In serological studies blood groups of intervillous thrombi of the placenta were identical with maternal blood groups in 88.9%. Identity with fetal blood groups occurred in 11.1%. Utilizing an elution technique in smears prepared from intervillous thrombi, 11.68% of the erythrocytes were of fetal, 88.32% of maternal origin. In conclusion, intervillous thrombi are mainly composed of maternal cells. The existence of small amounts of fetal cells simultaneously, points to the possible etiology: Local damage of the trophoblastic epithelium of the villi is followed by escape of fetal cells into the

Being able to detect fetal abnormalities that may be associated with hydramnios would be extremely useful, especially when diabetes mellitus, Rh isoimmunization, and multiple pregnancy are ruled out. For this purpose the new technique of fetography, consisting of injecting a small amount of 2 radioque media (liposoluble and hydrosoluble), was used. Four out of 6 fetuses were correctly predicted to be abnormal. They were 1 case of esophageal atresia, 1 of suspicious chromosomal abnormality (after birth it was confirmed as having the Smith-Lemli-Opitz syndrome), and 2 of trisomy 18. It is felt t

OBJECTIVE: This study aims at determining a cutoff value differentiating the fetal from the adult coenzyme Q10 (CoQ10) values and comparing substantial increases in CoQ10 plasma levels in fetuses with hypoxic hypoxia and nonimmune fetal hydrops.
METHODS: We have selected 61 pregnancies and determined the CoQ10 levels in fetal and maternal samples obtained by cordocentesis. Our study included a control group and pregnancies with intrauterine growth retardation, Rh isoimmunization, nonimmune fetal hydrops, and fetal malformations.
RESULTS: To differentiate the fetal from the adult values we ha

BACKGROUND: Hemolytic disease of the fetus and newborn (HDFN) is a condition due to maternal blood group antibodies targeting antigens in fetal red blood cells, with significant prenatal/perinatal morbidity and mortality. Severe HDFN cases are often associated with alloimmunization against Rhesus D (RhD) or Kell antigens. Information about HDFN epidemiology and treatment in Latin American countries is limited. This review aims to identify and synthesize the available evidence on the epidemiology and management of HDFN in this region.
METHODS: In July 2023, EMBASE, PubMed, LILACS, and other da

Rh is a complex blood group system with diverse genotypes that may encode weak and partial D variants. Standard serologic analysis may identify clinically significant D variants as D+; nevertheless, individuals with these D variants should be managed as D- patients to prevent antibody formation to absent D epitopes. Variant identification is necessary during pregnancy to allow for timely and appropriate Rh immune globulin (RhIG) prophylaxis for hemolytic disease of the fetus and newborn (HDFN) as D alloimmunization can occur with some D variants. Here, we describe two cases of the RHD*DAU5 all

BACKGROUND: Transfusion of D+ red blood cells (RBCs) to D- recipients can be accidental or necessary due to D- RBC shortage. Alloimmunization can complicate future transfusions; implications for women of childbearing age are compounded by possible hemolytic disease of the fetus and newborn. Rh immunoprophylaxis is effective, and indicated, for preventing alloimmunization. Reports of massive D+ mismatch (e.g., in the case of fetal-maternal bleed) are limited, and standard recommendations for managing these rare events are lacking. The cases discussed herein of women of childbearing age who suff

Although anti-RhD was once the major etiology of hemolytic disease of the fetus/newborn (HDFN), the widespread adoption of antenatal and postpartum Rhesus immune globulin has resulted in a marked decrease in the prevalence of alloimmunization to the RhD antigen in pregnancy. Maternal alloimmunization to other red cell antigens continues to play a role as the cause of fetal disease since no prophylactic immune globulins are available to prevent the formation of these antibodies. An increasing incidence of the Kell (anti-K1) antibody has been noted in the United States. Guidelines for interventi

