Severe acromioclavicular joint osteoarthritis is associated with acromial stress fractures after reverse shoulder arthroplasty.

BACKGROUND: Little is known about the role of disorders of the acromioclavicular joint (ACJ) and how they relate to complications after reverse shoulder arthroplasty (RSA). The purpose of this study is to compare the severity of ACJ osteoarthritis in patients undergoing RSA with and without postoperative acromial and scapular spine fractures. METHODS: A retrospective review was performed to identify all patients who underwent primary RSA between 1/1/2007 and 10/31/2019 with a postoperative acromial or scapular spine stress fracture from a single institution. Patients who underwent RSA with a

Angular stable plate versus reverse shoulder arthroplasty for proximal humeral fractures in elderly patient.

BACKGROUND: Treatment of complex proximal humeral fractures in the elderly is a challenge and reverse shoulder arthroplasty (RTSA) is now an important alternative to open reduction internal fixation (ORIF) with angular stable plate. The purpose of this study is to compare clinical and radiological outcomes of RTSA and ORIF in the elderly. METHODS: We retrospectively analyzed patients treated for three- or four-part displaced fractures of the proximal humerus. Range of motion, disabilities of the arm, shoulder and hand (DASH) and Constant scores were recorded. X-ray exam in three projections c

Reverse total shoulder arhroplasty for the treatment of failed fixation in proximal humeral fractures.

BACKGROUND: This study evaluates the clinical outcomes in patients with proximal humerus fractures (PHF) treated with reverse total shoulder arthroplasty (RTSA) as a revision procedure for failed fixation that have more than 5 years of follow-up. MATERIALS AND METHODS: This is a retrospective study of 270 RTSA. The inclusion criteria for this study consisted of patients initially treated with either open reduction and plate fixation (ORIF) or fixation using an intramedullary nail (IMN), who were managed with a single-stage revision to a RTSA, and had a minimum of 24 months clinical and radiol

Increasing Use of Reverse Total Shoulder Arthroplasty for Proximal Humerus Fractures in Elderly Patients.

This study described surgical treatment patterns for proximal humerus fractures among elderly patients, focusing on reverse total shoulder arthroplasty (TSA), and evaluated how the type of fixation affects inpatient factors (cost, length of stay), transfusion rates, and patient disposition (home vs skilled nursing facility). With Nationwide Inpatient Sample data from 2011 to 2013, the authors identified patients 65 years and older who had proximal humerus fractures and divided them into 3 groups: (1) open reduction and internal fixation (ORIF); (2) hemiarthroplasty; and (3) reverse TSA. From 2

Hemiarthroplasty versus reverse shoulder arthroplasty for treatment of proximal humeral fractures: a meta-analysis.

BACKGROUND: We performed a meta-analysis of studies with at least Level IV evidence to compare outcomes between hemiarthroplasty and reverse shoulder arthroplasty for the treatment of proximal humeral fractures. METHODS: Three electronic databases (PubMed, Cochrane, and EMBASE) were searched. The quality of each study was investigated, and data on radiographic and functional outcomes were extracted and analyzed. RESULTS: The analysis included 1 Level I study, 1 Level II study, 3 Level III studies, and 2 Level IV studies. Reverse shoulder arthroplasty was more favorable than hemiarthroplasty

Outcome of shoulder hemiarthroplasty in acute proximal humeral fractures: a frustrating meta-analysis experience.

Limited evidence is available on the optimal treatment of complex fractures of the proximal humerus. No randomized prospective studies have compared hemiarthroplasty, open reduction internal fixation and/or conservative treatment. Two systematic reviews are available but they do not include angle-stable plate osteosynthesis or third-generation prosthetic designs. We conducted a systematic review of studies published in the last 10 years. The MOOSE guidelines were followed and we focused on functional outcome and its relation to age, gender and tuberosity healing after hemiarthroplasty. Sixteen

[Primary fracture protheses and reverse shoulder arthroplasty in complex humeral head fractures : An alternative to joint-preserving osteosynthesis?].

