Readmission drivers for children with medical complexity: Home nursing shortages cause health crises.

OBJECTIVE: Children with medical technology dependence (MTD) are frequently readmitted to the hospital. However, due to their medical fragility, it is often difficult to untangle the root causes for readmissions to identify the most effective preventive approaches. We sought to explore environmental and family factors driving hospital readmissions for children with MTD. DESIGN: Semi-structured, in-person interviews were conducted with state-wide care coordinators for children with MTD in Illinois with at least 1 year of experience. Interview topics related to children with MTD transitioning f

Interventions for Health: Why and How Health Care Systems Provide Programs to Benefit Unhoused Patients.

Individuals experiencing homelessness have poorer health than housed individuals, while also utilizing more emergency department care and fewer preventive services. Several interventions - including permanent supportive housing, medical respite, and mobile medical clinics - are cost-effective means to improve health outcomes for homeless populations, yet few health systems have invested in such programs. This study aimed to determine the reasons some health systems initiated these interventions, and the early experience of those health systems that did. Quantitative analyses of health systems

Factors related to survival discharge in trisomy 18: A retrospective multicenter study.

Infants with trisomy 18 (T18) previously had a poor prognosis; however, the intensive care of these patients has markedly diversified the prognosis. We investigated the current situation of patients with T18, clarified factors for survival discharge, and surveyed actual home healthcare. A total of 117 patients with T18 admitted to nine institutions between 2000 and 2015 were retrospectively investigated. After excluding four patients whose outcomes were unclear, we divided 113 patients into two groups-the survival discharge group (n = 52) and the death discharge group (n = 61)-and compared mat

Pediatric healthcare costs for patients with 22q11.2 deletion syndrome.

BACKGROUND: The 22q11.2 deletion syndrome is a variably expressed disorder that can include cardiac, palate, and other physical abnormalities, immunodeficiency, and hypocalcemia. Because of the extreme variability in phenotype, there has been no available estimate of the total medical expenditure associated with the average case. METHODS: We have developed a model to estimate the cost from the time of diagnosis to age 20. Costs were based on patients seen at a specialty center but also considered those components of care expected to have been provided by external healthcare facilities. Expens

Caregiver impact and health service use in high and low severity Dravet syndrome: A multinational cohort study.

PURPOSE: To estimate costs associated with the current management of Dravet syndrome (DS), explore psychosocial aspects of the disease in caregivers and siblings, and identify patient characteristics associated with higher costs in a large, predominantly European survey cohort of patients and their caregivers conducted in 2016. METHODS: Health and social care resource use, productivity and quality of life (QoL) data were summarised. Costs for European five (EU5) countries (France, Germany, Italy, Spain and UK) were calculated and patients with high and low current seizure burden compared. Dir

Preliminary Findings of an Adapted Nurse-Led Palliative Care Intervention.

Context: Despite the increased number of people living with Alzheimer's disease and related dementias (PLWD), limited early palliative care interventions exist for this population. Adapting promising interventions for other progressive disease conditions may address this need. Few published studies have examined this topic using recognized adaptation frameworks. Objectives: To systematically adapt a nurse-led palliative care intervention for people with idiopathic pulmonary fibrosis and caregivers (A Program of SUPPORT™) for PLWD and caregivers before pilot/feasibility testing. Methods: The St

Parental views on otitis media: systematic review of qualitative studies.

UNLABELLED: This study aims to describe parental experiences and perspectives of caring for a child with otitis media. We conducted a systematic review of qualitative studies on parental perspectives on caring for a child with otitis media. We searched electronic databases to July 2015. Seventeen studies involving 284 participants from six countries were included. We identified seven themes: diminishing competency (guilt over failure to identify symptoms, helpless and despairing, fear of complications, disempowered and dismissed); disrupting life schedules (disturbing sleep, interfering with w

Burden of Alzheimer's disease: population-based estimates and projections for New Zealand, 2006-2031.

