Looking into the joint when it is frozen: A report on dynamic shoulder ultrasound.

Frozen shoulder is a common disorder characterized by pain and limitation of the glenohumeral joint motions. While it is usually clinically diagnosed, as it can also mimic several other painful shoulder pathologies, imaging techniques can contribute to the differential diagnosis. In the literature, static and dynamic ultrasonographic findings have been reported to better characterize this condition and plan for treatment. We studied a 50-year-old male patient with capsular stiffness of the shoulder on whom Ultrasound (US) examination was performed. Our study exemplifies another interesting ult

Managing Bony Defects of the Shoulder Joint that Occur in Association with Dislocation.

BACKGROUND: Defects to the articular surface of the humeral head have been known to be associated with shoulder dislocation since the 19th century. It wasn't until 1934 that the first description of the ubiquitous compression fracture of the posterolateral humeral head that occurs with traumatic anterior instability appeared. From 1940, this defect became referred to as a Hill-Sachs lesion after the investigators who reported the condition. The significance of, and therefore treatment of, these and other such bony defects around the shoulder joint has been hotly debated. METHODS: We reviewed

Joint Kinetics to Assess the Influence of the Racket on a Tennis Player's Shoulder.

This study aimed at investigating the influence of three rackets on shoulder net joint moments, power and muscle activity during the flat tennis serve under field- conditions. A 6-camera Eagle(®) motion analysis system, operating at 256 Hz, captured racket and dominant upper limb kinematics of the serve in five tennis players under three racket conditions (A: low mass, high balance and polar moment, B: low three moments of inertia, and C: high mass, swingweight and twistweight). The electromyographic activity of six trunk and arm muscles was simultaneously recorded. Shoulder net joint moments

In vivo measurement of shoulder joint loads during walking with crutches.

BACKGROUND: Following surgery or injury of the lower limbs, the use of walking aids like crutches can cause high loads on the shoulder joint. These loads have been calculated so far with computer models but with strongly varying results. METHODS: Shoulder joint forces and moments were measured during crutch-assisted walking with complete and partial unloading of the lower limbs. Using telemeterized implants in 6 subjects axillary crutches and forearm crutches were compared. A force direction was more in the direction of the long humeral axis, and slightly lower forces were assumed using axill

[Cementless humeral head replacement for dislocation arthropathy of the shoulder joint].

BACKGROUND: Only a few reports in the literature describe that total shoulder replacement provides better functional results in dislocation arthropathy than hemiarthroplasty. Nevertheless, the risk of aseptic loosening in young and active patients is high after long-term follow-up. We evaluated the results of cementless humeral head resurfacing in this group. METHODS: We evaluated 29 patients with dislocation arthropathy who had been followed up for a mean of 39 months. The mean age at the time of operation was 43.7±14.7 years. Preoperatively, cuff deficiency was seen in 11 cases. RESULTS: T

Surgical normalization of the shoulder joint in obstetric brachial plexus injury.

Obstetric brachial plexus injuries (OBPI) result in bony derangements that include posterior humeral head subluxation, glenoid retroversion, and joint incongruity. Often these deformities are accompanied by scapular hypoplasia, elevation, and rotation, which further exacerbate shoulder dysfunction. The purpose of the current study was to investigate the effects of triangle tilt surgery on glenohumeral joint anatomy in 100 OBPI patients. The triangle tilt surgery restores the distal acromioclavicular triangle from an abnormal superiorly angled position to a neutral position, thereby restoring n

Anterosuperior dislocation of the shoulder joint in an older patient with Parkinson's disease.

An 83-year-old woman, subsequently diagnosed with Parkinson's disease, fell on her right shoulder. Radiographs showed the humeral head dislocated anterosuperiorly. The supraspinatus tendon was massively disrupted and judged irreparable. The subscapularis tendon was repaired, however re-dislocation of the humeral head was confirmed after the surgery but further treatment was rejected as she was almost pain free and had a low activity level. The patient was followed up for one year after surgery and her shoulder became almost pain-free, but range of motion with active elevation was limited. In s

[Current treatment concepts in first-time dislocation of the shoulder joint].

