Management and rehabilitation of ligamentous injuries to the ankle.

The management of ligamentous injuries to the ankle is controversial. Neither the methods for classification and diagnosis, or the procedures for treatment are clear cut. Ankle sprains are a common occurrence, with the majority involving the lateral ligament complex. Within this complex, the anterior talofibular ligament is injured most frequently, usually while the foot is in the plantar flexed position. Ankle injuries can be diagnosed through physical exam, including the anterior drawer test and/or a stress exam, or through roentgenographic evaluation. The purpose of the stress roentgenogram

Good clinical scores, no evidence of excessive anterior tibial translation, a high return to sport rate and a low re-injury rate is observed following anterior cruciate ligament reconstruction using autologous hamstrings augmented with suture tape.

INTRODUCTION: Augmented anterior cruciate ligament reconstruction (ACLR) techniques have been proposed to reduce the high reported re-injury rates and low rates of return to sport (RTS). This study reports clinical outcomes, RTS and re-injury rates in patients undergoing ACLR using autologous hamstrings augmented with suture tape. MATERIALS AND METHODS: A total of 53 patients were prospectively recruited, undergoing ACLR using hamstrings with suture tape augmentation, combined with a structured rehabilitation programme. Outcomes were collected to 24 months, including patient-reported outcome

Editorial Commentary: Prolonged Joint Effusion After Hip Arthroscopy May Be Associated With Persistent Symptoms.

Research to investigate causes for continued symptoms in patients who undergo hip arthroscopic treatment is an opportunity to improve outcomes. A disconcertingly large number of patients (47%-66%) show a joint effusion/synovitis on magnetic resonance imaging at 6 to 12 months after hip arthroscopy for femoroacetabular impingement syndrome, and a recent study shows that this is associated with inferior 2-year clinical outcomes compared to patients without effusions. Perhaps the effusions are associated with comorbid structural abnormalities such as cartilage degeneration, capsule defect, labral

Rehabilitation: Proprioception, Incoordination, and Paresis.

Physiotherapeutic exercises aimed at stimulating motor control, flexibility, and stability are regularly employed in human physical therapy programs. Specifically, the use of such exercises has been shown to reduce both pain and reinjury. Pursuant to the equine patient, several core strengthening exercises and their role in activating deep epaxial musculature to subsequently improve postural motor control and alter thoracolumbar kinematics have been investigated. Both baited and passive exercises offer opportunities to facilitate stretching during dynamic phases and strengthening during static

Open Re-rupture of the Achilles Tendon Following Minimally Invasive Repair: A Case Report.

In this report, we describe a rare complication of an open re-rupture of the Achilles tendon following a minimally invasive Achilles tendon repair on a healthy 29-year-old active male. The reinjury happened 19 weeks following the primary surgical repair of a spontaneous rupture, performed by minimally invasive technique with the help of a jig using partially absorbable sutures and four locking stitches. The wound of the open re-rupture was transverse, in a perpendicular orientation relative to the longitudinal approach used in the index procedure. Increased scar tissue formation, the absence o

The incidence of ankle sprains in soccer.

This study investigated the relationship between exposure time and ankle sprains in soccer. Forty-one teams (639 players) from four male senior soccer divisions at different levels of skill (divisions I-VI) were followed prospectively for 1 year. The exposure to soccer and the number of injuries per player were higher in higher divisions, but the injury incidence, percentage of ankle injuries and incidence of ankle injuries were the same at different levels of skill. Of all injuries 17 to 20% were ankle sprains and the incidence varied between 1.7 to 2.0 ankle injuries per 1,000 hours of expos

Treatment of Aneurysmal Bone Cyst with Endoscopic Resection and Bone Allograft with Platelet-Rich Plasma: A Case Report.

An aneurysmal bone cyst (ABC) is a rare bone tumor usually observed in long bones. The surgical treatment of this pathology is often related to high recurrence rates, so novel biological techniques can help to enhance tissue regeneration and bone consolidation. We present a case of a patient with ABC of the calcaneus treated with an endoscopic resection followed by grafting with an autologous-based matrix composed of allograft bone chips and autologous platelet-rich plasma (PRP) in semisolid and liquid states. Patient demonstrated excellent defect filling in both magnetic resonance imaging and

The treatment of soft tissue after spinal injury.

There are a number of modalities available for the treatment of soft tissue that are applicable to spinal injuries. Each has a long history of use; however, there is relatively little scientific data to support the effects often seen in the clinical setting. Only through careful evaluation and specific diagnosis can these methods of treatment be utilized to their fullest potential. The underlying pathology must always be addressed as well as its soft-tissue manifestations. Direct treatment of the mechanical derangement must take precedence when such conditions exist. The total treatment must i

Redefining Readiness - Bridging Knowledge Gaps in Return-to-Sport Criteria.

CONTEXT: Return-to-sport (RTS) decision-making is a pivotal phase in athletic rehabilitation, yet current practices are often fragmented and applied inconsistently. Inadequate interdisciplinary collaboration, misinterpretation of physician clearance, and systemic barriers contribute to suboptimal outcomes and elevated reinjury risk. The purpose of this review is to highlight critical knowledge gaps in RTS criteria and propose a multidisciplinary, biopsychosocial framework that integrates physical, psychological, and sport-specific readiness. The review emphasizes the importance of standardized

Anterior Cruciate Ligament Reconstruction Return to Sport Testing Passing Rates for Healthy People: A Systematic Review.

BACKGROUND: Return to sport (RTS) is a common goal after anterior cruciate ligament (ACL) reconstruction (ACLR) but carries a relatively high risk of reinjury with up to 20% to 25% of athletes experiencing graft rupture or contralateral ACL tear. While there is increased emphasis on establishing safe RTS criteria for athletes to return to previous activity levels, studies show that even healthy individuals have difficulty passing RTS testing. PURPOSE: To synthesize data concerning whether healthy individuals can pass ACLR RTS rehabilitation tests. STUDY DESIGN: Systematic review; Level of ev

Core Muscle Injuries: Preoperative Imaging and Postoperative Considerations.

Core muscle injuries constitute a significant source of pain and functional impairment among elite athletes. Understanding the anatomy and biomechanics centered at the pubic bone joint is necessary to interpret imaging findings. Dedicated athletic pubalgia magnetic resonance imaging (MRI) plays a crucial role in accurately diagnosing and assessing injuries affecting the pubic plate and associated muscular attachments. Preoperative findings can include harness muscle injury, pubic plate detachment, and/or osteitis pubis. Various surgical approaches are used for core muscle repair, requiring clo

Evaluation and management of hamstring injuries.

Muscle injuries are the most common injuries in sports, with hamstring injuries accounting for 29% of all injuries in athletes. These injuries lead to prolonged impairment and have a reinjury risk of 12% to 31%. They range from mild muscle damage without loss of structural integrity to complete muscle tearing with fiber disruption. Novel MRI scores are increasingly being used and allow a more precise prediction of return to sport. In this article, the authors review the history, mechanisms of injury, and classification systems for hamstring injuries as well as present the latest evidence relat
