The Epidemiology and Management of Maxillofacial Fractures at a Tertiary Care Hospital in a Conflict-Torn Region in Somalia.

The etiology of maxillofacial fractures (MFFs) varies according to the geographical location and density of the population. This study aimed to analyze the etiology, pattern, and treatment of MFFs. Epidemiological characteristics and treatment modalities of MFFs have never been evaluated in Somalia. The study included 45 patients who were operated on for MFFs at a tertiary care hospital in Somalia (2018-2019). Patient demographics, fracture causes, types, associated non-facial injuries, treatment modalities, and hospitalization-time were evaluated. The most common etiological factors of the MF

Prevalence of serum antibodies against bloodborne and sexually transmitted agents in selected groups in Somalia.

Somalia has suffered from a civil war during the last 10 years. In this period the use of whole blood has increased at least twofold in Mogadishu, Somalia compared with pre-war. Screening possibilities are limited. Recent data concerning the prevalence of infections with blood-borne and sexually transmitted agents are not available from this country. To investigate the spread of human immunodeficiency virus (HIV-1/2) and other blood-borne or sexually transmitted agents we tested a total of 256 serum samples collected in the summer of 1995 from blood donors, hospitalized children and adults in

Identification of Rickettsia conorii infection by polymerase chain reaction in a soldier returning from Somalia.

A soldier developed characteristic manifestations of boutonneuse fever shortly after leaving Somalia. Rickettsial DNA was detected in a biopsy sample of the tache noire by a polymerase chain reaction (PCR) in which primers derived from the 190-kD antigen gene of Rickettsia rickettsii were used. The source of this DNA was identified as Rickettsia conorii by restriction fragment length polymorphism (RFLP) analysis of the PCR product. R. conorii was also isolated from the skin biopsy specimen. The patient did not develop a significant increase in specific antibodies, as assessed by indirect fluor

Human toxoplasmosis in Somalia. Prevalence of Toxoplasma antibodies in a village in the lower Scebelli region and in Mogadishu.

The acquisition of Toxoplasma antibodies in various age groups was studied in 2 populations in Somalia, inhabitants of a village in the southern part of the country and residents in Mogadishu. The overall prevalence of antibodies was 56% in the village and 40% in Mogadishu. In both populations, antibodies were acquired early in life. At the age of 10 years, 44% of the villagers and 31% of the Mogadishu children were seropositive. In Europe and the USA the highest rate of antibody acquisition has been recorded in young adults. This difference may indicate different routes of transmission. In Eu

The prevalence of 'Guboow' among children in Somalia and the impact of socioeconomic factors: a cross-sectional study.

BACKGROUND: Various traditional medical procedures are widely practiced in Somalia. The most important reasons for this are difficulties in accessing modern health facilities, the low cost of traditional medicine and local beliefs in the beneficial effects of healers and local pharmacies. Guboow is a painful traditional treatment involving the application to the body of fire-heated wooden or similar objects. Although Guboow is known to be practiced in Somali society, community attitudes towards the practice and levels of dependence are unclear. In this study, we aimed to assess Guboow use and

The asset-vulnerability trap: climate shocks and health-seeking behavior for chronic diseases in Somalia.

Somalia faces a dual crisis of high chronic disease burden and extreme climate vulnerability. This study examines the predictors of health-seeking behavior (HSB) for chronic diseases among Somali households. Using data from the 2020 Somali Demographic and Health Survey (SDHS), survey-weighted probit model was applied to a sample of 69,998 individuals to analyze predictors of regular treatment-seeking behavior. Approximately 6.6% of the population reported a chronic disease, with 48.8% of those individuals seeking regular treatment. Wealth (AME = 0.094, p < 0.001) and urban residence (AME = 0.0

Operationalizing public health emergency operations centers in Somalia: a scoping review of challenges and strategies.

BACKGROUND: Public Health Emergency Operations Centers (PHEOCs) are central to national health security systems, enabling coordinated preparedness, surveillance, and response to public health emergencies. However, their operationalization remains particularly challenging in fragile and conflict-affected settings such as Somalia, where systemic constraints, governance fragmentation, and limited resources undermine emergency coordination capacity. This study aimed to systematically map the existing evidence on the challenges and strategies for operationalizing PHEOCs in Somalia and comparable fr

Improving Tuberculosis Treatment Outcomes in Somalia: A Narrative Review of Strategies, Challenges, and Policy Recommendations.

Tuberculosis (TB) remains a major public health concern in Somalia, driven by prolonged conflict, population displacement, fragile health systems, and adverse social determinants of health. Although Somalia has made progress in expanding TB services and achieving favourable treatment outcomes for drug-susceptible TB, the country continues to experience a high TB burden and emerging drug-resistant tuberculosis (DR-TB). Persistent challenges include delayed diagnosis, treatment interruption, catastrophic patient costs, paediatric TB care gaps, and reliance on external donor funding. This narrati

Artificial Intelligence as a Radiology Gap-Filler in Somalia: A Narrative Review with a Pragmatic Implementation Framework.

