Neurolysis and free microvascular omentum transfer in the treatment of postactinic palsies of the brachial plexus.

In this paper the results of treatment in 19 cases of postactinic paralysis of the brachial plexus are reported. The surgical technique aimed at resolving this condition, which is serious and often extremely painful, involves neurolysis and a long, narrow free omental graft and removal of the clavicle; the donor vessels are the left gastroepiploic arteries and veins; the recipient vessels are those distant from the irradiated area (often in the neck, transverse and facial arteries, veins affluent to the external jugular).

Microvascular anastomoses for bone grafts in the treatment of massive defects in bone.

Six patients with large defects in bone are described in whom we performed microvascular anastomoses of grafted fibular vessels (arteries and veins) to vessels in the recipient site. Two other patients, with massive loss of bone and skin, were treated by grafting of osteocutaneous composites also using microvascular anastomoses. All but one defect healed successfully. There is a wide potential for applications of these two techniques in the treatment of large segmental bone defects secondary to trauma or following tumor resection.

Surgical Complexities in Kidney Transplant of a Redo Donor and Recipient: A Case Report.

Performing surgery on a previously operated kidney can be difficult and technically demanding. Like re-operative renal surgery, redo donor nephrectomy is also risky, because normal tissue planes are obscured by perinephric fibrosis and adhesions, and dissection of hilar vessels can result in serious vascular damage. Revisiting an aborted implantation site in a recipient is also very demanding, and presence of arterial calcification adds to the difficulty. We report a unique case involving a donor and recipient who had previously proceeded with transplant surgery, but the transplant had been ab

Free muscle transfer.

Eight cases of free muscle transfer with the use of microsurgical technique are reported, comprising five patients with Volkmann's contracture of the forearm, two with poliomyelitis, and one with an old brachial plexus injury. Operation was successful in the six patients with traumatic injuries, but proved unsuccessful in the two with poliomyelitis. Generally, the vessels and nerves of the recipient site were anastomosed with their counterparts in the grafted muscle. In one patient with an old brachial plexus paralysis in whom nerves in the recipient site could not be used, muscle graft an int

Technical Considerations in Pedicle Management in Upper and Midfacial Free Flap Reconstruction.

OBJECTIVES: Mid and upper face free flaps frequently are associated with challenges due to pedicle length. We sought to evaluate the frequency at which alternative pedicle maneuvers were required for these reconstructions and determine if there was any association with flap survival or postoperative complications. STUDY DESIGN: Retrospective review at three tertiary care institutions. METHODS: Database review. RESULTS: Free flap reconstruction of the upper and midface was performed in 295 patients (108 bony, 187 soft tissue). In 82% of cases, the vessels reached the ipsilateral neck for ana

Major ABO-incompatible hematopoietic stem cell transplantation: study of post-transplant pure red cell aplasia and endothelial cell chimerism.

BACKGROUND: In contrast to human leukocyte antigen (HLA) matching, ABO-blood group incompatibility plays a minor role in the success of allogeneic hematopoietic stem cell transplantation (HSCT). Incompatible ABH histo-blood group antigens, expressed on recipient endothelial cells (EC) and donor erythroid progenitor cells, may represent targets for graft-versus-host disease (GVHD) and host-versus-graft reactions, respectively. The aims of the current study were to investigate: (1) red blood cell (RBC) engraftment and (2) EC chimerism as a potential result of replacement of recipient EC by donor

The fibula free flap: advantages of the muscle sparing technique.

The amount of muscle that should be retained on the free fibula during harvest is unresolved. Muscle is used to protect the periosteum, but by harvesting a large muscle cuff, the recipient and donor site morbidity increases. A retrospective review of 47 free fibula flaps performed between January 1997 and March 2002 was undertaken. There was an average follow-up of 15 months. The dissection method used for all cases was a muscle sparing technique where the peroneal vessels were skeletonised anteromedially. Only a very thin rim of muscle (1-2 mm) was left attached to the fibula. The recipient a

Mouse hind limb transplantation: a new composite tissue allotransplantation model using nonsuture supermicrosurgery.

BACKGROUND: The development of microsurgical techniques has facilitated the establishment of vascularized composite tissue transplant models in small mammals. Because the mouse would be the ideal model to study various composite tissue allotransplantation (CTA)-related problems, we designed two new surgical techniques for orthotopic (ORT) and heterotopic (HET) hind limb transplantation. METHODS: BALB/c hind limbs were transplanted to BALB/c or C57BL6 recipients using a nonsuture cuff technique. ORT: donor femoral vessels were anastomosed to recipient femoral vessels, the sciatic nerve approxi

Determination of the adequate vascular perfusion time of cross-leg free latissimus dorsi myocutaneous flaps in reconstruction of complex lower extremity defects.

BACKGROUND: Lower limb defects may be present due to various causes including infections, vascular diseases, tumor resections, and crush or avulsion injuries. Management of lower leg defects is a complex problem, especially when they are large with deep soft tissue loss. These wounds are difficult to be covered with local skin flaps, distant skin flaps or even conventional free flaps because of the compromised recipient vessels. In such cases, the vascular pedicle of the free flap could be anastomosed to the recipient vessels of the contralateral healthy leg temporarily and then divided after

Flap technology for reconstructions of urogenital organs.