OBJECTIVE: To estimate the burden of Alzheimer's disease (AD, including 'mixed' dementia) in New Zealand in 2006, and project this burden out to 2031. METHOD: An incidence to prevalence methodology was utilized, the foundation of which is a discrete time Markov model allowing for multiple stages of disease (early vs late). Population estimates and projections, and all-cause mortality rates, were obtained from Statistics New Zealand. In the absence of usable New Zealand data, data on disease incidence and progression were obtained from systematic reviews of the international (mainly European,

Caregiving Burden and Quality of Life Among Parents of Individuals With Angelman Syndrome: Gender Differences and the Impact of Financial Well-Being.

BACKGROUND: A person with Angelman syndrome (AS), a rare neurogenetic condition that significantly affects health and daily functioning, needs lifetime specialized care. Consequently, parents of individuals with AS experience decreased quality of life, high levels of parental stress and burden, and severe financial strain. This study analyses the experiences of Polish parents of persons with AS. Gender differences between mothers and fathers are also reported, and the association between financial well-being, caregiving burden, and quality of life is evaluated. METHODS: A self-administered, a

The influence of auditory experience on the behavior of preterm newborns.

This review of the research literature is addressed to clinicians who care for, interact with, and advise parents of preterm newborns. In summary, research on the effects of sound on preterm infant behavior and development provide only a little reliable clinical guidance as many of the studies are flawed, some badly. Some studies use a high sound level (i.e., loud) stimulus yet fail to report effects of the stimulus on vital signs, movement, and behavioral state. Only a handful of studies report long term effects regardless of the length of exposure to the experimental stimulus. A consistent p

"I Have Always Existed, but Never Been Noticed": A Hermeneutic Phenomenological Study of Social Frailty Among Prostate Cancer Survivors.

BACKGROUND AND AIM: Survivors may experience social frailty due to physical, psychological, and social challenges after treatment. However, social frailty remains underrecognized in prostate cancer survivorship care. This study aimed to explore survivors' lived experiences of social frailty using a Heideggerian phenomenological approach and provide insights for multidisciplinary survivorship care. METHODS: A qualitative design grounded in Heideggerian interpretive phenomenology was employed. Data were collected between May and July 2025 at the Department of Urology, Shanghai East Hospital. Pa

Chemical restraint as behavioural euthanasia: case studies from the Royal Commission into Aged Care Quality and Safety.

BACKGROUND: The prescription of psychotropic medication to older people living with dementia in residential aged care has become an increasing concern. The use of prescription medication is often prefaced as a way of preventing harm to self and others. However, the use of such medications has been considered a way of managing some of the behavioural and psychological symptoms of dementia. Using a large secondary data set, this study aimed to identify the precursors and mediating factors that influence the use of chemical restraint of older people in residential aged care. METHODS: Publicly av

The Impact of Dementia Caregiving on the Health of the Spousal Caregiver.

Dementia caregiving represents a major public health challenge, with spousal caregivers assuming the greatest burden. Spouses, themselves typically older adults, provide high intensity, long-term, and largely unpaid care across all stages of cognitive decline. Despite their central role in dementia care, the health consequences experienced by spousal caregivers remain insufficiently characterized in the literature and inadequately addressed in clinical and public health practice. This structured narrative review synthesizes current evidence on the multidimensional impact of dementia caregiving

Children with medical complexity: a scoping review of interventions to support caregiver stress.

BACKGROUND: Caring for children with chronic and complex medical needs places extraordinary stress on parents and other family members. A scoping review was undertaken to identify and describe the full range of current interventions for reducing caregiver stress. METHODS: Applying a broad definition of caregiver stress, a systematic search of three scientific databases (CINAHL, Embase and Ovid Medline), a general internet search and hand searching of key peer-reviewed articles were conducted. Inclusion criteria were as follows: (i) published in English between 2004-2016; (ii) focused on famil

Caregivers' Difficulty Rating Scale: Development and Initial Validation of a Tool to Identify the Unmet Needs of Indian Caregivers.

BACKGROUND: Caring for a cancer patient is debilitating for caregivers, especially among Indian population, as culturally people prefer to care at home than at nursing home. Unavailability of palliative care services and professional caregivers adds to the family burden. OBJECTIVES: Caregiving difficulties need assessment, but cancer-specific burden instruments are rare in the Indian context. This article presents development and initial validation of Caregiver's Difficulty Rating Scale (CDRS) and highlights the nature of caregiving burden on primary caregivers. METHODS: A total of 108 items
