Ontario health technology assessment series2005eng
OBJECTIVE: The objective of this health technology policy assessment was to determine the effectiveness of spinal cord stimulation (SCS) to manage chronic intractable neuropathic pain and to evaluate the adverse events and Ontario-specific economic profile of this technology.
CLINICAL NEED: SCS is a reversible pain therapy that uses low-voltage electrical pulses to manage chronic, intractable neuropathic pain of the trunk or limbs. Neuropathic pain begins or is caused by damage or dysfunction to the nervous system and can be difficult to manage. The prevalence of neuropathic pain has been est

This study describes a man with a long history of oesophageal pain that led to inability to swallow food and drink. Over a period of 8 years, he had multiple oesophageal operations that were unsuccessful. He presented, to the pain management team, with persistent oesophageal pain and required jejunostomy tube feeding to maintain nutrition. Conservative pain management strategies failed. Spinal cord stimulation (SCS) was suggested, and after counseling, an electrode was placed in the high thoracic region. Stimulation covered the area of the chest pain. He achieved immediate reduced pain on swal

Cervical spinal cord stimulation (SCS) has been used in an animal model of acute ischaemic brain oedema. In anaesthetized rats a temporary carotid bilateral occlusion of 60 minutes was carried out, followed by a reperfusion period of 30 minutes before sacrifice. Two electrodes were placed in each side of the duramater at C2 level. SCS was started either 60 minutes before ischaemia or at the beginning of the reperfusion period. The parameters of stimulation were: pulse width 0.1 ms; frequency of 25 cps; intensity at one third of the threshold which produced motor responses. The specific gravity

Spinal cord and subcortical brain stem evoked potentials had an amplitude at least 2 times higher when the cauda equina rather than bilateral peripheral nerves was stimulated. Cauda equina stimulation is indicated when potentials to peripheral nerve stimulation are absent or are too low in amplitude to permit reliable surgical monitoring. The technique is essentially without risks, but should be performed with a small lumbar puncture needle (21 to 22 gauge), and is contraindicated in patients with general infections, increased cerebrospinal fluid pressure, or a hemorrhagic tendency (thrombocyt

Chronic pain continues to be a major global health burden and is frequently refractory to conventional pharmacologic and conservative therapies. Spinal cord stimulation (SCS) has emerged as an important neuromodulatory treatment for selected patients with chronic neuropathic and mixed pain syndromes. Since its introduction in the 1960s, SCS has evolved from paresthesia-based tonic stimulation into more adaptive and personalized neuromodulation. This review summarizes the current evidence regarding the mechanisms, clinical applications, technological advances, and future directions of SCS thera

INTRODUCTION: Spinal cord injury (SCI) is one of the most dreaded complications after spinal cord stimulation (SCS) implantation surgery. As a result, intraoperative neurophysiological monitoring (IONM) has been proposed to avoid accidental damage to nervous structures under anesthesia and confirm positioning for optimal stimulation. Our study uses a large administrative claims database to determine the 30-day risk of SCI after SCS implantation.
METHODS: This retrospective cohort study used the IBM MarketScan Commercial and Medicare Supplemental Databases from 2016 to 2019. Adult patients und

PURPOSE OF REVIEW: Spinal cord stimulation has been increasing in influence as an option to regulate pain, especially in the chronic pain patient population. However, even with the numerous changes made to this technology since its inception, it is still prone to various complications such as hardware issues, neurological injury/epidural hematoma, infections, and other biological concerns. The purpose of this article is to thoroughly review and evaluate literature pertaining to the complications associated with percutaneous spinal cord stimulation.
RECENT FINDINGS: Lead migration is generally

ObjectivesSpinal cord stimulation (SCS) has been shown to be an effective and safe option to treat patients with intractable pain in the general population. Our study examined the experience of US veterans with SCS.MethodsWe reviewed electronic health records and conducted phone interviews with 65 veterans who had SCS from 2008 to 2020 at the Southeastern Louisiana Veterans Health Care System (SLVHCS). Our primary outcome measure was veteran would recommend SCS to peers. Secondary outcomes were improvements in activities of daily living and ability to decrease opioid pain medications.ResultsA

