PURPOSE: The physical compatibility of telavancin with select i.v. drugs during simulated Y-site administration was evaluated.
METHODS: Telavancin for injection was reconstituted according to manufacturer's recommendations and diluted with 0.9% sodium chloride injection, 5% dextrose injection, or lactated Ringer's injection to a concentration of 7.5 mg/mL. Two elderly diabetic patients with abdominal pain were demonstrated to have complications of phenformin hydrochloride therapy. The first developed severe lactic acidosis treated with sodium bicarbonate given intravenously and followed by rebound alkalosis. The second showed severe acidosis (specimens for lactate determination were unfortunately unsatisfactory for analysis) and similar alkalotic rebound after therapy. Sodium dichloroacetate (DCA) is an investigational drug that shows promise in the treatment of acquired and congenital mitochondrial diseases, including myocardial ischemia and failure. Sodium nitroprusside was found to be a very strong inhibitor of the glucose formation from lactate in the perfused guinea pig liver. Hepatic cell respiration and oxidative phosphorylation appeared to be simultaneously suppressed by this compound. The pattern of some hepatic metabolite concentrations was similarly affected by nitroprusside as described for biguanides and pyridinium chloride derivatives. Cyanide had to be added to the perfusion medium in a tenfold higher concentration to achieve a comparable inhibition of gluconeogenesis from lactate.

Severe lactate acidosis developed in nine diabetics on biguanide. When lactate acidosis was diagnosed all patients had reduced renal function, six being oligoanuric. Pre-existing chronic renal failure as a factor in the development of lactic acidosis was excluded in five patients, normal renal function being restored later. BACKGROUND: Fluid resuscitation with normal saline (NS) can aggravate lactate production. The objective of this study was to evaluate the efficacy of small-volume resuscitation using 3% hypertonic sodium chloride (HS) and its comparison with NS in trauma patients. The primary endpoint was an increase in lactate clearance after 1 hr of fluid resuscitation.
