Diagnosis and misdiagnosis of necrotizing soft tissue infections: three case reports.

BACKGROUND: Today, gas gangrene is rare, but still many of the patients die, despite having received timely treatment. CASE PRESENTATION: This report highlights the cases of three different patients, who were transferred to our surgical department in 2006. The first patient (Patient_A), with the suspected diagnosis "femoral hematoma", a second patient (Patient_B) because of an "acute abdomen" and the third patient (Patient_C) with suspected gas gangrene of the right leg. CONCLUSION: The first two cases demonstrate gas gangrene should always be kept in mind, especially in high-risk-patients.

[Necrotizing skin and soft tissue infections: definitions, clinical and microbiological features].

Among the three types of soft tissue and skins infections (cellulitis, necrotising cellulitis and necrotising fasciitis) only the necrotising injuries are commonly managed in intensive care unit. Location, depth and severity of these potentially life threatening infections may vary and cannot be predicted on clinical grounds No microbiological deduction can be made from clinical signs. A mixed aerobic-anaerobic flora is cultured in 40 to 80% of the cases. Magnetic resonance imaging and CT-scan are the best techniques to assess the diagnosis and determine the extension of infection. However, th

Severe group a streptococcal soft-tissue infections in Ontario: 1992-1996.

A prospective, population-based, surveillance study of invasive soft-tissue infections due to group A streptococci was conducted in Ontario, Canada, from 1992 through 1996. Demographic and clinical information was obtained by patient interview and chart review. Isolates were characterized by M protein and T agglutination typing. The incidence of necrotizing fasciitis (NF) increased from 0.08 cases per 100,000 population in 1992 to 0.49 cases per 100,000 population in 1995. The case-fatality rate was 13% (68 of 520 patients died). Hypotension and multiorgan dysfunction complicated 64 cases (12%

Hyperbaric oxygen in the treatment of life-threatening soft-tissue infections.

Necrotizing soft-tissue infections are rapidly spreading bacterial infections that account for a relatively small proportion of infections, but are aggressive in nature and nearly uniformly fatal if left untreated. Prompt recognition, antibiotic therapy, aggressive surgical debridement, and hyperbaric oxygen (HBO) therapy have reduced the mortality resulting from these infections. Oxygen, at increased pressures, augments tissue oxygen partial pressure, allowing increased bacterial killing by providing substrate for the formation of oxygen free radicals and augmenting respiratory burst. Diabetic Ketoacidosis and Necrotizing Soft Tissue Infection.

AUDIENCE: Emergency medicine (EM) residents at all levels of education and medical students on EM rotation. INTRODUCTION: Diabetes is a chronic disease diagnosed in over 28 million people in the United States which causes serious acute complications and is responsible for more than two million ED visits per year.1,2 Diabetic ketoacidosis (DKA) is one of the most serious complications of diabetes; it is diagnosed with the triad of hyperglycemia, anion gap metabolic acidosis, and ketonemia. The most common cause of DKA is infection, but it can also be precipitated by medication noncompliance, c

Ureaplasma urealyticum necrotizing soft tissue infection.

Ureaplasma urealyticum is a urogenital commensal and often associated with localised infection. With the advent of monoclonal immunosuppressive therapy and improved diagnostic methods, reports of non-urogenital infections are accumulating. We report a rare case of U. urealyticum necrotizing soft tissue infection and left hip septic arthritis in a hypogammaglobulinaemic patient. Consideration of this organism as an etiological agent, and potential early use of nucleic-acid diagnostic investigation with empiric therapy including activity against Ureaplasma in this patient population may be warra

Extensive necrotizing soft tissue infection of the perineum.

Synergistic necrotizing fasciitis of the penis and scrotum was first described by Fournier, and remains a rare, but life-threatening disease. The reported mortality rates in different series range between 22 and 60%. We present a study of 20 patients treated over a 9 year period by early aggressive excision. Four of the treated patients (20%) died. A high index of suspicion, combined with early radical surgical intervention, is the key to effective treatment. Delayed diagnosis and treatment were the primary factors responsible for death.

Soft tissue infection in children in south Trinidad.

A review of 391 children in the first decade of life admitted to a surgical ward with soft tissue infections revealed predisposing factors in 38.4% of cases, mainly trauma and adjacent skin sepsis. Superficial abscess formation occurred in 90% of cases, muscle abscesses in 9.5%, and necrotizing fasciitis in less than 1%. The most frequent organism isolated was the staphylococcus aureus which was found in 52.8% of the available cultures. Coliform organisms were found in 14.6%, and haemolytic streptococci in just over 10%. A study of the culture and sensitivity profiles obtained made it possible

The diabetic foot. Soft tissue and bone infection.

Diabetic patients, as a consequence of various neurologic, vascular, and metabolic perturbations, are at high risk for developing infections of the soft tissue and bones of the feet. The microbial etiology of soft tissue in infections is best determined by cultures of a tissue curetting or aspiration, rather than a swab. Aerobic gram-positive cocci are the major pathogens in diabetic foot infections; these may be the sole isolate(s) in acute uncomplicated infections, but they are usually accompanied by aerobic gram-negative bacilli or anaerobes in chronic or previously treated infections. Soft-tissue infection in lower extremity trauma.

Soft-tissue infection following lower extremity trauma has not been studied in detail in light of recent data on biology of infection. This article examines specific problems in lower extremity trauma that allow the wound to become susceptible to wound infection. It also illustrates the various principles of wound management in lower extremity trauma that serve to prevent infection. Two case examples are used to illustrate principles of management. Other wound problems in lower extremity trauma are also discussed, such as rabies, necrotizing soft-tissue infection, tetanus, and diabetic foot in

Clinical pathways of hospitalized extremity soft-tissue infections: a retrospective study.

