Management of Traumatic Anterior Skull Base Fractures and Cerebrospinal Fluid Fistulas.

OBJECTIVES: Blunt trauma and falls from heights are the primary causes of skull base fractures, which comprise ∼20% of all skull fractures. One-third of these fractures lead to a cerebrospinal fluid (CSF) fistula, typically managed conservatively and without surgery. AIMS: The authors analyzed the factors affecting the formation and closure time of CSF fistulas. METHODS: The authors followed up 31 patients with traumatic anterior skull base fractures (ASBF) between November 15, 2020 and October 10, 2023. The clinical characteristics, tomography data, and occurrence of CSF fistulas were docum

Understanding Bilateral Skull Fractures in Infancy: A Retrospective Multicenter Case Review.

BACKGROUND: Bilateral skull fractures in infancy often raise suspicion for abuse. Nevertheless, literature suggests that they may occur accidentally. However, empiric data are lacking. OBJECTIVE: This multicenter retrospective review aimed to characterize bilateral skull fractures in a large sample. PARTICIPANTS AND SETTING: Medical records for infants younger than 24 months with bilateral skull fractures involving hospital consultation with a child abuse pediatrician (CAP) were reviewed from 2005 to 2020 at 13 nationally represented institutions. METHODS: Standardized data collection acros

The role of computed tomography in following up pediatric skull fractures.

BACKGROUND: Despite the added radiation exposure and costs, the role of computed tomography (CT) in following pediatric skull fractures has not been fully evaluated. METHODS: We reviewed the radiology reports and images of the initial and follow-up head CT examinations of children with skull fractures to determine whether any interval changes in the fracture morphology and associated complications necessitate a change in clinical management. RESULTS: A total of 316 pediatric cases of skull fractures were identified, including 172 patients with and 144 without follow-up scans. At follow-up, 7

A history of depressed skull fractures from ancient times to 1800.

The story of managing depressed fractures illustrates how knowledge of proven value does not always get handed down. Celsus was the first to describe sensible management for depressed fractures. As he wrote in Latin this was forgotten. Galen's Greek writings survived forming the basis of management until the sixteenth century. In 1517, Hans von Gersdorff published a formidable illustrated surgical text. One illustration depicts an instrument for elevating depressed bone fragments. It looked dramatic but could not work and its defects were finally defined in the eighteenth century. Ambroise Par

Isolated skull fractures: trends in management in US pediatric emergency departments.

STUDY OBJECTIVE: Previous studies have suggested that children with isolated skull fractures are at low risk of requiring neurosurgical intervention, suggesting that admission to the hospital may not be necessary in many instances. We seek to evaluate current practice for children presenting to the emergency department (ED) for isolated skull fractures in US children's hospitals. METHODS: We conducted a retrospective multicenter cross-sectional study of children younger 19 years with a diagnosis of isolated skull fracture who were evaluated in the ED from 2005 to 2011, using the Pediatric Hea

Treatment of depressed skull fractures in neonates using percutaneous microscrew elevation.

Depressed skull fractures in neonates involve the inward buckling of the skull bones to resemble a cup shape or "ping-pong ball" shape. In the past, several methods have been used to treat these fractures for cosmetic effect, including the use of vacuum extraction. The aim of this study was to demonstrate the use of a novel treatment method for these skull fractures. All patients presented to Hershey Medical Center between May 2007 and March 2010. All underwent treatment of their ping-pong skull fracture using a novel technique of percutaneous screw elevation with self-tapping microscrews (4 o

[Angiography of the carotid artery in centro-lateral skull base fractures?].

BACKGROUND: In latero-basal, central or spheno-fronto-orbital skull base fractures the internal carotid artery is more frequently involved in severe lesions than expected. PATIENTS AND METHODS: Between 1996 and 2003 we examined 684 patients with Glasgow Coma Scales (GCS) between 2 and 15, median 7.2, using computed tomography (CT). In suspicion of a latero-basal, central or spheno-fronto-orbital fractur they got an additional high resolution skull base CT. If the bony canal of the internal carotid artery (ICA) was involved the patient underwent digital subtraction angiography (DSA). The ICA l

Cerebrospinal fluid leak following traumatic skull base fractures: Predictors and outcomes.

BACKGROUND: Traumatic skull base fractures carry a risk of cerebrospinal fluid (CSF) leak, predisposing patients to meningitis, neurological complications, and prolonged hospitalization. Predictive factors for posttraumatic CSF leak remain incompletely defined. The primary objective of this study is to identify clinical and radiological predictors of CSF leak among patients with traumatic skull base fractures and evaluate associated outcomes at a Level-1 trauma center. METHODS: A retrospective cohort study was conducted at King Abdulaziz Medical City, Riyadh, including all patients with compu

Depressed "ping pong" skull fractures in the newborns: A cohort study.

BACKGROUND AND AIMS: A ping pong fracture is a rare depressed skull fracture (DSF) observed in infants. It occurs due to the inward buckling of the calvarium, creating a cup-like shape. Trauma during childbirth, particularly from instrumental delivery or the application of pressure by physicians or midwives during challenging deliveries, is the primary cause. This study aimed to investigate the epidemiologic characteristics associated with DSF in newborns and to identify the main factors related to its incidence and the type of hematoma involved. METHODS: This is a retrospective case-control

Pediatric skull fractures: could suture contact be a sign of abuse?

PURPOSE: Skull fractures in infants and young children can occur as a result of both accidental trauma and abuse. 1/3 of children with abuse-related head trauma and 1/5 of children with abuse-related fractures were overlooked during the initial evaluation. In this study, we aim to investigate the prevalence of skull fractures that come into contact with the suture in head traumas caused by accidents and abuse, and also to see if contact of the fracture line with the suture could be used as a sign for abuse in the pediatric population. METHODS: Forry-four patients with head trauma were retrosp

Management of Isolated Skull Fractures in Pediatric Patients: A Systematic Review.

OBJECTIVES: Isolated skull fractures (ISFs) in children are one of the most common emergency department injuries. Recent studies suggest these children may be safely discharged following ED evaluation with little risk of delayed neurological compromise. The aim of this study was to propose an evidence-based protocol for the management of ISF in children in an effort to reduce medically unnecessary hospital admissions. METHODS: Using PubMed and The Cochrane Library databases, a literature search using the search terms (pediatric OR child) AND skull fracture AND (isolated OR linear) was perform
