Pain and disability associated with new vertebral fractures and other spinal conditions.

Although vertebral fractures are common among older women, little is known about associations with pain and disability. In this prospective study, fractures which occurred after the initial radiograph were strong predictors of back pain and disability at the end of followup, with odds ratios (OR) of approximately 4-5. Other, independently significant predictors were self-reported history of (1) disk disease (OR = 3-9), (2) traumatic back injury (OR = 4-7), and (3) spinal arthritis (OR = 3-6). In longitudinal analyses, new fractures were associated (OR = 6.4; 95% CI = 2.6, 15.6) with increases

Cervical fractures and spinal cord injury: outcome of surgical and nonsurgical management.

Data were collected retrospectively for 102 consecutive patients with a cervical spinal cord injury admitted to a spinal cord injury center between 1976 and 1986. Frankel's classification and level of spinal cord injury stayed the same or improved in all patients. The complications that occurred compared favorably with outcomes reported in the literature. Approximately 60% of patients achieved a catheter-free voiding status before dismissal from primary rehabilitation. Patients treated with early surgical stabilization of the cervical column were hospitalized a mean of 21 fewer days than their

Case report. Posterior splint for leg fractures in spinal cord injured patients.

Traumatic fracture of the long bones of the lower extremities may frequently occur in spinal cord injured patients. In the past, conservative treatment with pillow or sand bag splinting was advocated by many clinicans. This paper presents two cases in which posterior splinting was used on the affected leg for stabilization of the fracture with good result and well accepted by patients. Also, the use of Mud bed for prevention of pressure sores was illustrated.

A review of long-bone fractures in patients with spinal cord injuries.

In a retrospective investigation of 38 long-bone fractures in 30 spinal cord injured patients (mean follow-up, 22 months), fractures were classified acute (occurring at the time of spinal cord injury) or pathologic (occurring in the chronically injured patient). The methods of treatment and complications were recorded and evaluated. The incidence of long-bone fractures was 6.7%. Complication rates were high, ranging from 20% to 40%, with open or closed treatment of extremity fractures. Strict adherence to established operative fracture management principles in paraplegic patients irrespective

Spinal canal restoration by posterior distraction or anterior decompression in thoracolumbar spinal fractures and its influence on neurological outcome.

Thirty consecutive patients who had suffered unstable fractures and dislocations of the thoracolumbar spine mostly associated with neurologic impairment and bony encroachment on the spinal canal were treated either with Harrington distraction rods combined with sublaminar wires or with the Zielke-VDS device. These patients were subsequently assessed for neurologic outcome, spinal canal clearance, sagittal and coronal spinal deformity correction preoperatively and postoperatively with a minimum follow-up of 26 months. In the follow-up evaluation, the patients who underwent surgery with Harringt

Spinal Epidural Tuberculoma with Adjacent Vertebral Fracture: A Case Report.

CASE: An 85-year-old woman was transported to our institution due to difficulty in walking. Preoperative imaging showed spinal cord lesions indicative of spinal cord tumor at the T7-8 level, accompanied by T8 vertebral fracture. Intraoperatively, the spinal lesion was suspected to be an epidural abscess; therefore, the capsule was resected, and the abscess was drained. We added pedicle screw fixation at the T6-10 level. Postoperatively, the spinal cord lesion was definitively diagnosed as spinal epidural tuberculoma. CONCLUSION: Spinal epidural tuberculomas should be considered in the treatme

Bone Imaging and Fracture Risk after Spinal Cord Injury.

Spinal cord injury (SCI) is characterized by marked bone loss and an increased risk of fracture with high complication rate. Recent research based on advanced imaging analysis, including quantitative computed tomography (QCT) and patient-specific finite element (FE) modeling, has provided new and important insights into the magnitude and temporal pattern of bone loss, as well as the associated changes to bone structure and strength, following SCI. This work has illustrated the importance of early therapeutic treatment to prevent bone loss after SCI and may someday serve as the basis for a clin

Manubriosternal dislocation with spinal fracture: A rare cause for delayed haemothorax.

Type 2 manubriosternal dislocations with concomitant spinal fracture are rare and may be associated with thoracic visceral injuries. The complication of delayed haemothorax has not been reported yet. We report a case of a young male who suffered manubriosternal dislocation with chance type thoracic spine fracture due to fall of a tree branch over his back. The haemothorax presented late on day three. The possible injury mechanism is discussed along with review of literature. We conclude that a lateral chest radiograph is indicated in spinal fracture patients complaining of midsternal pain. Com

Spinal tuberculosis occurring after a closed bursting fracture of the vertebrae.

INTRODUCTION: Only 12 cases of musculoskeletal tuberculosis (TB) that occurred at the site of recent blunt trauma have been previously described. MATERIALS AND METHODS: A 43-year-old previously healthy man experienced an uncomplicated bursting fracture at the L2 vertebra without any spinal tuberculosis (ST)-associated evidence. During the conservative treatment, he was in good physical condition with back pain totally subsiding and occasional ambulation in a spinal brace, whereas 7 months after the original trauma, he presented with a characteristic manifestation of ST at the same vertebra.

Fracture risk in patients with muscular dystrophy and spinal muscular atrophy.

We aimed at studying fracture risk in patients with Duchenne's muscular dystrophy (DMD), Becker's muscular dystrophy (BEMD), and spinal muscular atrophy type II and III (SMA II and III). A self-administered questionnaire was mailed to 293 patients with DMD, BEMD, SMA II or SMA III of which 229 returned the questionnaire. Each respondent was compared with an age- and gender-matched control subject. The mean age was 23.9 +/- 15.9 years for the patients and 23.3 +/- 16.5 years for the controls. There were significantly more fractures among patients than controls after the diagnosis was made (RR =

Segmental spinal instrumentation in the management of fractures of the thoracic and lumbar spine.

Eleven cases of fracture of the thoracic and lumbar spine were treated with segmental spinal instrumentation (Luque rods). The majority of these injuries occurred in motor vehicular accidents or falls from a height and were usually associated with multisystem trauma. This method of instrumentation provided immediate rigid internal fixation, which obviated the need for postoperative orthoses, allowed easier nursing care and early ambulation, and facilitated rehabilitation of patients with spinal cord injury. At 13 months' average follow-up, there have been no instances of wire breakage, rod def

The management of long bone fracture in the spinal cord injured patient.

Principles of management of long bone fractures in patients with spinal cord injuries vary with the type of fracture and duration of quadriplegia. Class I fractures (acute injuries) are managed by existing principles of long bone fracture management with a tendency to open reduction and internal fixation to dispense with the necessity of plaster casts on insensate skin. Class II fractures (pathologic--osteoporotic) heal well, require minimal specific treatment, and open reduction is usually contraindicated. Class III fractures (acute injuries in chronic paraplegia) are treated with methods lea