PURPOSE OF REVIEW: The purpose of this review is to summarize the different reconstructive options for urogenital indications. The development of various flap techniques to restore congenital and acquired urogenital defects is presented. RECENT FINDINGS: Various reconstructive techniques have been demonstrated recently. On the basis of the reconstructive requirements, two main techniques can be defined: the standard local or regional flap technique (pedicled flap) and the more sophisticated microvascular free flap technique. Free tissue transplantation (transfer) is a procedure that involves

Extrarenal arteriovenous fistula. An unusual complication of percutaneous renal transplant biopsy.

A percutaneous allograft biopsy in a renal transplant recipient with declining function was followed by haemorrhage from the external iliac artery requiring repair of the puncture site. A common iliac arteriogram performed subsequently showed an external iliac arteriovenous fistula. In retrospect, an isotope scan performed at 48 h post-transplant, showed the upper pole of the graft overlying the iliac vessels. To avoid such complications we recommend a careful assessment of the anatomical relationships of the transplant kidney to major blood vessels prior to biopsy.

Supermicrosurgical reconstruction of nasal tip defects using the preauricular reversed superficial temporal artery flap.

BACKGROUND: Microsurgical reconstruction of nasal tip defects is difficult to achieve. The free composite auricular flap allows for repair in a one-stage procedure. However, anastomosis to the recipient facial artery is often complicated because of its variable anatomy and the need for a vessel graft. In this study, we describe our experience using the alar artery and angular vein as recipient vessels for direct super microsurgical anastomosis. METHOD: From February 2004 to December 2015, thirty-two patients with different degrees of full-thickness multi-subunit nasal tip defects were include

Reconstruction of severe atherosclerotic and obstructive diabetic feet using thoracodorsal artery perforator flaps with long vascular pedicles.

BACKGROUND: Foot ulcers in diabetic patients often require free flaps. A long vascular pedicle is mandatory so that more proximal vessels can be used as recipient vessels, especially when the flap is used to reconstruct distal foot defects. We evaluated the outcome of diabetic foot defect that were reconstructed using thoracodorsal artery perforator (TDAP) flaps with long vascular pedicles. PATIENTS AND METHODS: We reconstructed the foot defects of 22 diabetic patients. Nineteen patients were male, and the mean age was 56.4 years. The defects were located in big toe in 11 patients; fourth and

A reappraisal of the free groin flap in aesthetic craniofacial reconstruction.

Craniofacial defects are often associated with 3-dimensional volumetric contour deficiencies. The free groin flap was historically popular though technical limitations subsequently caused its use to fall from favor. We sought to determine if a role remained for the free groin flap in aesthetic craniofacial reconstruction. From May 2005 to July 2010, 15 patients had 18 consecutive free groin flaps performed by the senior surgeon at the R Adams Cowley Shock Trauma Center and the Johns Hopkins Medical Institute. Data collected included age, sex, operative date, etiology, defect size, recipient ve

Use of Recanalized Vessels for Diabetic Foot Reconstruction: Pushing the Boundaries of Reconstruction in a Vasculopathic Lower Extremity.

BACKGROUND: Free flap after lower extremity revascularization may enable limb salvage in defects after critical limb ischemia. This study examined the outcomes of reconstruction of ischemic diabetic foot according to the severity of the vessel occlusion and assessed whether recanalized vessels may serve as a reliable recipient vessel. METHODS: A total of 62 patients who underwent diabetic foot reconstruction with free flaps after successful percutaneous transluminal angioplasty (PTA) from February of 2010 to February of 2016 were identified and divided into three groups: group 1, nonoccluded

Bridging Lymphatic Vessel Defects with a Perforator Flap that Contains Retrograde Axial Lymphatic Flow.

Axiality of lymphatic flow is possibly an important consideration for stimulating recanalization of flow between the transferred flap and the recipient site during wound healing. Antegrade lymphatic flow reconstruction has previously been reported. Here we report the first case where lymph stasis following groin lymph nodes dissection could be recanalized in a pedicled anterolateral thigh flap in which the collecting lymphatics were arranged retrograde. A 78-year-old male patient presented with bilateral lower extremity lymphedema and bilateral inguinal skin ulcers with lymphorrhea. He had bee

Reconstruction of the Distal Leg and Foot Using Free Anterolateral Thigh Flaps in Patients With High-Voltage Electrical Burns.

In a patient with a high-voltage electrical burns, the extent of burning is greatest at the entrance and exit points of the electric current. As the exit point is usually the ankle and/or foot, these areas may be the most severely damaged. As local tissue is limited in this region, free tissue transfer is usually required for reconstruction. Eleven anterolateral thigh free flaps were placed for the reconstruction of foot and ankle defects caused by electrical burns. When the defects were large, we placed the flaps with two or three perforators. In six patients, recipient vessels were prepared

Twin-twin transfusion syndrome recipient with arterial calcification and heterozygous variant in ABCC6: Evidence of a gene-environment interaction?

We report a case of a twin-twin transfusion syndrome (TTTS) recipient who, after successful fetoscopic surgery, developed a large pericardial effusion and calcifications of the aorta and main pulmonary artery. The donor fetus never had cardiac strain and never developed cardiac calcifications. A heterozygous likely pathogenic variant in ABCC6 (c.2018T > C, p.Leu673Pro) was identified in the recipient twin. While TTTS recipient twins are at risk of arterial calcifications and right heart failure secondary to the disease, calcifications of the great vessels are also observed in generalized arter