The rapid development of arthroscopic techniques has raised questions on the treatment of first shoulder dislocations. Primary arthroscopic or even open labral repair have been recommended. However, the available prospective studies demonstrate a recurrence rate of not more than 50% in the global population. This number increases steeply if certain risk factors are present: young age (under 25 years), shoulder sports, adequate trauma with acute unidirectional instability. For this selected group, arthroscopic labral repair should be proposed and should be performed by a surgeon specifically tr

Impingement of the rotator cuff in athletes caused by instability of the shoulder joint.

An impingement of the rotator cuff can be caused by chronic anterior instability of the shoulder joint. This particular disease is often found in athletes engaged in overhead motion in abduction/external rotation of the arm, such as in ball sports like volleyball or European handball, racket sports like tennis or badminton, or swimming. For those patients that cannot be cured by conservative treatment such as muscular stabilization, surgical treatment is indicated: anterior reconstruction of the capsule and/or the glenoid labrum, and in addition--if necessary--subacromial decompression and rev

[Amputation and arthrodesis of the shoulder joint after brachial plexus lesions].

Nine patients with severe irreversible brachial plexus lesions were submitted to arthrodesis of the shoulder joint and/or amputation of the forearm, the latter with the object of fitting an artificial limb. In one patient, the arm was re-implanted and biceps function of strength M2 was obtained. Three patients were submitted to microsurgical interventions on the plexus. All of the patients were visited in their homes after an average period of observation of three years (1-8 years). Seven patients were equipped with prostheses, three of these with myoelectric prostheses. One patient employed a

[Possibilities of magnetic resonance tomography in diagnostic imaging of the shoulder joint].

By virtue of its multiplanar representation, magnetic resonance imaging (MRI) allows clear visualization of the complex anatomical relationships of the shoulder joint. In addition to axial planes, slices perpendicular and parallel to the glenoid cavity are used to good advantage. In tears of the rotator cuff an increase in signal intensity within the cuff is recognized in T2- and proton-density-weighted images. Lesions of the glenoid labrum following luxations of the glenohumeral joint can be detected and classified using MRI. The diagnostic value of MRI as compared with other imaging modaliti

[Post-traumatic pain syndromes of the shoulder joint--differentiation and treatment].

The main symptom of posttraumatic trouble in the shoulder-joint is the painful immobility. We had to differentiate between genuine shoulder pain originating in shoulder alternation and shoulder pain in consequence of other diseases. Significant causes of posttraumatic shoulder pain are the impingement syndrome with tendon alteration or lesion or, due to fractures, the instable shoulder joint as well as shoulder rigidity. An exact diagnosis and, in consequence an exact treatment can only be accomplished basing on accurate history and specific clinical examination.

[Posterior shoulder joint instability. Classification, pathomechanism, diagnosis, conservative and surgical management].

The posterior instability of the shoulder is a more difficult diagnostic and therapeutic challenge than the anterior instability. There are many etiologies and causes of posterior instability. Most studies in the literature are retrospective and yield a great variation in therapeutic recommendations. Generally it has to be separated in traumatic and atraumatic instabilities. Most of the traumatic dislocations are impaction fractures of the humeral head against the dorsal glenoid. Therapy is depending on the size of the humeral defect, the duration of dislocation and the functional demand of th

Synovial Fluid Fatty Acid Profiles Are Differently Altered by Inflammatory Joint Pathologies in the Shoulder and Knee Joints.

Anomalies of fatty acid (FA) metabolism characterize osteoarthritis (OA) and rheumatoid arthritis (RA) in the knee joint. No previous study has investigated the synovial fluid (SF) FA manifestations in these aging-related inflammatory diseases in the shoulder. The present experiment compared the FA alterations between the shoulder and knee joints in patients with end-stage OA or end-stage RA. SF samples were collected during glenohumeral or knee joint surgery from trauma controls and from OA and RA patients (n = 42). The FA composition of SF total lipids was analyzed by gas chromatography with