BACKGROUND: Current available therapies for Parkinson disease (PD) have strong limitations, and patients usually present with refractory symptoms despite all efforts. Deep brain stimulation (DBS), which has been used in PD patients for decades (since 1987), has best indications for symptoms like tremor, motor fluctuations, or dyskinesia. However, postural instability and gait disturbances (PIGD) have restricted benefits with DBS. In 2009, spinal cord stimulation (SCS), a well-established therapy for chronic pain, has emerged as a potential alternative therapy that may help control unresponsive

OBJECTIVE: Implantation of a spinal cord stimulator (SCS) is one option for pain control in individuals with chronic lumbosacral radicular or axial lumbar pain. The expected positioning of SCSs based on the location of pain, the types of electrodes (percutaneous vs surgical paddle), and the types of electrode arrays and the potential complications have not been described to date in the radiology literature.
MATERIALS AND METHODS: A 5-year retrospective review of the radiology information system at our institution revealed 24 patients with images of 36 SCSs. Those images were reviewed to ident

Spinal cord stimulation is increasingly utilized as a treatment to alleviate low back pain and lumbar radiculopathy, particularly in patients with failed back surgery syndrome. We present an illustrative case of early, rapidly progressive methicillin-resistant Staphylococcus aureus (MRSA) infection after a brief stimulator trial lead implantation. Operators should maintain a high level of suspicion for deep infection, including epidural abscess, even when only minor symptoms and signs are present. Because of the poor ability to clear infections in the presence of a retained foreign body, the d

BACKGROUND: Over the last few decades, spinal cord stimulation (SCS) has become one of the main treatments in the therapeutic arsenal available to pain treatment units. New stimulation systems have been developed and the indications of neurostimulation have been expanded. The premises for a successful technique remain the same; good patient selection, good surgical technique, and good management of electrical parameters when programming.
DESIGN: An observational report.
OBJECTIVES: The primary objective of the study was to determine the relationship between changes in impedance (R) and energ

Sensory organization at the spinal segment level is commonly inferred from dermatomal maps that assume a fixed correspondence between cutaneous regions and spinal segments. However, based on the complexities of spinal neuroanatomy and neurophysiology, the distribution of sensory signals within the cord may be broader and less segment-specific than dermatomal maps suggest, leaving the segment-level localization of sensory-evoked activity in humans uncertain. Spinal cord functional magnetic resonance imaging (fMRI) is currently the only technique capable of noninvasively mapping sensory activity

Evoked Compound Action Potentials (ECAPs) have been studied for their potential use as a feedback signal for paresthesia-based spinal cord stimulation (SCS) for chronic neuropathic pain. While paresthesia-based SCS therapy is still commonly used, paresthesia-free modalities have emerged over the past decade and are commonly preferred by patients because they avoid persistent stimulation-induced paresthesia. Characterizing the relationship between ECAP detection thresholds and both paresthesia-free and paresthesia-based SCS dosing may assist clinicians in selecting appropriate SCS systems and p

Epidural spinal cord stimulation (SCS) is a neuromodulatory, reversible procedure for the treatment of selected chronic pain conditions, primarily neuropathic in nature. The updated German S3 guideline provides evidence-based recommendations for the use of SCS and dorsal root ganglion stimulation (DRG-S). Key innovations include recent technical advances and increased integration into multimodal pain management. The guideline aims to enhance the care of patients with chronic pain by offering rational, scientifically substantiated recommendations. A systematic literature review was conducted, i

PURPOSE OF REVIEW: Chronic low back pain (LBP) is a prevalent and debilitating condition affecting millions worldwide. Among emerging interventions, spinal cord stimulation (SCS) has gained attention as a potential alternative for managing chronic LBP, particularly when alternative approaches fail to provide adequate relief.
RECENT FINDINGS: This systematic review focuses on both residual pain levels and ability to perform daily tasks after treatment with SCS. The present investigation includes a systematic search for studies from PubMed, Google Scholar, and Cochrane, and Embase. Sources were
