[The coexistence of spinal canal stenosis in fragility fractures of the spine].

BACKGROUND: In trauma care of fragility fractures of the spine, degenerative stenosis is often seen as an accompanying pathology. This may lead to a possible compression of neurogenic structures. The stenosis of the spinal canal can manifest itself with clinically significant complaints before the fracture occurs. This coexistence may have an impact on the injury itself or may provoke a complicated treatment of the fracture. AIM: The aim of this work is to differentiate these pathophysiologies and their merger in terms of clinical diagnostics and treatment options. DIFFERENTIAL DIAGNOSIS: Th

Urinary Retention and Air in the Spinal Canal; a Case Report.

Cauda equina syndrome (CES) although uncommon, is a very serious condition, which should be diagnosed as soon as possible. Urinary dysfunction following a lumbosacral trauma is a key for the physician to consider CES as the most probable diagnosis. Up to 62% of CES patients report a recent episode of trauma. We herein report a young man with CES due to sacral fracture with an interesting imaging.

Sarcoidosis of the thyroid gland and spinal canal: unique localizations.

Sarcoid involvement of the thyroid gland and spinal canal are rare individual manifestations of sarcoidosis. In this article, we will describe two concurrent cases of spinal canal and thyroid sarcoid involvement. Diagnostic criteria usually include histologic identification of a non-caseating granuloma, supportive laboratory or imaging tests or both, and a compatible clinical course. Our goal is to discuss the presentations and clinical manifestations of our patients, and we will also review the literature involving sarcoid involvement of the spinal canal and thyroid. Although rare, atypical l

Surgical removal of a microchip from a puppy's spinal canal.

A 1.6 kg, six-week-old Tibetan Terrier was admitted with a 12-hours history of acute onset of progressive tetraparesis following insertion of a microchip to the dorsal cervical region. Neurological examination indicated a lesion to the Ce(1) to Ce(5) spinal cord segments. Radiographic examination confirmed the intra-spinal location of a microchip foreign body at the level of the second cervical vertebra. Microchip removal was achieved following dorsal hemi-laminectomy; significant intra-operative haemorrhage was encountered. The puppy was ambulatory at day seven. Follow-up telephone interview

"Spinolith": case report of a loose body in the spinal canal.

STUDY DESIGN: Case report. A hitherto unreported finding of a bony loose body found lying in the spinal canal causing spinal canal stenosis is presented. SUMMARY OF CLINICAL DETAILS: A 68-year-old, fit man presented with a history of progressive neurologic claudication and neurologic deficit in both his lower limbs. Clinical examination revealed excellent range of movements in his lumbar spine and bilaterally normal straight leg raising. He had no significant pain in his back. Neurologic examination showed affection of L5 and S1 dermatomes and myotomes bilaterally. Magnetic resonance imaging

[Metastasis of a spinal myxopapillary ependymoma to the inner auditory canal].

Ependymomas are usually benign tumors of the central nervous system that derive from the ependymal cells lining liquor-filled spaces. Myxopapillary ependymomas are a variant occurring almost exclusively in the region of the cauda equina. Histologically, these tumors are benign (WHO grade I). The following case describes the first reported spread of a spinal myxopapillary ependymoma into the internal auditory canal. The symptoms and diagnostic results are similar to other tumors of this location. There are no typical findings in the diagnostic imaging (CT, MRI). The extended middle cranial foss

Mechanisms of neurovascular compression within the spinal and intervertebral canals.

OBJECTIVE: To describe some possible causes of encroachment on human spinal and intervertebral canal (foramen) neurovascular II structures. DATA SELECTION AND SYNTHESIS: A review of some imaging films of patients aged 38 to 52 years and some human autopsy histopathologic sections from 40- to 60-year-old cadavers to determine what structures may be responsible for neurovascular compression in individuals in this relatively young-to-middle-age group and to illustrate some examples. RESULTS: Stenosis of the spinal and intervertebral canal neurovascular structures can be caused by various bony an

Effect of low back posture on the morphology of the spinal canal.

OBJECTIVE: To define the possible mechanism of posture-dependent symptoms of spinal stenosis by measuring the effect of low back posture on morphologic changes of the intervertebral discs and spinal canal in healthy young people. DESIGN: Twenty healthy young volunteers underwent magnetic resonance imaging while supine with their spine in neutral, flexed, extended, and right and left rotational positions. The axial MR images at the middle of the intervertebral discs of L3-4 and L4-5 were analyzed to measure the difference in the size and shape of the intervertebral discs and spinal canal in ea

[Measurements of the lumbar spinal canal of Chinese by computed tomography].

