OBJECTIVE: Nontraumatic spinal cord injury (NT/SCI), which can occur secondary to spinal stenosis, has been shown to represent a significant proportion of individuals admitted for SCI rehabilitation. The objective of this study was to compare demographics and outcomes of patients with spinal stenosis-induced SCI (SS/SCI) with those with traumatic spinal cord injury (T/SCI) following inpatient rehabilitation.
DESIGN: This 7-year prospective review compared 81 patients with SS/SCI and 102 patients with T/SCI admitted to an SCI rehabilitation unit with similar levels and completeness of injury.

Anemia is not known to cause intermittent claudication. We herein present the case of a 74-year-old male patient with intermittent claudication, in whom the primary symptom was severe iron-deficiency anemia. The characteristics of claudication mimicked a neurogenic disorder, but lumbar magnetic resonance imaging ruled out lumbar spinal stenosis, and the symptoms resolved after the anemia improved. This case highlights the need to consider anemia in the differential diagnosis of patients presenting with intermittent neurogenic claudication. A comprehensive evaluation, including tests to rule ou

Gout is a common cause of inflammatory arthritis, typically affecting the joints of the appendicular skeleton. In this report, we present the relatively less common scenario of chronic tophaceous gout affecting the lumbar spine and pelvis, complicated by compressive neuropathy, and notable for its advanced initial presentation in a young patient. We review the pathophysiology underlying gout and discuss its clinical and laboratory presentation. We also use our case as an example to present the radiographic, CT, and MR imaging features of gout affecting the lumbar spine, which can often present

INTRODUCTION: Unlike traditional open laminectomy, minimally invasive decompression (MID) spares the important midline structures of the spine (i.e., the spinous process and the supraspinous and interspinous ligaments).
STEP 1 PREOPERATIVE PLANNING: Determine the levels and laterality for the decompression on the basis of the symptoms and findings on the MRI scan.
STEP 2 OPERATING ROOM SETUP: Ensure the correct positioning of the patient and the proper setup of the equipment.
STEP 3 MARKING THE LEVELS: Use fluoroscopy to localize the level(s) of the stenosis.
STEP 4 SKIN INCISION AND TUBE

Lumbar Spinal Stenosis is a typical disease of the elderly patient that mainly originates in degenerative multisegmental changes of the lumbar vertebral column. The classical symptom of pain irradiation into the legs whilst walking and relief with standing is similar to peripheral arterial disease presentation but differs in the sense that symptoms can be triggered through lumbar extension and relieved with lumbar flexion whereas arterial disease is correlated with pathological arteriovascular findings. Diagnosis is usually confirmed through magnetic resonance imaging (MRI) and response to con

Degenerative lumbar spinal stenosis is caused by mechanical factors and/or biochemical alterations within the intervertebral disk that lead to disk space collapse, facet joint hypertrophy, soft-tissue infolding, and osteophyte formation, which narrows the space available for the thecal sac and exiting nerve roots. The clinical consequence of this compression is neurogenic claudication and varying degrees of leg and back pain. Degenerative lumbar spinal stenosis is a major cause of pain and impaired quality of life in the elderly. The natural history of this condition varies; however, it has no

OBJECTIVE: To study the indication for reoperations after lumbar decompression, the factors predisposing to redo operations, and the effect of prior instrumentation on developing adjacent level stenosis requiring reoperation.
METHODS: Kaplan-Meier analysis was used to compare the median interval to first reoperation. Cox regression was used for multivariate analysis of time to first reoperation.
RESULTS: Of 335 patients who underwent surgery for lumbar spinal stenosis, 63 (18%) underwent instrumentation in addition to decompression. There were 50 reoperations performed in 44 patients (13%).

Degenerative lumbar stenosis is a common source of pain and disability in the elderly. It presents clinically with a variety of symptoms, though neurogenic claudication is the hallmark. There is a multifactorial pathogenesis to lumbar stenosis and its symptoms, and thus, there are multiple management approaches available. Epidural steroid injections (ESIs) are a popular choice in management, however, the literature is vague in definitive support of their use, and providers that utilize injections can use variable techniques to access the spinal canal in order to deposit the steroid at the appr

Presented here is a prospective study assessing the efficacy of decompression of concomitant noncontiguous level (thoracic & lumbar) stenosis in accordance with neurological findings, nerve root blocks, and myelographically proven disease. The objective was to determine the efficacy, clinical outcome, and functional recovery in patients undergoing simultaneous decompression. No previous study has focussed on the clinical outcome of such simultaneous decompression. Twenty-one patients with neurological claudication, progressive gait disturbance, upper motor neuron symptoms, and findings of myel

Lumbar spinal stenosis is a major cause of low back and leg pain resulting in disability in elderly patients. There is now increasing number of patients who have lumbar spinal stenosis. Symptoms in spinal stenosis are posture-dependent, because the magnitude of pressure to the nerve at the stenotic area changes by postures. Clinically, symptoms in lumbar spinal stenosis are generally elicited by standing and simple walking, but relieved by sitting. There are many treatment methods including conservative and surgical treatments for lumbar spinal stenosis. The improvement of the quality of life

Early results (12 weeks after surgery) of the lumbar spinal stenosis operative treatment were assessed in the group of 36 patients (aged 40-65 years). In 72% of patients posterior fusion was applied. In the results evaluation lumbar pain, low extremities pain, neurogenic claudicat ion, sensory disturbances and functional status were taken in account. A significant decrease in low extremities and lumbar pain was observed. Some sensory disturbances after operative treatment did not affect general high patients assessment of the performed surgical procedures (positive 80.5% cases).
