Lessons to learn from the analysis of routine health data from Moria Refugee Camp on Lesvos, Greece.

BACKGROUND: Refugees in humanitarian settings commonly experience many health needs and barriers to access healthcare; health data from these settings are infrequently reported, preventing effective healthcare provision. This report describes health needs of refugees in Moria Camp on Lesvos, Greece-Europe's largest refugee camp. METHODS: A set of routinely collected service data of 18 131 consultations of 11 938 patients, attending a primary care clinic in the camp over 6 months in 2019-20, was analysed retrospectively, focusing on chronic health conditions. RESULTS: The most frequent chroni

Understanding and improving the quality of primary care for people in prison: a mixed-methods study.

BACKGROUND: People in prison are generally in poorer health than their peers in the community, often living with chronic illness and multimorbidity. Healthcare research in prisons has largely focused on specific problems, such as substance use; less attention has been paid to conditions routinely managed in primary care, such as diabetes or hypertension. It is important to understand how primary care in prisons is currently delivered in the United Kingdom and how it can be improved, in order to reduce health inequalities. OBJECTIVE: To understand the quality of primary care in prison, includi

Methodologies to measure the coverage of vitamin A supplementation: a systematic review.

Countries are increasingly transitioning from event-based vitamin A supplementation (VAS) distribution to delivery through routine health system contacts, shifting also to administrative, electronic-based monitoring tools, a process that brings certain limitations affecting the quality of administrative VAS coverage. At present, there is no standardised methodology for measuring the coverage of VAS delivered through routine health services. To address this gap, we conducted a systematic review of the literature to identify and recommend methods to measure VAS coverage, with the aim of providin

Beyond the dataset: integrating public voices in data science.

The digitalisation of health data has helped drive initiatives like the Scottish Collaborative Optometry-Ophthalmology Network eResearch (SCONe), which links retinal images from community optometry practices with other routinely collected health data to enhance disease detection. As data-driven approaches expand throughout the healthcare system, patient and public involvement and engagement (PPIE) is increasingly recognised as essential for improving the quality, relevance, and acceptability of health research. However, despite growing endorsement, challenges remain, including inconsistent ter

Obstetrics and childbirth at the Aitape Health Centre, Papua New Guinea.

Routinely collected obstetric data at the Aitape Health Centre, Papua New Guinea over the years 1986, 1988 and 1989 were analyzed and compared with figures from western New Guinea of about thirty years ago and with those of rural African hospitals. Data from 924 deliveries were analyzed. 30% were born before arrival (BBA). The number of abnormal deliveries was high. This was in part due to the high frequency of twin births (4.5%). Among the BBAs puerperal sepsis and stillbirth were significantly higher than among those who delivered in the health centre. The stillbirth frequency was 3.4%. The

To adopt or adapt an existing neonatal core outcome set in Kenya: a study protocol.

BACKGROUND: Development and use of core outcome set(s) (COS) in research can reduce research wastage by ensuring that a minimum set of outcomes are always reported on. Neonatal morbidity and mortality are a big burden in low- and middle-income countries (LMICs). Research is continuously being undertaken to reduce this burden. Currently, there is no COS for neonatal research in LMICs but there exists one for neonatal research in high-income countries (HICs). OBJECTIVES: To determine outcomes that are useful for neonatal care in Kenya to inform whether an existing COS should be adopted or adapt

Development and validation of a novel model for characterizing migraine outcomes within real-world data.

BACKGROUND: In disease areas with 'soft' outcomes (i.e., the subjective aspects of a medical condition or its management) such as migraine or depression, extraction and validation of real-world evidence (RWE) from electronic health records (EHRs) and other routinely collected data can be challenging due to how the data are collected and recorded. In this study, we aimed to define and validate a scalable framework model to measure outcomes of migraine treatment and prevention by use of artificial intelligence (AI) algorithms within EHR data. METHODS: Headache specialists defined descriptive fe

Vaccination Capability Inventory of Community, Migrant, and Homeless Health Centers: A Survey Report.

