Reevaluating fibromyalgia diagnosis: a proposal to integrate deep tendon reflex responses into current criteria.

Fibromyalgia is a complex condition characterized by widespread pain, fatigue, and various other symptoms. The symptoms of fibromyalgia overlap with numerous other disorders (e.g., infections, chronic fatigue syndrome), which makes diagnosis challenging. Existing diagnostic criteria for fibromyalgia rely particularly on subjective patient reports. Such a limitation may lead to both missed diagnoses and potential overdiagnoses. Recent research has identified significantly increased deep tendon reflex (DTR) responses in patients with fibromyalgia. The results also demonstrated the potential for

Multivariable passive ankle impedance in stroke patients: A preliminary study.

Multivariable ankle mechanical impedance was estimated in four stroke survivors, in coupled dorsi- plantarflexion and inversion-eversion. We applied external torque perturbation with an ankle robot and used multi-input, multi-output stochastic system identification methods to estimate impedance, in both paretic and nonparetic limbs. Subjects were instructed to remain at rest throughout the four trials performed on each leg. Impedance projected onto the directions of maximum and minimum stiffness was fit to a 2nd order linear model, including inertia, viscosity and stiffness. For most trials, s

Contributors to excess antagonist activity during movement in children with secondary dystonia due to cerebral palsy.

Children with secondary dystonia due to cerebral palsy exhibit abnormal upper extremity postures and slow voluntary movement. However, the interaction between abnormal postures and abnormal movement in dystonia is still unclear. Some mechanisms by which postures are maintained in dystonia include stretch reflexes, overflow of muscle activation to other muscles, and direct coactivation of antagonist muscles. This study explored the independent contributions of each of these postural mechanisms to abnormal biceps brachii (antagonist) activity during elbow extension, which slows movement. A linea

Hypoxia potentiates, oxygen attenuates deflation-induced reflex tracheal constriction.

The reflex tracheomotor responses of in situ isolated segments of the extrathoracic trachea of anesthetized, paralyzed, and ventilated dogs were monitored. Reflex tracheal constriction was evoked by passive lung deflation. The purpose of this study was to determine whether the prevailing state of oxygenation altered the magnitude of this reflex. Compared with the magnitude of the response during normoxia [arterial O2 tension (PaO2) = 78 Torr], that during hypoxia (PaO2 = 44 Torr) was nearly threefold larger while that during hyperoxia (PaO2 greater than 250 Torr) was about 50% smaller. The iso

Neurophysiological observations on corticospinal projections to the upper limb in subjects with Rett syndrome.

The aim of the present study was to investigate the excitability of corticospinal neurons and the integrity of their projections to the alpha motor neurons through the corticospinal tract in subjects of different ages with Rett syndrome. Electromagnetic stimulation of the motor cortex and cervical motor roots was used to evoke motor action potentials in the biceps brachii and hypothenar muscles. The phasic stretch reflex in the biceps brachii was also recorded to study the excitability of spinal alpha motor neurons. Motor cortex stimulation evoked motor action potentials at low threshold and w

Flexor carpi radialis H-reflex in different body positions in patients with post-stroke.

BACKGROUND: Spinal stretch reflex (SSR) hyperexcitability reflected by the H-reflex has been reported in more strongly affected extremities after stroke. The H-reflex in the lower extremities is modulated by body position normally and alternatively modulated post-stroke. OBJECTIVE: This study aimed to preliminarily explore how upper extremity (UE) H-reflexes are modulated by body position after stroke, which remains unknown. MATERIALS AND METHODS: Three patients after stroke with hemiparesis/hemiplegia were included. Bilateral flexor carpi radialis (FCR) H-reflexes were examined in the supin

The patellar reflex in preeclamptic women with subtherapeutic and therapeutic serum magnesium levels.

The normalization of hyperreflexia has been proposed by some authorities as an indicator of adequate seizure prophylaxis in preeclamptic women receiving magnesium sulfate. To test this hypothesis the patellar reflex response was measured with a quantitative technique in 20 preeclamptic patients during their progression from subtherapeutic to therapeutic serum levels of magnesium. Patients meeting selected criteria for hyperreflexia demonstrated a 46% decrease in their reflex response. Those without initial hyperreflexia failed to demonstrate a significant decrease.

Physiologic and essential tremor.

We studied physiologic and essential hand tremor using inertial loading; hand acceleration and forearm EMG data were analyzed by auto- and cross-spectral analysis. Early essential tremor was qualitatively similar to the 8- to 12-Hz component of physiologic tremor, suggesting that this tremor component is a forme fruste of essential tremor. Advanced essential tremor had a frequency of 4 to 8 Hz. Patients with tremor frequencies in both ranges were observed in each of 10 families. In antagonistic forearm muscles, both synchronous and alternating tremor bursts were observed in 11 of 44 patients.