The study of the lumbar canal sizes in Chinese has not been reported before. Thirty adult patients, aged 20 to 40 years, who sustained thoracolumbar or lumbar fracture-dislocation and needed further computed tomographic (CT) scanning were included in this study. There was no significant back pain or sciatica on these patients before. Measurements of the med-sagittal anteroposterior (A-P) and interpedicular (I-P) diameters of the spinal canals L1 to L5 were requested at the same time during CT scanning. The results show that the lowest normal limits of the A-P and I-P diameters of the lumbar ca

Sparganosis in the spinal canal with partial block: an uncommon infection.

A case of rare intraspinal sparganosis treated by surgical excision is described. In this 59-year-old male with paraparesis and voiding problem, an intradural mass was noted on myelogram. Magnetic resonance (MR) and computed tomographic appearance of spinal and associated cerebral lesion are illustrated and possible route of migration discussed. This represents, to our knowledge, the first MRI demonstration of intraspinal sparganosis reported in the literature.

Central spinal cord lesions in stenosis of the cervical canal.

34 patients suffering from cervical spondylotic myelopathy confirmed by myelography were examined by delayed CT 6-10 h after myelography. Twelve patients showed bilateral intramedullary collections of contrast medium, predominantly cranial to the stenosis. In these patients males predominated, the duration of clinical symptoms lasted longer although their age was lower. There was no correlation to the degree and the extension of the narrowing of the cervical spinal canal. Half of 20 patients undergoing consecutive decompressive surgery showed intramedullary contrast enhancement, and this was s

[Radiodiagnosis of the spinal canal using a nonionic contrast medium].

In 320 cases contrast investigations of the vertebral column were carried out injecting by lumbar tap Metrizamide in 174 cases and Amipaque in 126 cases. In 259 cases the investigation was limited to the lumbar part, in 25 cases to the thoracic part, and in 19 cases to the cervical part of the spine. In 12 cases the whole vertebral canal was explored in this way. In 5 cases only epidurography was done. Very good or good contrast filling of the dural sac was obtained in the thoracolumbar segment using contrast medium 200 mg I/ml. In some cases this degree of concentration was inadequate for goo

Cervical myelopathy caused by developmental stenosis of the spinal canal.

The authors present six patients with myelopathy caused by developmental stenosis of the cervical spine. Hyperextension injuries precipitated the onset of symptoms in two patients, aged 19 and 20 years. In four, 41 to 69 years of age, symptoms were gradual in onset, progressing to severe disability. X-ray films revealed narrowing of the dorsoventral diameter of the spinal canal to as little as 1.0 cm. The myelograms showed widening of the cord in the transverse plane strongly suggestive of an intramedullary tumor. A unique finding was maldevelopment with flattening of the neural arch often hid

[State of the vertebral canal in vertebral and spinal cord injuries].

The results of the analysis of 128 patients who had died after trauma of the spine and spinal cord are discussed. Total disturbance of the anatomical intactness of the vertebral canal walls was demonstrated on all specimens. In 85% of patients there were fractures with or without displacement of the injured vertebra; 15% of patients had suffered injury only to the ligamentous-capsular apparatus, the disk included, with no displacement--dislocation of the vertebra. The shape of the vertebral canal was distorted only in cases with displacement of the vertebra. The area of the canal was reduced t

Subarachnoid block and enlargement of the spinal canal in hypertrophic neuritis.

A case of Dejerine-Sottas hypertrophic neuritis is reported. The patient, a 45-year-old male, suffered from chronic hypertrophic polyneuropathy, abnormal pupils, fasciculations, tremor, back pain, impotence, sphincter disorders, cramps, and lightning pains in the lower extremities. Besides extensive subarachnoid block, there was X-ray evidence of enlargement of the bony spinal canal with scalloping of the lumbar vertebrae. Surgical exploration showed these abnormalities to be due to extreme hypertrophy of the cauda equina. Histologic findings in peripheral nerve and lumbar root biopsies were t

Measurements of the lumbar spinal canal in 2 racial groups.

Direct measurements of 1,340 lumbar vertebrae in 275 skeletons in two racial groups are reported. The average figure, and the upper and lower limits of normal are reported for the anteroposterior diameter of the canal and for Spinal Index. Radiological studies have established a technique for defining the posterior margin of the spinal canal on plain X-ray films. It is formed by a line joining the apex of the superior to the inverted apex of the inferior articular facet. The Spinal Index is not a reliable factor. The anteroposterior diameter alone is the essential parameter in assessing spinal

Neurogenic intermittent claudication and stenosis of the lumbar spinal canal.

Neurogenic intermittent claudication is briefly reviewed and correlated with twenty consecutive cases of lumbar spinal canal stenosis. Irreversible damage may occur if surgical decompression is unduly delayed. A unilateral decompression may be sufficient in carefully selected cases.