BACKGROUND: The surgical management of complex humeral head fractures has adapted dynamically over the course of the last decade. The primary use of reverse shoulder arthroplasty in elderly patients has gained in relevance due to promising short and middle-term results. Long-term results, however, are still pending. The appliance of anatomical hemiarthroplasty, on the other hand, has lost in significance in favour of osteosynthesis and reverse shoulder arthroplasty. INDICATIONS: This review article follows the question as to under which circumstances primary fracture arthroplasty reflects an

Reverse total shoulder arthroplasty for type I fracture sequelae after internal fixation of proximal humerus fractures.

INTRODUCTION: Complications after internal fixation of proximal humerus fracture are common and may require surgical revision. Reverse total shoulder arthroplasty (RTSA) is frequently performed in such cases. The aim of the present study was to evaluate the functional results and complications after RTSA for the treatment of type I fracture sequelae after internal fixation of proximal humerus fractures. MATERIALS AND METHODS: 26 patients (18 female, 8 male) underwent surgical revision of type I fracture sequelae of the proximal humerus after locking plate (n = 22) or intramedullary nail (n =

Does excessive lateralization influence tuberosity healing in reverse total shoulder arthroplasty for proximal humeral fractures?

BACKGROUND: This study evaluated the influence of stem global lateralization on tuberosity consolidation and functional outcomes in patients undergoing reverse total shoulder arthroplasty (rTSA) for proximal humeral fractures (PHFs). METHODS: A retrospective, two-institution study included patients who underwent rTSA for acute PHFs implanted with one of 3 stems: the Arrow (global lateralization 21.4 mm), the Humelock II (11.2 mm), or the Unique (14.7 mm). Stems were classified as very highly lateralized (VHL; >20 mm, Arrow) or lateralized (L; 10-15 mm, Unique and Humelock II). Tuberosity cons

Effect of tuberosity repair on functional outcome of reverse shoulder arthroplasty in proximal humerus fractures.

PURPOSE: Unsatisfactory results of hemiarthroplasty in Neer's 3- and 4-part proximal humerus fractures in elderly, have led to the shift towards reverse shoulder arthroplasty (RSA). The objective of our study was to repair the tuberosities that are generally overlooked during RSA and observe its impact on the functional outcome and shoulder scores. METHODS: We include elderly patients with acutely displaced or dislocated 3- or 4-part proximal humerus fractures from July 2013 to November 2019 who were treated with RSA along with tuberosity repair by non-absorbable sutures and bone grafting har

Radiographic and anatomic variations on postoperative acromion fractures after inlay and lateralized reverse shoulder arthroplasty.

INTRODUCTION: Postoperative fracture of the acromion is a complication uniquely more common after reverse shoulder arthroplasty (RSA) than other forms of shoulder arthroplasty. There is limited knowledge regarding the etiology of these fractures or the anatomic risk factors. The purpose of this study is to identify associations of the acromioclavicular (AC) joint and relative humeral and glenoid positioning on the occurrence of acromial fractures after RSA. METHODS: A retrospective case-controlled study was performed on primary RSA patients treated by a single surgeon from September 2009 to S

Functional outcomes of operative fixation of clavicle fractures in patients with floating shoulder girdle injuries.

BACKGROUND: Double disruptions of the superior suspensory shoulder complex, commonly referred to as 'floating shoulder' injuries, are ipsilateral midshaft clavicular and scapular neck/body fractures with a loss of bony attachment of the glenoid. The treatment of 'floating shoulder' injuries has been debated controversially for many years. The purpose of this study was to demonstrate the clinical and functional outcomes of patients with 'floating shoulder' injuries who underwent operative fixation of the clavicle fracture only. MATERIALS AND METHODS: Between 2002 and 2010, 32 consecutive float

Shoulder geometry after reverse total shoulder arthroplasty with a medialized glenoid and a lateralized humerus predicts subacromial notching and acromial or scapular spine fractures.

BACKGROUND: The primary purpose of this study was to assess whether the difference between the distance from the acromion to the glenosphere center of rotation (DA) and the distance from the greater tuberosity to the glenosphere center (DGT) influences the incidence of subacromial notching (SaN) in shoulders following reverse total shoulder arthroplasty (rTSA) with a medialized glenoid and a lateralized humerus. The secondary purpose was to evaluate whether this relationship also impacts the incidence of acromion or scapular spine fractures. METHODS: We conducted a retrospective cohort study
