Pain management in the wilderness and operational setting.

The wilderness and operational setting places unique constraints on one's ability to treat pain. In this article we will discuss methods for treating pain both in the wilderness and operational setting. By operational we mean the austere deployed military setting, to include both noncombat and combat operations. The authors combined experience with wartime trauma pain management consists of experience in Operation "Just Cause" (Panama Invasion), Operation "Desert Storm" (Persian Gulf War), Operation "Uphold Democracy" (Haiti liberation), Operation "Enduring Freedom" (Afghanistan conflict), and

Preplacement nerve testing for carpal tunnel syndrome: is it cost effective?

UNLABELLED: Is not hiring otherwise-qualified workers who have an abnormal post-offer preplacement (POPP) median nerve test a cost-effective strategy to reduce workers' compensation expenses related to carpal tunnel syndrome (CTS)? We performed a retrospective dynamic cohort study based on 2150 workers hired at a company between January 1996 and December 2001 and who underwent POPP median nerve testing. Workers were followed until they left the company or until follow-up ended in May 2003. RESULTS: Thirty-five cases of work-related CTS occurred during follow-up, and 9.13 cases could have been

Rescuing beyond cure: Adapting rescue fantasies in palliative care.

Rescue fantasies, or the desire to "save" patients, are common among healthcare providers. However, in palliative care, when curing is not an option, these fantasies may conflict with the realities of end-of-life care. This study explores how palliative care staff modify their rescue fantasies to align with the unique characteristics of their work. Thirteen staff members from an Israeli hospital's palliative care unit participated in in-depth, semi-structured interviews. The data were analyzed using thematic analysis. Participants acknowledged the impossibility of saving patients' lives in pal

[Life-saving air supported avalanche mission at night in high alpine terrain].

This is a case report about a helicopter emergency medical service (HEMS) operation during the night in response to an avalanche accident with two completely buried victims. One of the victims was rescued alive after 9.2 h presenting with a patent airway and an air pocket and was successfully rewarmed with forced air from 23°C core temperature without any neurological deficits. After the rescue the patient developed lung edema which resolved spontaneously within 2 days. The second victim was found dead presenting with an air pocket but solid frozen thorax. The special circumstances of the resc

Retrospective analysis of 616 air-rescue trauma cases related to the practice of extreme sports.

INTRODUCTION: Extreme sports (ESs) are increasingly popular, and accidents due to ESs sometimes require helicopter emergency medical services (HEMSs). Little is known about their epidemiology, severity, specific injuries and required rescue operations. AIM: Our aims were to perform an epidemiological analysis, to identify specific injuries and to describe the characteristic of prehospital procedures in ES accidents requiring HEMSs. METHODS: This is a retrospective study, reviewing all rescue missions dedicated to ESs provided by HEMS REGA Lausanne, from 1 January 1998 to 31 December 2008. ES

Patterns of death among avalanche fatalities: a 21-year review.

BACKGROUND: Avalanches are a significant cause of winter recreational fatalities in mountain regions. The purpose of this study was to determine the relative contributions of trauma and asphyxia to avalanche deaths. METHODS: We reviewed all avalanche fatalities between 1984 and 2005 that had been investigated by the offices of the British Columbia Coroners Service and the Chief Medical Examiner of Alberta. In addition, we searched the database of the Canadian Avalanche Centre for fatal avalanche details. We calculated injury severity scores for all victims who underwent autopsy. RESULTS: The

The great East Japan earthquake of March 11, 2011, from the vantage point of blood banking and transfusion medicine.

BACKGROUND: The Great East Japan Earthquake of March 11, 2011, and subsequent tsunami took nearly 20 000 lives in Tohoku, the northeastern part of Japan's main island. Most victims were either carried away by the tsunami or drowned. The ability to collect blood was disrupted on the Pacific coast of Tohoku. Inland areas were less affected, but allogeneic blood collected in Tohoku is tested at the Miyagi Red Cross Blood Center (Miyagi Center) in the coastal city of Sendai. Miyagi Center was damaged and could not test for 2 months. OBJECTIVES: The aims of this study are as follows: (1) to assess

Evacuating damaged and destroyed buildings on 9/11: behavioral and structural barriers.