Diagnostic imaging supports emergency care, trauma assessment, tuberculosis detection, maternal and neonatal care, cancer diagnosis, and surgical decision-making, yet timely specialist interpretation remains limited in many low-resource health systems. Somalia is rebuilding a health system affected by workforce shortages, fragmented information systems, uneven service distribution, infrastructure constraints, and limited regulatory capacity. This narrative review integrates peer-reviewed evidence, global guidance, and Somalia-specific policy sources to examine where artificial intelligence cou

Somalia's Drought-Disease Spiral: Climate Shocks as an Emerging Public Health Security Threat.

This narrative commentary examines Somalia's drought-disease spiral using selected published literature, humanitarian reports, surveillance updates, and climate-health evidence. It focuses on how recurrent droughts, floods, food insecurity, displacement, malnutrition, and fragile health-system capacity interact to increase the risk of climate-sensitive infectious diseases. Somalia is among the countries most vulnerable to climate variability, and repeated environmental shocks have intensified humanitarian needs while placing additional pressure on an already fragile health system. Recent clima

Doctors' experiences on dealing with informed consent required for lifesaving interventions for pregnant women in Somalia.

BACKGROUND: Informed consent is a crucial legal and ethical requirement in the physician-patient relationship for all aspects of care. Despite, patients have the right to make their own decision in health, women in the Middle East and Africa, including Somalia, often have limited autonomy in healthcare decisions due to patriarchal structures. In Somalia, male family members including husbands frequently hold the ultimate authority in women's healthcare choices, sometimes restricting access to lifesaving sexual and reproductive health services. PURPOSE: To explore doctors' experiences of delay

A qualitative study of patient choices between herbal and hospital care for infectious diseases in Somalia.

Somalia's healthcare system is marked by a reliance on traditional medicine and under-resourced modern facilities. This study explores how communities navigate treatment choices for infectious diseases, such as malaria and cholera, amid competing trust in herbal remedies and biomedical care. We conducted in-depth interviews with 30 participants, including 15 patients, 10 traditional healers, and 5 healthcare providers, alongside 3 focus group discussions (FGDs). Data were analyzed using thematic analysis. Three key themes emerged: (1) the cultural legitimacy of traditional medicine, (2) the pe

Hypokalemic periodic paralysis as the first sign of thyrotoxicosis- a rare case report from Somalia.

BACKGROUND: Thyrotoxic hypokalemic periodic paralysis (THPP) is a rare complication of hyperthyroidism characterized by thyrotoxicosis, hypokalemia, and paralysis. It is the most common form of acquired periodic paralysis. THPP is precipitated by strenuous exercise, a high carbohydrate diet, stress, infection, alcohol, albuterol, and corticosteroid therapy. It is most common in Asian men with hyperthyroidism and exceptionally rare in black people. CASE PRESENTATION: A 29-year-old man was admitted to the emergency department in Somalia with a sudden onset of paralysis after a high carbohydrate

Ticks and tick-borne pathogens of domestic animals in Somalia and neighbouring regions of Ethiopia and Kenya.

Ticks and tick-borne pathogens (TBPs) of domestic animals in Somalia and neighbouring regions of Ethiopia and Kenya are reviewed to identify knowledge gaps in these regions, where unrestricted livestock movements across borders are common. Major scientific databases, such as PubMed, Web of Science, Scopus, CABI, and Google Scholar were searched, to retrieve articles based on papers published between 1960 and March 2023. Thirty-one tick species representing six genera (Rhipicephalus, Hyalomma, Amblyomma, Haemaphysalis, Ornithodoros and Argas) were reported to infest domestic animals, mainly liv

Seroprevalence of anti-Toxoplasma gondii antibodies and associated risk factors in pregnant women in Somalia.

Toxoplasma gondii infection is a worldwide protozoan parasitic disease caused by the parasite Toxoplasma gondii. T. gondii infection in gestation period can lead to fetal or neonatal death, and besides congenital defects. So far, there are no data available regarding T. gondii seropositivity and associated risk factors in pregnant women in Somalia. The investigations of these aspects were done in 403 pregnant women attending the antenatal care (ANC) at Benadir hospital, in Mogadishu city. Blood samples were collected and analyzed for anti-T. gondii antibodies by enzyme-linked immunosorbent ass

Evaluation of the electronic Early Warning and Response Network (EWARN) system in Somalia, 2017-2020.

BACKGROUND: In 2008, Somalia introduced an electronic based Early Warning Alert and Response Network (EWARN) for real time detection and response to alerts of epidemic prone diseases in a country experiencing a complex humanitarian situation. EWARN was deactivated between 2008 to 2016 due to civil conflict and reactivated in 2017 during severe drought during a cholera outbreak. We present an assessment of the performance of the EWARN in Somalia from January 2017 to December 2020, reflections on the successes and failures, and provide future perspectives for enhancement of the EWARN to effectiv

COVID-19 Vaccine Acceptability and Adherence to Preventive Measures in Somalia: Results of an Online Survey.

Most countries are currently gravitating towards vaccination as mainstay strategy to quell COVID-19 transmission. Between December 2020 and January 2021, we conducted a follow-up online survey in Somalia to monitor adherence to COVID-19 preventive measures, and COVID-19 vaccine acceptability and reasons for vaccine hesitancy. Adherence was measured via a composite adherence score based on four measures (physical distancing, face mask use, hand hygiene, and mouth covering when coughing/sneezing). We analyzed 4543 responses (mean age: 23.5 ± 6.4 years, 62.4% males). The mean adherence score duri