Self-reinnervated muscles lose autogenic length feedback, but intermuscular feedback can recover functional connectivity.

In this study, we sought to identify sensory circuitry responsible for motor deficits or compensatory adaptations after peripheral nerve cut and repair. Self-reinnervation of the ankle extensor muscles abolishes the stretch reflex and increases ankle yielding during downslope walking, but it remains unknown whether this finding generalizes to other muscle groups and whether muscles become completely deafferented. In decerebrate cats at least 19 wk after nerve cut and repair, we examined the influence of quadriceps (Q) muscles' self-reinnervation on autogenic length feedback, as well as intermu

Characteristics of the locomotor-like muscle activity during orthotic gait in paraplegic persons.

We previously found that orthotic gait training can induce 'locomotor-like' coordinated muscle activity of the paralyzed lower limb in persons with spinal cord injury (SCI). The purpose of the present study was to characterize the locomotor-like muscle activity based on data obtained from electromyographic recordings and motion analysis during orthotic gait in nine motor complete SCI subjects. Seven of nine subjects showed a common EMG activation pattern mainly in the ankle (soleus: Sol) and hip extensor (biceps femoris: BF) muscles. The locomotor-like muscle activity was well synchronized wit

Overview of human tremor physiology.

The physiology differs in the many forms of human tremor. Tremors may derive from mechanical oscillations, mechanical reflex oscillations, normal central oscillators, and pathologic central oscillators. Methods of studying tremor include accelerometry and electromyography (EMG). An excellent method consists of accelerometry and EMG combined with spectral analysis and weighting of the body part, which allows separation of tremors coming from mechanical reflex and central oscillators. Physiologic tremor is a mechanical tremor with a possible contribution of the normal 8-12 Hz central oscillator;

Sympathetic nerve responses to muscle contraction and stretch in ischemic heart failure.

Congestive heart failure (CHF) induces abnormal regulation of peripheral blood flow during exercise. Previous studies have suggested that a reflex from contracting muscle is disordered in this disease. However, there has been very little investigation of the muscle reflex regulating sympathetic outflows in CHF. Myocardial infarction (MI) was induced by the coronary artery ligation in rats. Echocardiography was performed to determine fractional shortening (FS), an index of the left ventricular function. We examined renal and lumbar sympathetic nerve activities (RSNA and LSNA, respectively) duri

[Function of proprioceptors of the cervical spine in the cervico-ocular reflex].

Disease of the cervical spine may cause vestibular vertigo. The anatomical basis for this type of cervical vertigo are the proprioceptive stretch receptors of the upper cervical joints. These are connected to the vestibular nuclei, and, thus, to ocular reactions (nystagmus). Quantitative data about this cervico-ocular reflex are still scarce since the reflex is strongly suppressed in healthy persons. However, suppression ceases in bilateral complete vestibular deficiency. We measured the cervico-ocular reflex in 6 patients with complete bilateral labyrinthine deficiency. One patient showed a c

Interstitial cells of Cajal mediate mechanosensitive responses in the stomach.

Changes in motor activity are a basic response to filling of smooth muscle organs. Responses to gastric filling, for example, are thought to be regulated by neural reflexes. Here, we demonstrate a previously uncharacterized aspect of stretch-dependent responses in visceral smooth muscles that is mediated by mechanosensitive interstitial cells of Cajal. Length ramps were applied to the murine antral muscles while recording intracellular electrical activity and isometric force. Stretching muscles by an average of 27 +/- 1% of resting length resulted in 5 mN of force. Increasing length caused mem

A brief physiology and pathophysiology of the anorectum based on the Integral Theory paradigm.

The remit of this review is confined to the experimental scientific works and surgeries based on the Integral Theory paradigm. The video abstract summarizes the anorectal function, how ligaments cause dysfunction and cure of fecal incontinence and obstructed defecation by ligament repair. Anorectal function is reflex and binary, with cortical and peripheral components. The same three oppositely acting reflex muscle forces which open and close the bladder, contract against the pubourethral (PUL) and uterosacral (USL) ligaments: (I) to close the anorectum for continence when the puborectalis mus

Desensitization of the Mechanoreceptors in Müller's Muscle Reduces the Increased Reflex Contraction of the Orbicularis Oculi Slow-Twitch Fibers in Blepharospasm.

OBJECTIVE: Although the mixed orbicularis oculi muscle lacks the muscle spindles required to induce reflex contraction of its slow-twitch fibers, the mechanoreceptors in Müller's muscle function as extrinsic mechanoreceptors to induce reflex contraction. We hypothesize that strong stretching of these mechanoreceptors increases reflex contraction of the orbicularis oculi slow-twitch muscle fibers, resulting in blepharospasm. METHODS: We examined a 71-year-old man with right blepharospasm and bilateral aponeurosis-disinserted blepharoptosis to determine whether the patient's blepharospasm was w
