Continuous polygraphic recordings of six patients with vascular brain stem lesions were evaluated to investigate the relationship between 1) the localization or extent of the lesions and the alteration of the sleep-waking cycle, and 2) the evolution of the clinical condition and the sleep-waking cycle. Absent REM sleep and severely disorganized NREM sleep were associated with extensive pontine lesions involving the pontine tegmentum. In "locked-in" syndrome, the neural mechanisms responsible for both REM and NREM sleep continued to function, but the sleep-waking cycle was disturbed and marked

During non-REM sleep the EEG shows characteristics waves that are generated by the dynamic interactions between cortical and thalamic oscillators. In thalamic neurons, low-threshold T-type Ca(2+) channels play a pivotal role in almost every type of neuronal oscillations, including slow (< 1 Hz) waves, sleep spindles and delta waves. The transient opening of T channels gives rise to the low threshold spikes (LTSs), and associated high frequency bursts of action potentials, that are characteristically present during sleep spindles and delta waves, whereas the persistent opening of a small fracti

Suicidal patients often report problems with their sleep. Although sleep-related complaints and EEG (electroencephalographic) changes have been seen widely across the spectrum of psychiatric disorders, sleep complaints such as insomnia, hypersomnia, nightmares, and sleep panic attacks are more common in suicidal patients. The subjective quality of sleep as measured by self-rated questionnaires also appears to be more disturbed in suicidal depressive patients. Sleep studies have reported various polysomnographic findings including increased REM (rapid eye movement) time and REM activity in suic

Alzheimer's disease (AD) is a leading cause of mortality and morbidity among aging populations worldwide. Despite arduous research efforts, treatment options for this devastating neurodegenerative disease are limited. Sleep disturbances, through their link to changes in neural excitability and impaired clearance of interstitial abnormal protein aggregates, are a key risk factor for the development of AD. Research also suggests that the neuroprotective effects of sleep are particularly active during slow wave sleep. Given the strong link between sleep disturbance and AD, targeting sleep in the

To clarify the role of growth hormone-releasing hormone (GHRH) in the regulation of the episodic growth hormone (GH) secretion which is known to occur constantly in the initial slow wave stage of nocturnal sleep in man, we studied the relation between the secretions of plasma immunoreactive(IR)-GHRH and GH while recording electroencephalograms. In subjects who showed a normal sleep pattern, the plasma IR-GHRH level increased 3- to 4-fold just before the surge of plasma GH, suggesting that GH release in the initial slow wave stage of sleep is mainly mediated by GHRH. However, when there was an

OBJECTIVE: To describe the alterations in the macrostructure of sleep in a large cohort of sleep-disturbed patients with Parkinson's disease (PD) and investigate influencing factors.
METHODS: A cohort of sleep-disturbed but otherwise unselected PD patients (n=351) was investigated with video-supported polysomnography. We analyzed the influence of age, disease duration, disease severity, and dopaminergic medication on subjective sleep perception, sleep efficiency, the amount of slow wave sleep, awakenings, periodic leg movements in sleep (PLMS), and REM sleep behavior disorder (RBD).

STUDY OBJECTIVES: The severity of obstructive sleep apnea (OSA) is known to vary according to sleep stage; however, the pathophysiology responsible for this robust observation is incompletely understood. The objective of the present work was to examine how ventilatory control system sensitivity (i.e. loop gain) varies during sleep in patients with OSA.
METHODS: Loop gain was estimated using signals collected from standard diagnostic polysomnographic recordings performed in 44 patients with OSA. Loop gain measurements associated with nonrapid eye movement (NREM) stage 2 (N2), stage 3 (N3), and

Electroencephalography (EEG) is an important clinical tool for reviewing sleep-wake cycling in neonates in intensive care. Tracé alternant (TA)-a characteristic pattern of EEG activity during quiet sleep in term neonates-is defined by alternating periods of short-duration, high-voltage activity (bursts) separated by lower-voltage activity (inter-bursts). This study presents a novel approach for detecting TA activity by first detecting the inter-bursts and then processing the temporal map of the bursts and inter-bursts. EEG recordings from 72 healthy term neonates were used to develop and evalu

STUDY OBJECTIVES: This study examined whether basal electromyographic (EMG) amplitude during sleep is altered in patients with isolated REM sleep behavior disorder (iRBD) and its relationship to disease progression, as well as changes across the sleep cycle.
METHODS: Baseline polysomnograms collected from 95 iRBD patients (15 converters, 80 non-converters) and matched 24 controls were analyzed. EMG amplitude was calculated from submentalis recordings across all sleep stages using 3-s mini-epochs, with REM sleep further classified as REM sleep without atonia (RWA) and non-RWA (RA) according to

OBJECTIVES: The physiologic relationship between slow-wave activity (SWA) (0-4 Hz) on the electroencephalogram (EEG) and high-frequency (0.1-0.4 Hz) cardiopulmonary coupling (CPC) derived from electrocardiogram (ECG) sleep spectrograms is not known. Because high-frequency CPC appears to be a biomarker of stable sleep, we tested the hypothesis that that slow-wave EEG power would show a relatively fixed-time relationship to periods of high-frequency CPC. Furthermore, we speculated that this correlation would be independent of conventional nonrapid eye movement (NREM) sleep stages.

