Deciphering the mechanisms: Pathophysiology of migraine-related cognitive dysfunction.

Migraine is increasingly understood as a disorder of brain network dysfunction, where attack-related cognitive symptoms (attention deficits, slowed processing speed and executive dysfunction) can be as disabling as pain and may persist into the interictal period. Such symptoms are associated with functional and structural changes across the migraine cycle, involving the prefrontal cortex, thalamus, hypothalamus, hippocampus and cerebellum. Interictal deficits in working memory, visuospatial processing, verbal fluency and executive function are also documented. Rodent models show impairments in

Psychopathological intersection between obsessive-compulsive disorder and post-traumatic stress disorder: scoping review of similarities and differences.

INTRODUCTION: Although post-traumatic stress disorder and obsessive-compulsive disorder have distinct diagnostic criteria, some psychopathological phenomena seem to be shared, which may lead to misdiagnosis and erroneous treatment decisions. This scoping review explores the psychopathological similarities and differences between these two disorders. METHOD: The review complies with the recommendations of the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) and included articles published in Portuguese, English, or Spanish in the last 50 years indexed in the PubMed d

Optimizing single-chamber pacing in dogs Part 1: Rate determinations, rate interventions and hysteresis.

Determining ideal pacing rates to meet physiological needs and optimizing programming to prevent unnecessary right ventricular pacing in dogs requires an understanding of heart rate profiles and applicable pacing technology. The heart rate and rhythm of the dog is complex necessitating investigation of rate requirements of activity and circadian influences. Overlaying this information are a multiplicity of other factors such as age, breed, temperament, cardiovascular disease and underlining rhythm disorders that contribute to the difficulty in making general conclusions. However, all such info

Lingual muscle activity across sleep-wake States in rats with surgically altered upper airway.

Obstructive sleep apnea (OSA) patients have increased upper airway muscle activity, including such lingual muscles as the genioglossus (GG), geniohyoid (GH), and hyoglossus (HG). This adaptation partially protects their upper airway against obstructions. Rodents are used to study the central neural control of sleep and breathing but they do not naturally exhibit OSA. We investigated whether, in chronically instrumented, behaving rats, disconnecting the GH and HG muscles from the hyoid (H) apparatus would result in a compensatory increase of other upper airway muscle activity (electromyogram, E

The interplay between chronobiology, neuroinflammation, and the glymphatic system in neurodegenerative diseases.

Neurodegenerative diseases are traditionally characterized by the progressive accumulation of misfolded proteins and chronic neuroinflammation. However, emerging evidence indicates that the pathogenesis of these disorders is increasingly recognized to involve the disruption of temporal homeostasis. This conceptual review examines the highly interconnected, bidirectional crosstalk between the molecular circadian clock, glial-mediated neuroimmunity, and the macroscopic glymphatic clearance system. Under physiological conditions, the intrinsic circadian machinery helps maintain microglia in a qui

Head-Up Tilt Sleeping to Treat Orthostatic Intolerance in a Patient with Advanced Parkinson's Disease: A Case Report.

INTRODUCTION: Orthostatic hypotension is common in people with Parkinson's disease (PD) due to autonomic dysfunction and medication use and can have a significant negative impact on quality of life. Pharmacological treatment is often complicated due to complex blood pressure regulation problems. This case report presents a patient whose symptoms of orthostatic intolerance were successfully treated with the non-pharmacological method of head-up tilt sleeping (HUTS). CASE PRESENTATION: A 69-year-old man with PD and prominent autonomic failure received recommendation from the neurologist to use

Pupillary response to blue light as a biomarker of seasonal pattern in Major Depressive Episode: A clinical study using pupillometry.

Depressive disorders are characterized by disturbances in light signal processing. More specifically, an alteration of the melanopsin response is suggested. The post-illumination pupillary response (PIPR) to blue light (post-blue PIPR) is increasingly used as a marker of the activity of intrinsically photosensitive melanopsin ganglion cells (ipRGCs). We hypothesized that individuals with Major Depressive Episode (MDE) who exhibited a higher vulnerability to season patterns showed a decreased ability to transmit light signals to the brain. We explored the correlation between the post-blue PIPR

Objectively measured sleep characteristics among early-middle-aged adults: the CARDIA study.

Despite mounting evidence that sleep duration is a risk factor across diverse health and functional domains, little is known about the distribution and determinants of sleep. In 2003-2004, the authors used wrist activity monitoring and sleep logs to measure time in bed, sleep latency (time required to fall asleep), sleep duration, and sleep efficiency (percentage of time in bed spent sleeping) over 3 days for 669 participants at one of the four sites of the Coronary Artery Risk Development in Young Adults (CARDIA) study (Chicago, Illinois). Participants were aged 38-50 years, 58% were women, a

Benchmark Algorithm for Surface Electromyography-Based Periodic Limb Movement Detection.

Periodic limb movement disorder (PLMD) is a sleep disorder characterized by repetitive involuntary leg movements (LMs) during sleep called periodic limb movements (PLMs). In clinical settings, PLMs are detected through a two-stage procedure comprising LM detection and PLM classification based on surface electromyography (sEMG) recordings from the bilateral tibialis anterior. Although there is a scoring manual for LM detection and PLM classification, the strict interpretation of this may result in the underestimation of LMs due to the lack of intrinsic sEMG characteristics such as rapid amplitu

Sex, stress and sleep apnoea: Decreased susceptibility to upper airway muscle dysfunction following intermittent hypoxia in females.

