BACKGROUND AND PURPOSE: Reirradiation for locally recurrent rectal cancer (LRRC) has historically been restricted by concerns regarding significant adverse effects, particularly with reirradiation doses (in EQD2Gy) ≥ 30 Gy. Advances in radiation technology now enable safer delivery, renewing interest in (chemo)reirradiation as part of multimodal management. Given the scarcity of prospective data and the considerable variability in radiotherapy approaches, there is a clear need for recommendations to enhance treatment homogeneity and quality.
METHODS: A writing committee was established by the

The process of reirradiation in oncology presents a distinctive set of challenges, wherein clinical decision-making relies on a delicate balance between the efficacy of the treatment and the risk of severe toxicity. Despite the paucity of randomized data, there is an increasing number of data encouraging its consideration for patients with local or metastatic recurrence. While awaiting dedicated guidance from supranational scientific societies, the following publication is based on the "Recommendations Commission" of the Société française de radiothérapie oncologique (the French society for ra

We present the updated recommendations of the Société française de radiothérapie oncologique (the French society for radiation oncology) regarding the indications and technical modalities of radiotherapy for bone metastases. This treatment is commonly used in the management of patients with cancer. In most cases, it involves relatively simple radiotherapy technique with proven efficacy in reducing pain or managing spinal cord compression. More complex treatments using stereotactic conditions can be proposed for patients with painful bone metastases, oligometastatic disease, or in case of reirr

This study aimed to examine the efficacy and toxicity of reirradiation in patients with locally recurrent oral, pharyngeal, and laryngeal cancers. We conducted a retrospective, multi-institutional analysis of 129 patients with previously irradiated cancer. The most frequent primary sites were the nasopharynx (43.4%), oral cavity (24.8%), and oropharynx (18.6%). With a median follow-up duration of 10.6 months, the median overall survival was 14.4 months and the 2-year overall survival rate was 40.6%. For each primary site, the 2-year overall survival rates were 32.1%, 34.6%, 30%, 60.8%, and 5.7

Improvements in systemic therapy are translating into more patients living longer with metastatic disease. Bone is the most common site of metastasis, where spinal lesions can result in significant pain impacting quality of life and possible neurological dysfunction resulting in a decline in performance status. Stereotactic body radiation therapy (SBRT) of the spine has emerged as a promising technique to provide durable local control, palliation of symptoms, control of oligoprogressive sites of disease, and possibly augment the immune response. SBRT achieves this by delivering highly conforma

BACKGROUND: Reirradiation (reRT) is increasingly used for locoregional recurrences of lung cancer. Radiation pneumonitis (RP) remains a major dose-limiting toxicity, yet cumulative lung dose constraints are not well established.
METHODS: A systematic scoping review was conducted following PRISMA guidelines. Studies published between January 2010 and December 2024 were included if they reported an association between at least one lung dose-volume parameter and RP occurrence or severity after thoracic reRT. Only studies with explicit dose-toxicity data were retained. Evidence strength was asses

As reirradiation becomes increasingly common due to longer patient survival, accurately mapping previous radiation dose is essential to balance toxicity and tumour control. Image registration (IR) is widely used to align previous dose distributions with current anatomy. However, significant anatomic changes between treatment courses pose unique challenges, and it remains unclear how existing IR practices-largely developed for other contexts-translate to reirradiation. This systematic review aims to report on what is currently being done in the field regarding the use of IR for dose mapping in

Patients with bone metastasis are prevalent among those receiving palliative radiotherapy (RT), with approximately 20 % requiring reirradiation (reirradiation). The goal of bone reirradiation may be local control (oligoreoccurrence or oligoprogression of a previously treated lesion or in a previous treatment field) or symptomatic (threatening or painful progression). Published data on bone reirradiation indicate almost two-thirds of overall pain response. The primary organ at risk (especially for spine treatment) is the spinal cord. The risk of radiation myelitis is<1 % for cumulative doses of

BACKGROUND: Reirradiation with stereotactic body radiotherapy (SBRT) for patients with primary or secondary lung malignancies represents an appealing definitive approach, but its feasibility and safety are not well defined. The purpose of this study was to investigate the tumor control probability (TCP) and toxicity for patients receiving reirradiation with SBRT.
PATIENTS AND METHODS: Eligible patients with recurrence of primary or secondary lung malignancies from our hospital were subjected to reirradiation with SBRT, and PubMed- and Embase-indexed articles were reviewed. The patient charact

PURPOSE: Breast reirradiation (reRT) after breast conserving surgery (BCS) has emerged as a viable alternative to mastectomy for women presenting with recurrent or new primary breast cancer. There are limited data on safety of different fractionation regimens. This study reports safety and efficacy among women treated with repeat BCS and reRT.
METHODS AND MATERIALS: Patients who underwent repeat BCS followed by RT from 2015 to 2021 at 2 institutions were analyzed. Univariate logistic regression models were used to identify predictors of acute and late toxicities. Kaplan-Meier estimates were u

BACKGROUND: Gynecological reirradiation using brachytherapy (GRIB) represents an important salvage therapy for loco-regionally recurrent gynecologic cancers in previously irradiated patients. This systematic review aimed to evaluate current evidence on GRIB with a focus on reporting practices, identify literature gaps, and propose a framework to optimize future GRIB reporting.
METHODS: A systematic literature search was conducted in PubMed/MEDLINE according to PRISMA guidelines from January 1990 to September 2025. Full English-language publications addressing GRIB were included, excluding cas

PURPOSE: Reirradiation (reRT) has become increasingly prevalent due to an aging population and advancements in cancer detection and treatment. However, the field is still lacking standardized dosimetric evaluation methods and reRT workflows, which leads to difficulty in correlating clinical outcomes with delivered dose. This study reports on the implementation and evolution of a standardized reRT workflow in the Department of Radiation Oncology at University of Michigan, describing insights gained from nearly 3000 external beam reirradiation courses delivered since 2017.

Conventional external beam radiotherapy (cEBRT) is effective for managing symptomatic bone metastases and continues to be in demand despite advances in stereotactic body radiotherapy. This review provides an overview of cEBRT for bone metastases, with a focus on the following: (1) Initial palliative radiotherapy: randomized controlled trials and meta-analyses have shown that single-fraction cEBRT at 8 Gy is as effective as multifractionated cEBRT for reducing pain due to bone metastases. Single-fraction cEBRT at 8 Gy may be a reasonable option for bone metastases with neuropathic pain in consi

INTRODUCTION: Radiotherapy is the most common curative treatment for non-metastatic prostate cancer; however, up to 13% of patients will develop local recurrence within 10 years. Patients can undergo further and potentially curative treatment including salvage surgery, brachytherapy (BT), external beam radiotherapy, high-intensity focused ultrasound and cryotherapy. Systematic review shows that high-dose-rate (HDR) BT and stereotactic body radiotherapy (SBRT) have the best outcomes in terms of biochemical control and lowest side effects. The reirradiation options for previously irradiated pros