Given the critical role of sleep, particularly sleep slow oscillations, sleep spindles, and hippocampal sharp wave ripples, in memory consolidation, sleep enhancement represents a key opportunity to improve cognitive performance. Techniques such as transcranial electrical and magnetic stimulation and acoustic stimulation can enhance slow oscillations and sleep spindles and potentially improve memory. Targeted memory reactivation in sleep may enhance or stabilize memory consolidation. Each technique has technical considerations that may limit its broader clinical application. Therefore, neurost

Morphine effects on many neural and behavioral measures, including tests of learning and memory, are attenuated by increased circulating glucose levels. Using systemic injections, we investigated the ability of glucose to attenuate sleep deficits induced by morphine administration in the rat. Morphine at 1 mg/kg produced a moderate decrease in slow wave sleep which was prevented by concomitant administration of 100 mg/kg of glucose. A higher dose of morphine (10 mg/kg) severely delayed the onset of both slow wave sleep and REM sleep. These delays were attenuated by concurrent administration of

Background: Sleep-dependent glymphatic clearance has emerged as a potential mechanism linking sleep disruption with Alzheimer's Disease (AD) pathology. However, the relationship between objectively measured sleep and glymphatic function across the AD continuum remains unclear. Methods: Four databases (PubMed, Embase, Cochrane Library, and PsycINFO) were systematically searched for studies assessing objective sleep metrics and glymphatic-related biomarkers or clearance measures in humans across the AD continuum. Following peer review of the search strategy, supplementary searches of PubMed and

In juvenile neuronal ceroid-lipofuscinosis (JNCL), sleep disorders are common. The purpose of this study was to investigate the sleep structure of 28 patients with JNCL compared with healthy controls subjects and to clarify the pathophysiology underlying the sleep disturbances in these patients. Each of 28 patients with JNCL (age range = 6-27 years), with or without sleep complaints, underwent one night of polysomnography. Electroencephalographic, electro-oculographic, electromyographic, and electrocardiographic findings were recorded. Sleep was scored and analyzed visually. The sleep paramete

Converging evidence suggests that sleep disturbances can directly contribute to a transdiagnostic combination of behavior and neurocognitive difficulties characterizing most forms of psychopathology. However, it remains unclear how the growing comprehension of sleep neurophysiology should best inform sleep quality assessment in mental health patients. To address this fundamental question, we performed deep multimodal sleep and behavioral phenotyping in 37 individuals at high genetic risk for psychopathology due to 22q11.2 Deletion Syndrome (Mean age:19±8.17, M/F = 22/15) and 34 Healthy Control

Tourette syndrome (TS) patients often complain of sleep problems, and questionnaire studies indicate that sleep disturbance is frequent. Decreased slow wave sleep and increased awakenings have been reported in laboratory polysomnography in TS patients, and a serotoninergic disorder of arousal has been postulated. We recorded outpatient sleep in 20 patients newly diagnosed with TS utilizing a 4-channel cassette EEG system. The newly-diagnosed patients were predominantly male, and ranged in age from 10 to 36 years. Some had taken psychotropic medications in the past, but none had been treated sy

Memory consolidation during sleep is thought to depend on the coordinated interplay between cortical slow waves, thalamocortical sleep spindles and hippocampal ripples, but direct evidence is lacking. Here, we implemented real-time closed-loop deep brain stimulation in human prefrontal cortex during sleep and tested its effects on sleep electrophysiology and on overnight consolidation of declarative memory. Synchronizing the stimulation to the active phases of endogenous slow waves in the medial temporal lobe (MTL) enhanced sleep spindles, boosted locking of brain-wide neural spiking activity

The propensity for narcolepsy, a clinical sleep disorder of unknown etiology, is virtually totally included within the HLA-DR2,DQw1 (DRw15,DQw6) phenotype. The disorder is characterized by decreased sleep latency, early onset of rapid eye movement sleep, and a paucity of nocturnal slow-wave sleep. Muramyl peptides, naturally occurring bacterial cell wall peptidoglycans, potently enhance the duration and amplitude of slow-wave sleep in animals, bind to murine mononuclear cells, and exhibit a major histocompatibility complex-restricted immunoadjuvant effect in mice. We examined the binding of mu
