumerary teeth. This article reports the rare case of 'nonsyndromic bilateral maxillary supernumerary premolars' in which the endodontic management of supernumerary premolar tooth is described. In this case the value of cone-beam com

Displacement of endodontic instruments in inferior alveolar canal.

Endodontic instrument breakage is a common occurrence during root canal treatment but the displacement of the separated instrument into the inferior alveolar canal is rare and has never been reported. We hereby present an unusual case of displacement of a separated instrument in the inferior alveolar canal and its retrieval by a simple technique.

Radiographic techniques for the pediatric patient.

Obtaining quality radiographs on pediatric patients can be a challenge. Suggestions for communicating with patients about radiation safety and the need for radiographs can facilitate the process. Guidelines for radiographic exposure intervals for young patients are key elements in the reduction of exposure to ionizing radiation and are presented in this paper. The child patient presents unique challenges for the dental professional and special techniques are presented in this article that may be helpful in the conducting radiographic examinations for this patient population.

Prevalence of hidden caries.

Clinical evaluations of the first and second permanent molars were compared with radiographic evaluations of the same teeth, for 359 patients (between six and eighteen years old) of the pediatric department of the Academic Centre for Dentistry Amsterdam (ACTA). The teeth were professionally cleaned and dried before the clinical evaluation. Despite the thorough examination, taking into account the fact that all discolorations and/or decalcifications were noted, 15 percent of the teeth that did not show signs of enamel caries clinically, showed a dentine lesion on the bitewing radiograph. These