Obstructive sleep apnoea syndrome (OSAS) is a devastating respiratory control disorder more common in men than women. The reasons for the sex difference in prevalence are multifactorial, but are partly attributable to protective effects of oestrogen. Indeed, OSAS prevalence increases in post-menopausal women. OSAS is characterized by repeated occlusions of the pharyngeal airway during sleep. Dysfunction of the upper airway muscles controlling airway calibre and collapsibility is implicated in the pathophysiology of OSAS, and sex differences in the neuro-mechanical control of upper airway paten

Altered Spontaneous Brain Activity Related to Neurologic and Sleep Dysfunction in Children With Obstructive Sleep Apnea Syndrome.

Childhood obstructive sleep apnea (OSA) is a common chronic sleep-related breathing disorder in children, which leads to growth retardation, neurocognitive impairments, and serious complications. Considering the previous studies about brain structural abnormalities in OSA, in the present study, we aimed to explore the altered spontaneous brain activity among OSA patients, using amplitude of low-frequency fluctuation (ALFF), fractional ALFF (fALFF), and regional homogeneity (ReHo) methods based on resting-state functional magnetic resonance imaging (MRI). Thirty-one untreated OSA children and 3

Paradoxical function of orexin/hypocretin circuits in a mouse model of Huntington's disease.

Huntington's disease (HD) is a neurodegenerative disorder involving progressive motor disturbances, cognitive decline, and desynchronized sleep-wake rhythms. Recent studies revealed that restoring normal sleep-wake cycles can improve cognitive function in HD mice, suggesting that some sleep/wake systems remain operational and thus represent potential therapeutic targets for HD. Hypothalamic neurons expressing orexins/hypocretins (orexin neurons) are fundamental orchestrators of arousal in mammals, but it is unclear whether orexin circuits operate normally in HD. Here we analyzed the electrophy

Collapsibility of passive pharynx in patients with acromegaly.

Sleep-disordered breathing (SDB), either central or obstructive in nature, is common in patients with acromegaly. However, no study has systematically examined the collapsibility of the pharynx in acromegaly to date. We evaluated intrinsic mechanical properties of passive pharynx in 10 anesthetized and paralyzed patients with active acromegaly before transsphenoidal adenomectomy for their pituitary adenoma. Static pressure-area relationships of the velopharynx and oropharynx were obtained by step changes in airway pressure during endoscopic cross-sectional area measurement of each segment. Mor

The role of retinal regulation of sleep in health and disease.

The process of photoentrainment, through the activation of photoreceptor transduction cascades, influences the circadian physiology of many life forms from primitive invertebrates to primates. In humans, a population of intrinsically photosensitive retinal ganglion cells (ipRGC's) is responsible for mediating the circadian rhythm and is susceptible to primary dysfunction affecting this cell population specifically, or disorders influencing light activation of retinal ganglion photoreceptors. The former may arise through cell depletion in conditions such as inherited or acquired optic neuropath

High-resolution daily profiles of tissue adrenal steroids by portable automated collection.

Rhythms are intrinsic to endocrine systems, and disruption of these hormone oscillations occurs at very early stages of the disease. Because adrenal hormones are secreted with both circadian and ultradian periods, conventional single-time point measurements provide limited information about rhythmicity and, crucially, do not provide information during sleep, when many hormones fluctuate from nadir to peak concentrations. If blood sampling is attempted overnight, then this necessitates admission to a clinical research unit, can be stressful, and disturbs sleep. To overcome this problem and to m

Central periodic breathing during sleep in 74 patients with acute ischemic stroke - neurogenic and cardiogenic factors.

OBJECTIVES: The aims of our study were 1) to better characterize central periodic breathing during sleep (CPBS) and its clinical relevance in acute stroke, 2) to better define the role of brain damage in its pathogenesis. METHODS: We included 74 consecutive patients admitted within 96 hours after stroke onset. Stroke severity at admission, stroke outcome at discharge and stroke topography were assessed. ECG and transesophageal echocardiography were performed. Nocturnal breathing was assessed with an ambulatory device the first night after admission. CPBS severity was represented as absolute t

[Effect of posterior nasal neurectomy combined with the inferior turbinate surgery for the patients with perennial allergic rhinitis or vasomotor rhinitis].

INTRODUCTION: Until recently, Vidian neurectomy had been applied mainly in intractable vasomotor rhinitis and severe perennial allergic rhinitis. Although the results were excellent, the operation has not been applied recently because of the adverse events such as xerophthalmia and trigeminal neuralgia. To resolve these problems, a new surgical technique, posterior nasal neurectomy, was developed. In this report, we examined the effectiveness of posterior nasal neurectomy combined with the inferior turbinate surgery for severe perennial allergic rhinitis and intrinsic rhinitis by questionnaire

Acetylcholine and hallucinations: disease-related compared to drug-induced alterations in human consciousness.

Newly proposed criteria for Lewy body dementia include alterations in consciousness. Lewy body dementia is also associated with a disturbance in cholinergic transmission; neocortical cholinergic deficits in this disorder are more extensive than in Alzheimer's disease and are correlated with symptoms commonly associated with delirium, such as visual hallucinations. The traditional view that derangements of the basal forebrain cholinergic system in Alzheimer's disease relate specifically to memory impairment is assessed in terms of a more general role for cortical acetylcholine in consciousness.

Towards the discovery of novel T-type calcium channel blockers.

Despite their presence in many tissues and their potential implication in various disease states, low-voltage activated T-type calcium channels (T-channels) have only recently become targets of interest. Unfortunately, the lack of selective T-channel blockers has hampered further characterisation of these channels. The recent availability of cloned T-channels, the Ca(V)3 proteins, facilitates identification of novel T-channel blockers. Also, studies performed in knockout animals have fostered novel interest. Selective inhibition of T-channels may have clinical importance in cardiovascular dise
