[Retinal detachment after excimer laser surgery].

The increasing number of patients with myopia who had an excimer operation arouses among retinal specialists and refraction surgeons the question whether laser operation can cause or accelerate retinal damage and initiate a motion of the retina. During the last 4 years the authors treated 5 patients with a motion of the retina who were previously subjected to excimer laser surgery on account of severe myopia. The laser procedures were implemented in different departments and then the patients were referred to us on account of complications. Poorer results and a poorer prognosis were recorded i

[Posterior scleritis with exudative retinal detachment].

PURPOSE/METHODS: We report the findings in a patient who presented with exudative macular detachment and discuss the most useful ancillary test and the treatment of this uncommon illness. The diagnostic tests show the typical features of this pathology. RESULTS/CONCLUSIONS: The macular exudative retinal detachment is one of the most frequent features of the posterior scleritis. The case that we report has similar findings to other cases described in the literature. Posterior scleritis is an uncommon disease, but we must take it into account in the differential diagnosis of a macular exudative

Retinal detachment surgery in Marfan's syndrome.

BACKGROUND: Retinal detachment is a known complication of Marfan's syndrome. The literature is scarce on the results of surgery in these cases. PURPOSE: To determine the functional and anatomic outcome of retinal detachment surgery in Marfan's syndrome. METHOD: Retrospective review of charts of Marfan's syndrome patients who had retinal detachment surgery at King Khaled Eye Specialist Hospital between 1983 and 1995. RESULTS: Fourteen patients with 16 surgically treated eyes were identified. Retinal detachment was bilateral in 9/13 (69%) patients. The lens was ectopic in all eyes. Retinal br

[Toxoplasmic chorioretinitis complicated by retinal detachment].

PURPOSE: To describe and analyse relationship between chorioretinal toxoplasmosis and retinal detachment. PATIENTS AND METHODS: Seven immunocompetent patients examined and treated between November 1992 and March 1996, with ocular toxoplasmic retinochoroiditis and retinal detachment. RESULTS: Of the 7 patients examined, 5 had active retinochoroiditis and 2 had typical inactive scars. Of the patients with active focus 3 had giant retinal tears, one had a posterior retinal tear and one had a retinal tear located at the edge of an atrophic scar. Of the patients with inactive lesions, one had tra

[Vitreous surgery in complicated retinal detachment].

Vitreous surgery in complicated retinal detachment has three main objectives: 1. elimination of traction and mobilization of the retina, 2. inner tamponade of the retina, 3. retinopexy. Pars plana vitrectomy with inner tamponade is indicated: 1. In rhegmatogenous retinal detachment, where vitreous hemorrhage or exsudate make the finding and localization of the retinal break impossible. 2. In rhegmatogenous retinal detachments, where the localization of the break in the posterior eye pole or the size of a giant tear prevent to close the retinal break by means of a cryosurgical procedure with ep

Retinal detachment in spondyloepiphyseal dysplasia congenita.

Retinal detachment is one of the most serious complications of spondyloepiphyseal dysplasia congenita (SEDC). The ocular complication of 12 patients with SEDC was studied. Three patients developed bilateral retinal detachments; five of the six detachments occurred during the period of rapid growth spurt. Our results suggest that patients with SEDC have an increased risk for development of retinal detachment during the period of rapid growth spurt.

[Retinal detachment with posterior chamber lens].

Pseudophakic retinal detachment was seen in 55 cases out of 13,692 patients that underwent cataract surgery and posterior chamber lens implantation in a 5 year period. The six-month incidence was 0.4%. Main risk factors were: myopia, capsular tear resp. zonular dialysis, and male gender. Sulcus- and capsular bag fixation showed the same incidence. 42% of pseudophakic retinal detachments occurred within the first 12 months after cataract surgery and 84% within the first three years. The resulting cumulation rate is discussed. The outcome of detachment surgery was reduced with increased rate of

[Aphakic retinal detachment in myopic eyes].