Despite the time elapsed since their development, and substantial human and economical efforts searching for alternatives, platelet transfusion, continues to be the main therapeutic measure available for the management of patients suffering from quantitative and qualitative platelet disorders. However, there are still aspects of their use that are not completely established. One of these is ABO and/or Rh(D) compatibility. A major ABO group incompatibility appears to decrease the response to a platelet transfusion, whereas a minor ABO incompatibility can be associated with, sometimes lethal, ac

The D-- phenotype is an exceptionally rare Rh blood group phenotype characterized by expression of D in the complete absence of C, c, E, and e. Individuals with this phenotype are at risk of developing anti-Rh17 (anti-Hr0), a clinically significant alloantibody that can severely complicate transfusion support and pregnancy management because compatible blood is exceptionally difficult to obtain. This review critically synthesizes the available published evidence regarding the D-- phenotype and anti-Rh17 alloimmunization, integrating data from case reports, case series, family investigations, a

Fetomaternal haemorrhage (FMH) in RhD-negative individuals can lead to alloimmunization with future antibody-mediated destruction of fetal red blood cells. Accurate estimation of FMH is essential for guiding the dose of Rh immune globulin and mitigating alloimmunization. We conducted a national survey and performed a scoping review to determine the availability, technical characteristics and clinical limitations of FMH tests. We describe the evolution of FMH testing, including the qualitative methods (alpha-fetoprotein level, microscopic weak D test, enzyme-linked antiglobulin test, rosette te

The Rh blood group system was last reviewed in Immunohematology in 2010 (Chou ST, Westhoff CM. The Rh and RhAG blood group systems. Immunohematology 2010;26:178-86). This update focuses on RHD, RhD structure, alterations in D expression, anti-D alloimmunization, and applications of RHD genotyping for weak and discrepant D phenotypes; identification of RHD genotypes that encode partial D phenotypes; and prevention and management of anti-D in pregnancy. Updates to the RHAG system and to RHCE and its encoded antigens are in recent or upcoming publications of Immunohematology, respectively.

IMPORTANCE: Rhesus alloimmunization refers to the sensitization of an Rh D-negative mother after exposure to D-positive fetal red blood cells, which can lead to significant fetal and neonatal morbidity and mortality.
OBJECTIVE: The aim of this study was to review and compare the most recently published international guidelines on the prevention of maternal alloimmunization.
EVIDENCE ACQUISITION: A comparative review of guidelines from the American College of Obstetricians and Gynecologists, the British Committee for Standards in Hematology, the International Federation of Gynecology and Obst

PURPOSE: The pharmacologic properties of Rhesus (Rh) immune globulin (RhIG) and clinical data on its effectiveness in preventing Rh-antigen alloimmunization in pregnant women are reviewed.
SUMMARY: RhIG is a human plasma derivative that targets red blood cells (RBCs) positive for Rh(O) antigen (also called D antigen). In the United States and other countries, the widespread use of RhIG has markedly reduced the occurrence of hemolytic disease of the fetus and newborn (HDFN), a devastating condition caused by D-antigen sensitization of a pregnant woman via exposure to fetal RBCs (usually during

Twenty cases of nonimmunologic hydrops fetalis were reviewed. The incidence of nonimmunologic hydrops fetalis was 1/2,029 (20 cases in 40,588 deliveries). The diverse etiologies of nonimmunologic hydrops fetalis are demonstrated. The incidence of erythroblastosis fetalis caused by Rh isoimmunization declined markedly. The perinatal mortality rate was 14/18 or 78%. Prematurity, the presence of congenital anomalies, and the severity of hydrops fetalis contribute to this poor prognosis. However, a better understanding of the pathophysiology of hydrops fetalis, along with early detection by ultras

BACKGROUND: Despite the introduction of anti-D prophylaxis into clinical practice, RhD alloimmunization still presents a problem to date. The actual incidence of RhD alloimmunization in pregnant women remains unknown in most countries. Anti-D immunoglobulin is administered to RhD negative women at a fixed dose and in much greater amounts than is actually necessary. On the other hand, it is not possible to diagnose cases where greater doses are needed. To optimize the prevention of RhD alloimmunization in RhD negative women, it is important to diagnose conditions that lead to fetomaternal hemor
