Botulinum toxin type a blocks aquaporin 5 trafficking by decreasing synaptosomal-associated protein 23 in submandibular acinar cells.

The trafficking of aquaporin 5 (AQP5) is critical for salivary secretion. Synaptosomal-associated protein 23 (SNAP23) is an important regulator in the process of membrane fusion. However, the role of SNAP23 on AQP5 trafficking has not been explored. Botulinum toxin type A (BoNT/A) is a bacterial toxin that effectively treats sialorrhea. We previously reported that BoNT/A induced AQP5 redistribution in cultured acinar cells, but the mechanism remained unclear. In this study, SNAP23 was predominantly localized to the plasma membrane of acinar cells in the rat submandibular gland (SMG) and coloca

Transdermal Scopolamine Withdrawal Syndrome Case Report in the Pediatric Cerebral Palsy Population.

Sialorrhea in children with cerebral palsy (CP) results in aspiration, decreased social integration, and poor quality of life. Management options include transdermal anticholinergics such as the scopolamine patch. A controlled clinical trial has proven botulinum toxin (BTX) injections into the salivary glands are an effective alternative to transdermal anticholinergics with a safer side effect profile. Multiple studies of the injections in diverse populations demonstrate reduction in saliva production with improvement in quality of life and decrease in hospitalization-associated costs. The aut

Nonmotor symptoms groups in Parkinson's disease patients: results of a pilot, exploratory study.

Nonmotor symptoms (NMS) like neuropsychiatric symptoms, sleep disturbances or autonomic symptoms are a common feature of Parkinson's disease (PD). To explore the existence of groups of NMS and to relate them to PD characteristics, 71 idiopathic non-demented PD out-patients were recruited. Sleep was evaluated by the PD Sleep Scale (PDSS). Several neuropsychiatric, gastrointestinal and urogenital symptoms were obtained from the NMSQuest. Sialorrhea or dysphagia severity was obtained from the Unified PD Rating Scale activities of daily living section. MADRS depression scale was also administered.

Ultra-Rapid Cycling Bipolar Disorder: A Case Report.

Ultra-rapid cycling disorder, in which mood episodes shift over days rather than months, sits at one of the most difficult ends of the bipolar spectrum. It resists standard treatment, tends to worsen with antidepressants, and has very little long-term outcome data to guide clinical decisions. We report the case of a 51-year-old woman whose ultra-rapid cycling bipolar disorder was tracked for 130 weeks. She underwent trials of multiple mood stabilizers, five second-generation antipsychotics, and two courses of electroconvulsive therapy (ECT), none of which produced sustained remission. Clozapin

Localization of Salivary Glands for Botulinum Toxin Treatment: Ultrasound Versus Landmark Guidance.

BACKGROUND: Sialorrhea is a troublesome symptom in a variety of neurological diseases. Recently, local injection of botulinum toxin into the salivary glands was approved for treatment of sialorrhea, and injection guidance by anatomical landmarks was suggested. OBJECTIVE: To compare the accuracy of ultrasound versus previously proposed anatomical landmarks for localizing the salivary glands. METHODS: In a cross-sectional study in 21 adults, landmark positions (LM) of the parotid gland (PG) and the submandibular gland (SG) were identified following published recommendations. The ultrasound pos

Use of botulinum toxin in pediatric otolaryngology and laryngology.

The use of botulinum toxin in adult otolaryngology has been commonly used in conditions such as spasmodic dysphonia, cricopharyngeal spasm, palatal myoclonus, sialorrhea, and for cosmetic reasons. The current use of botulinum toxin in pediatric otolaryngology and laryngology has primarily been off label and in children older than 2 years of age. This review discusses the different applications of botulinum toxin in pediatric patients and its effectiveness in treating different pediatric conditions.

[Surgical treatment of salivary incontinence in cerebral palsy].

For patients with cerebral palsy, chronic drooling is the result of an abnormality in the cerebral control of orofacial motor functions. Sialorrhea is a troublesome problem for these patients and their attendants. Among various surgical techniques, rerouting of the Stensen's ducts and bilateral excision of the submandibular salivary glands, as described by Wilkie, gives good results. Transposition of the submandibular gland ducts in the tonsillar fossae is an easier procedure but less favorable. We present 19 patients treated by these two methods.

Ranulas--mucoceles of the oral cavity: experience in 26 children.

Ranulas or mucoceles of the floor of the mouth are uncommon but do occur in approximately 5% of patients undergoing submandibular duct relocation for the management of uncontrollable sialorrhea. Review of our experience with 26 patients over 12 years ending in 1986 substantiated a concept previously advocated by other authors that ranulas are usually extravasation pseudocysts developing after disruption of sublingual gland elements, and that excision of the ipsilateral sublingual gland is the management approach of choice.

Exploring the relationship between sleep bruxism and saliva: A systematic review and meta-analysis.

Sleep bruxism (SB) is defined as rhythmic masticatory muscle activity occurring during sleep. Evidence suggests that SB may be associated with alterations in salivary function; however, findings remain inconsistent across age groups. This study aimed to evaluate the relationship between SB and salivary flow and composition. A systematic search of PubMed®, Embase® and Web of Science was conducted to identify human studies reporting SB and saliva-related parameters. Twenty studies involving 4,543 participants were included. Risk of bias was assessed using the Risk of Bias Assessment tool for Non