The incidence of retinal detachment following cataract extraction was studied in a myopic population of 803 eyes, which were selected by a measurement of the axial length out of 3,184 cases consecutively operated on. The incidence of retinal detachment following intracapsular cataract extraction was 1.5%, which was very similar to that after extracapsular cataract extraction: 1.8%, in the lowest range of date published in the literature. The incidence of retinal detachment of the pseudophakic myopic eye (operated on with the extracapsular technique, combined with implantation of a posterior ch

Exudative retinal detachment after photodynamic injury.

Exudative retinal detachments occur in a variety of retinal and choroidal diseases. We created serous retinal detachment in the cat eye by means of photodynamic injury produced by activation of intravascular rose bengal using filtered, focused light (550 nm). Fluorescein angiography later revealed focal retinal and choroidal vascular occlusion surrounded by a larger area of leakage through the pigment epithelium. Serous retinal detachments occurred rapidly, gradually enlarged over the next 3 days, and resolved in all eyes after 14 to 21 days. Histopathologic and ultrastructural features of ear

[Results of pseudophakic retinal detachment surgery].

Between 8/81 and 9/89 2658 patients underwent retinal detachment surgery at the university eye hospital Erlangen/Nürnberg. From this number there were 30 (= 1.1%) pseudophakic retinal detachments consisting of 14 males and 16 females. Minimal age was 48, maximal 89 years, average age was 70.9 years. There were 4479 extracapsular cataract extractions with implantation of a posterior chamber lens performed in the same interval at our eye hospital. Out of this 4479 extracapsular cataract extractions we found 9 patients (= 0.2%) with pseudophakic retinal detachment. In our investigated group we fo

Treatment of macular hole retinal detachment.

Seven patients with macular hole retinal detachment were treated by intravitreal gas injection with or without release of subretinal fluid. Macular buckling, diathermy, cryopexy, or vitrectomy were not used. The patients were placed prone for eight hours a day until the gas had absorbed. In five of the seven patients the retina became reattached within three days and remained reattached with follow-up periods of three to 22 months (average nine months). It is believed that such detachments are due to vitreoretinal traction and the intravitreal gas bubble relieves this traction. This technique

Retinal detachment in penetrating keratoplasty patients.

We reviewed the records of 23 patients who had retinal detachment after penetrating keratoplasty. Seventeen retinas (74%) were reattached successfully. Of the six failures, four patients had hemorrhagic choroidal detachment at the time of keratoplasty. When these complex retinal detachments are subtracted, 17 of 19 patients (89%) had successful retinal reattachments. Of the 17 successes, only seven patients had visual acuity of 20/200 or better. Retinal detachments after penetrating keratoplasty can be repaired with a high rate of success, but visual results remain disappointing.

New retinal detachment after pneumatic retinopexy.

Thirteen cases of rhegmatogenous retinal detachment repaired by pneumatic retinopexy were reviewed retrospectively. All patients were followed for a minimum of 6 months after surgery. At the end of follow-up, retinal detachment repair by pneumatic retinopexy succeeded in nine cases (70%) and failed in four (30%). Within 2 days of pneumatic retinopexy, vitreous condensation and traction with new retinal tears and associated detachments in previously uninvolved quadrants developed in two patients. Traumatic retinal detachment--mechanisms and management.

Experience has shown that the overwhelming majority of traumatic retinal detachments are rhegmatogenous in origin. Retinal breaks are predominantly located within the vitreous base region but may occur at sites of focal scleral impact or from posterior vitreous avulsion. Although the use of scleral buckling techniques alone may be sufficient, closed microsurgery may be required to relieve trans-gel or surface retinal traction and to facilitate the identification and permanent closure of retinal breaks. Prophylactic measures including the use of closed microsurgery play a vital role in the mana

Syphilitic retinal detachment and uveal effusion.

A 32-year-old man developed a recurrent nonrhegmatogenous retinal detachment, uveal effusion, and visual loss as a result of latent secondary syphilis. Treponemes were found in the subretinal fluid and the Treponema pallidum hemagglutination test demonstrated substantially higher titers in the subretinal fluid than in the serum (1: 2,569 vs 1: 16). Despite scleral dissection and a scleral implant and treatment with penicillin, the patient's visual loss persisted and the last examination showed a thickened choroid, a flat nonrhegmatogenous retinal detachment, and reaccumulation of subretinal fl