BACKGROUND: Bacterial infections of the skin and soft tissues (SSTIs) are common and should be considered a key differential diagnosis in patients presenting with redness and swelling in these areas. They range from mild superficial cases to severe, complicated forms requiring prompt antimicrobial therapy and, often, surgical intervention. We aimed to describe clinical pathways, diagnostic evolution, treatment strategies, and outcomes in a hospitalized orthopedic/trauma surgery cohort. METHODS: We performed a retrospective single-center analysis of all inpatients treated for extremity SSTIs b

Fluorescence-Guided Surgery for Necrotizing Soft Tissue Infections: A Systematic Review.

Necrotizing soft tissue infections are life-threatening infections of soft tissue that are treated with rapid surgical debridement when the diagnosis is suspected. Necrotic tissue is removed until vital tissue is identified, guided by the surgeon's vision and hands. Fluorescence-guided surgery is increasingly popular for assessing tissue perfusion. This surgery may have the potential to improve the diagnosis and treatment of necrotizing soft tissue infections. A systematic review on fluorescence-guided surgery for necrotizing soft tissue infections was conducted according to the PRISMA guideli

Skin and soft tissue infections: swab or biopsy? A diagnostic dilemma.

PURPOSE OF REVIEW: Skin and soft tissue infections (SSTIs) are frequent clinical conditions in which microbiological investigation is selectively indicated. When an etiological diagnosis is pursued, the optimal sampling method - superficial swab vs. deep tissue biopsy or aspirate - remains controversial. This review critically examines the diagnostic performance, clinical impact, and limitations of different sampling strategies in SSTIs. RECENT FINDINGS: Evidence consistently shows that deep tissue biopsy or aspirate provides a higher microbiological yield and better correlation with clinical

Determining the optimal antibiotic duration for skin and soft tissue infections.

PURPOSE OF REVIEW: Optimal duration of therapy in SSTIs - a heterogeneous group of infections - remains unknown. The advances in knowledge of antibiotic duration of treatment in selected SSTIs that can impact clinical practice and published in the last 18 months are reviewed. RECENT FINDINGS: Recent evidence indicates that few patients receive guideline concordant empiric antibiotics and appropriate duration in the United States, although this likely can be extrapolated to other countries. One of the most commonly identified opportunities to improve antibiotic stewardship is duration of thera

Reported outcome measures in necrotising soft tissue infections: a systematic review.

INTRODUCTION: There are inconsistencies in outcome reporting for patients with necrotising soft tissue infections (NSTI). The aim of this study was to evaluate reported outcome measures in NSTI literature that could inform a core outcome set (COS) such as could be used in a study of hyperbaric oxygen in this indication. METHODS: A systematic review of all NSTI literature identified from Cochrane, Ovid MEDLINE and Scopus databases as well as grey literature sources OpenGrey and the New York Academy of Medicine databases which met inclusion criteria and were published between 2010 and 2020 was

Necrotizing Soft Tissue Infections in South Australia: A 15-Year Review.

Necrotizing soft tissue infections (NSTIs) are associated with high morbidity and mortality. We retrospectively examined the impact of empiric antimicrobials coupled with early surgery on mortality in NSTI. Early surgery independently reduced 30-day mortality (odds ratio, .16; 95% confidence interval, .05-.51; P < .001) that was not further augmented by empiric antimicrobial choice.

[Practicability of the German guidelines on skin and soft tissue infections].

The guidelines on calculated parenteral initial treatment of bacterial infections in adults from 2018 were the first German language S2k guidelines for these infections. This article summarizes the experiences with respect to their practicality in the clinical routine and the resulting supplementations and comments. In view of the many different terms for soft tissue infections, the guidelines had to first establish some definitions and diagnostic criteria. Among others, the guidelines introduced the provisional term limited phlegmons (phlegmons are usually termed cellulitis in Angloamerican l

Diagnosis of skin and soft tissue infections using near-infrared spectroscopy.

AIM: Skin and soft tissue infections are classified into cellulitis and necrotizing fasciitis, which are difficult to distinguish. Necrotizing fasciitis has a poor prognosis and requires immediate intensive care. The diagnostic gold standard is to incise the lesion to determine whether necrosis has reached the fascia. We aimed to show that these infections can be differentiated using near-infrared spectroscopy. METHODS: We describe two cases in an observational study about the utility of near-infrared spectroscopy. Case 1 involved a 77-year-old man with a chief complaint of pain, redness, and

Treatment of multidrug-resistant Gram-negative skin and soft tissue infections.

PURPOSE OF REVIEW: The increase in skin and soft tissue infections (SSTI) because of multidrug-resistant (MDR) pathogens is a global concern. Although MDR Gram-negative bacteria (GNB) are often overlooked as a cause of SSTIs, their burden on the morbidity of many subgroups of patients is high. There is a paucity in the available treatment options and guidelines on how to treat these pathogens. This manuscript reviews the management of SSTIs caused by carbapenem-resistant Enterobacteriaceae (CRE), Pseudomonas aeruginosa (CRPA), Acinetobacter baumannii (CRAB), and Stenotrophomonas maltophilia. Fungal osteomyelitis and soft tissue infections: Simple solutions to uncommon scenarios.

INTRODUCTION: Fungal osteoarticular/soft tissue infections (FOaSI) are an uncommon entity with protracted course due to variability in clinical picture, slow progression; resulting in misdiagnosis with empirical therapy. Recent studies have shown an alarming emergence of FOaSI in immunocompetent individuals with high mortality rates. This study recommends a protocol for managing these complex and confusing scenarios. METHODOLOGY: We have retrospectively analysed patients with FOaSI between January 2014 and December 2016, with a minimum 12 months follow up. RESULTS: 8 cases (6 male, 2 female)