CONTEXT: Federally funded Community, Migrant, and Homeless Health Centers provide health services to the most vulnerable communities in the United States. However, little is known about their capabilities and processes for providing vaccinations to adults. PROGRAM: We conducted the first national survey of health centers assessing their inventory, workflow, capacity for, and barriers to provision of routinely recommended adult vaccines. In addition, we asked health center leaders' perceptions regarding best practices and policy recommendations for adult vaccinations. IMPLEMENTATION: A survey

Health Risk and Underweight.

UNLABELLED: Anorexia nervosa is associated with a significant risk of morbidity and mortality. In clinical practice, health risk is assessed and estimated using routinely collected laboratory data. This study will develop a risk score using clinically relevant laboratory parameters. The related question is how to estimate the health risk associated with underweight using body weight, height and age. METHODS: We used routinely collected laboratory parameters from a total of 4087 patients. The risk score was calculated on the basis of electrolytes, blood count, transaminases and LDH. The nine p

Electronic health records-based research in Cardiology: The time has come for pragmatic trials.

Randomized clinical trials (RCTs) are the cornerstone of evidence-based medicine, as they minimize bias in the allocation of interventions. However, RCTs performed in a very selective population and overcontrolled conditions may impair the generalizability of results. Moreover, increasing running costs and regulatory complexity compromise the conduct of these studies. The need for pragmatic trial designs, with streamlined procedures and low running costs, will shape the short-term future of research in RCTs. Electronic health records (EHR) are routinely collected as part of the treatment of pa

Undernutrition in Acquired Brain Injury Rehabilitation: A Retrospective GLIM Exploration.

BACKGROUND: Neuroscience nurses play a pivotal role in identifying nutritional risk factors, monitoring patients, and initiating interventions to optimize recovery outcomes. Patients with moderate to severe acquired brain injury (ABI) are at risk of undernutrition during subacute rehabilitation, yet prevalence is rarely described using recent diagnostic criteria. This study aimed to explore the Global Leadership Initiative on Malnutrition (GLIM) criteria in routinely collected clinical data and to describe undernutrition in patients with ABI at admission and discharge from in-hospital rehabili

Monitoring vs. modeled exposure data in time-series studies of ambient air pollution and acute health outcomes.

BACKGROUND: Population-based short-term air pollution health studies often have limited spatiotemporally representative exposure data, leading to concerns of exposure measurement error. OBJECTIVE: To compare the use of monitoring and modeled exposure metrics in time-series analyses of air pollution and cardiorespiratory emergency department (ED) visits. METHODS: We obtained daily counts of ED visits for Atlanta, GA during 2009-2013. We leveraged daily ZIP code level concentration estimates for eight pollutants from nine exposure metrics. Metrics included central monitor (CM), monitor-based (

How far back do we need to look to capture diagnoses in electronic health records? A retrospective observational study of hospital electronic health record data.

OBJECTIVES: Analysis of routinely collected electronic health data is a key tool for long-term condition research and practice for hospitalised patients. This requires accurate and complete ascertainment of a broad range of diagnoses, something not always recorded on an admission document at a single point in time. This study aimed to ascertain how far back in time electronic hospital records need to be interrogated to capture long-term condition diagnoses. DESIGN: Retrospective observational study of routinely collected hospital electronic health record data. SETTING: Queen Elizabeth Hospit

Prediction of pre-eclampsia with machine learning approaches: Leveraging important information from routinely collected data.

BACKGROUND: Globally, pre-eclampsia (PE) is a leading cause of maternal and perinatal morbidity and mortality. PE prediction using routinely collected data has the advantage of being widely applicable, particularly in low-resource settings. Early intervention for high-risk women might reduce PE incidence and related complications. We aimed to replicate our machine learning (ML) published work predicting another maternal condition (gestational diabetes) to (1) predict PE using routine health data, (2) identify the optimal ML model, and (3) compare it with logistic regression approach. METHODS:

Strengthening geospatial surveillance for malaria control and elimination in Indonesia: a meeting report.

As Indonesia progresses toward malaria elimination, geospatial analysis offers opportunities to strengthen surveillance, improve intervention targeting, and support data-informed decision-making. However, despite increasing availability of georeferenced surveillance data, the routine use of geographic information systems (GIS) within malaria programs remains uneven. A three-day consultation and GIS training workshop was conducted in Jakarta, bringing together 25 representatives from National and Sub-National Malaria Programs and other disease control units. The workshop included hands-on R-bas