INTRODUCTION: Evacuation of the World Trade Center (WTC) twin towers and surrounding buildings damaged in the September 11, 2001 attacks provides a unique opportunity to study factors that affect emergency evacuation of high rise buildings. Problem The goal of this study is to understand the extent to which structural and behavioral barriers and limitations of personal mobility affected evacuation by occupants of affected buildings on September 11, 2001. METHODS: This analysis included 5,023 civilian, adult enrollees within the World Trade Center Health Registry who evacuated the two World Tr

[Experience of DMAT rescue activity by doctor-helicopter in Tohoku Area after the earthquake].

We operated rescue activities in Tohoku area after the earthquake of March 11th, 2011. From our hospital, a doctor-helicopter flew to the staging care unit at Hanamaki airport with two members of the disaster medical assistance team (DMAT), one of whom was an anesthesiologist. The helicopter carried ten patients by nine flight missions, who were the victims of tsunami after the earthquake. There were seven doctor-helicopters from all over Japan and did the missions based at Hanamaki airport. The missions was quite different from our usual job as an anesthesiologist, but we could transfer the p

Respiratory symptoms were associated with lower spirometry results during the first examination of WTC responders.

OBJECTIVE: Determine if World Trade Center (WTC) disaster responders had lower lung function and higher bronchodilator responsiveness than those with respiratory symptoms and conditions. METHODS: We evaluated cardinal respiratory symptoms (dyspnea, wheezing, dry cough, productive cough) and determined the difference in FEV1, FVC, and bronchodilator responsiveness. RESULTS: All respiratory symptoms were associated with a lower FEV1 and FVC, and a larger bronchodilator response. Responders reporting chronic productive cough, starting during WTC work and persisting, had a mean FEV1 109 mL lower

The Mt. Tyndall incident.

The authors describe the 53-hour rescue of a 6-foot, 1-inch tall, 250-pound hiker in the face of harsh environmental conditions in Sequoia National Park. This 43-year-old man fell 25 feet, injured his leg, and was noted to be hypothermic and hypovolemic. Weather, altitude, and the patient's size delayed and complicated his evacuation. After being carried down 1,500 vertical feet, he was hoisted into a hovering helicopter and flown to University Medical Center in Fresno, California. On arrival, the patient was determined to have a comminuted subtrochanteric right femur fracture, which was ultim

Reflux symptoms and disorders and pulmonary disease in former World Trade Center rescue and recovery workers and volunteers.

BACKGROUND: Gastroesophageal reflux disease is one of the most prevalent conditions among former World Trade Center (WTC) rescue and recovery workers. The reason for this proposed association with an inhalation injury is unclear. In this study, we clinically characterized the reflux disorders in former WTC workers, and we investigated their association with pulmonary function abnormalities and with clinical diagnoses of other WTC-related diseases. METHODS: Forty-two former WTC workers underwent the following testing: symptom inventories, physical examination, spirometry, esophagogastroduodeno

[Health consequences of the Chernobyl accident].

The Chernobyl accident in 1986 has been and still is the subject of great interest. Journalistic reports often contain exaggerations and undocumented statements and much uncertainty about the true consequences of the accident prevails in the population. This article reviews the current literature with the focus on reports from official commissions and documentation in the form of controlled studies. The fatal deterministic consequences comprise about 30 victims. The most important outcome is a marked increase in the incidence of thyroid cancer in children and adolescents in the most heavily co

Anatomically defined neuron-based rescue of neurodegenerative Niemann-Pick type C disorder.

Niemann-Pick type C disease is a fatal lysosomal storage disorder caused by loss of NPC1 function. The disorder severely affects multiple body systems, particularly the nervous system. To test whether rescue of NPC1 activity in neurons, astrocytes, or other cell types can correct the neurological defects, a Tet-inducible Npc1-YFP transgene was introduced into Npc1(-/-) mice for the cell type-specific rescue of NPC1 loss. NPC1-YFP produced in neurons prevented neuron degeneration, slowed reactive glial activity, and ameliorated the disease. NPC1-YFP produced in astrocytes or in cells of viscera

Energy Expenditure of Disaster Relief Operations Estimated Using a Tri-Axial Accelerometer and a Wearable Heart Rate Monitor.

The management of nutrition, food, and health for disaster relief personnel is one of the crucial aspects for carrying out effective rescue activities during large-scale natural disasters, such as a big earthquake, flooding, and landslide following heavy rainfall or man-made disasters, such as widespread fire in industrial areas. Rescue workers, such as fire fighters and rescue teams who work on the disaster relief operations, have to work long, hard, and irregular hours that require energy (both intake and expenditure), with especially altered eating patterns. Reliable estimates of the energy
