Long-term results of splenic artery embolization (SAE) were compared with those of surgical splenectomy and medical therapy for liver cirrhosis complicated by hypersplenism (50, 20 and 46 patients, respectively). Lethality and complication occurrence (AFTER) SAE were as high as 18% and 12% vs 10% and 5% after splenectomy. In long-term period correction of hypersplenism persisted only in 16%. However, clinical outcomes and survival of patients treated conservatively, did not significantly differ from results of transcatheter and surgical treatment. This investigation evidences that SAE has a hi

In a 47-year-old male cadaver the splenic artery traveled partially through the substance of the pancreas. Additionally, the pancreas was apparently large and had an unusual shape; the neck of the organ could not be differentiated; the uncinate processus was absent; two pancreatic ducts were present. The probable reason for this combined anomaly was abnormal fusion of the ventral and dorsal pancreatic buds due to different levels of origin.

We describe the improved ability to catheterize the distal splenic artery consistently using a flexible, flow-directed, 5-F, Swan-Ganz catheter so as to embolize the spleen beyond any pancreatic branch origins to achieve the safe treatment of hypersplenism.

OBJECTIVE: To determine the safety and outcome of splenic artery(SA) transposition in extra-anatomic hepatic arterial reconstruction (HAR) in living donor liver transplantation(LDLT).
METHODS: We retrospectively compared the outcome of the ten liver recipients who underwent HAR with the transposed splenic artery (SA group) with a matched cohort of 40 recipients who underwent HAR with the standard hepatic artery (HA group) between March, 2019 and December, 2020 at liver transplantation department, Pir Abdul Qadir Shah Jeelani Institute of Medical Sciences, Pakistan. The comparison of recipient

We present a rare case of spontaneous trans-splenic shunt and intra-splenic collaterals in a patient with liver cirrhosis and portal hypertension. The shunt and presence of cirrhosis and portal hypertension was incidentally detected by abdominal computed tomographic imaging during evaluation for abdominal pain. There has been a single report on the presence of trans-splenic shunt in two children with extra-hepatic portal venous obstruction but no cases that report intra-splenic collaterals: to the best of our knowledge, this is the first reported case of spontaneous trans-splenic shunt in the

OBJECTIVE: To identify predictive factors for post-embolization splenic infarction and to assess long-term remodeling patterns of the spleen, including regeneration and atrophy, in patients undergoing embolization for splenic artery aneurysm (SAA).
METHODS: This retrospective study included 64 patients (66 cases) with splenic artery aneurysms who underwent transcatheter arterial embolization between February 2007 and November 2023 at a single tertiary center. Embolization techniques included sac packing, trapping, combined approaches, and stent-graft placement.
RESULTS: Technical success was

BACKGROUND: Splenic sequestration crisis is a potentially fatal complication of sickle cell disease, mainly seen in young children. Only a few case series describe the acute splenic sequestration crisis in adults and its management, which primarily consists of supportive care and, in some cases, splenectomy. Splenic artery embolization has seldom been described in sickle cell disease. This is probably the first case in which an adult with sickle cell disease presented with an acute splenic sequestration crisis was managed successfully through splenic artery embolization.
RESULTS: This 22-year

Splenic artery embolization plays an important role in the management of various medical and surgical conditions that are non-traumatic in etiology, in addition to its well-established and widely discussed role in managing splenic trauma. In nontraumatic emergencies of catastrophic bleeding originating from the spleen or splenic artery, splenic artery embolization can be effective in achieving hemostasis as a definitive management, temporary stabilizing measure, or preoperative optimization technique. In addition to emergency clinical conditions, splenic artery embolization can be performed el

BACKGROUND: Rupture of a splenic artery aneurysm during pregnancy is a rare and serious event, occurring mainly during the third trimester. The risk of rupture for an existing splenic artery aneurysm is very high during pregnancy (from 20% to 50%). When it does rupture, the maternal mortality rate is around 75% and the fetal mortality rate 95%. Of 110 cases reported in the literature, only one ruptured during the first trimester.
OBJECTIVES: The aim of this case report is to make emergency physicians aware of this diagnosis, because only if it is considered can it be managed rapidly and appro

The left gastroepiploic artery (LGEA) is rarely described. The aim of this study is to report the fashion of its origin considering the greatly surgical interest of the short gastric vessels in the spleen-preserving distal pancreatectomy with excision of splenic pedicle. About 79 fresh cadaveric pieces we dissect and opacified the LGEA to specify its origin based on the Pinus's classification. We measured the distances between the origin of the LGEA and the terminal division of the splenic artery, the splenic hilus and the great gastric curvature. The LGEA arise from the splenic artery. HISTORY AND CLINICAL FINDINGS: A 62-year-old woman was admitted because of sudden onset of increasingly severe pain in the left lower leg and foot. She also reported having had diarrhoea for the past few days.
INVESTIGATIONS: Physical examination revealed a cold and pale left leg below the knee joint. Peripheral sensory and motor functions were normal. While femoral and popliteal arterial pulses were normally palpable, foot pulses were not felt. Magnetic resonance angiography of the pelvic and leg arteries showed an embolic occlusion of the left popliteal artery extending to the arteries of t

HISTORY AND FINDINGS: A pale looking 33-year-old man with a history of perforated gastric ulcer and pancreatitis with surgically drained pseudocyst was admitted because of painless anal bleeding. Heart rate was 100/min, blood pressure 90/60 mm Hg.
INVESTIGATIONS: Haemoglobin concentration (6.3 g/dl) and RBC count (2.4 mill./microliters) indicated anaemia due to bleeding. Sonography and computed tomography demonstrated chronic calcifying pancreatitis and thrombosis of splenic and mesenteric veins. There were grade 1 oesophageal varices on endoscopy. The source of bleeding was found by coloscop

Pigskin made commercially available as porcine-derived dermal collagen (PDC) contains a macrofibrillar collagen surface that initiates platelet aggregation and the intrinsic pathway clotting mechanism. Sterile-irradiated PDC, nonirradiated PDC, and lyophilized PDC were tested for hemostatic effectiveness and cost for hemostasis against each other and Avitene powder, Avitene web, Collastat, Gelfoam, Superstat, and Thrombostat. Canine arterial, liver, and splenic bleeding models were used. In arterial wounds PDC was more effective than Avitene powder, Avitene web, and Gelfoam (p less than 0.05)

Forty-eight year old man with epigastric pain. CT scan shows a common hepatic artery aneurysm (HAA) of 4,6 cm in size with permeable portal vein. Angiography demonstrates that it affects from the beginning of the celiac trunk until hepatic arteries bifurcation, producing a proximal splenic artery stenosis. Proximal and distal embolization is done achieving a complete aneurysm occlusion. A stent is placed in celiac trunk in order to maintain splenic flow. Permeability of distal hepatic artery through choledochal arteries is observed in a month follow-up CT scan. HAA are infrequent but potential

Aberrant splenic artery originating from the superior mesenteric artery (SMA) is extremely rare and recognition of this anomaly is important in the pre-operative planning of complex surgery such as pancreatic surgery, liver transplantation and vascular surgery. We present the case of an 80-year-old female diagnosed as septic shock due to mesenteric ischemia and obstructive pyelonephritis. Her splenic artery was originating from the SMA and the anomaly was readily appreciated on the pre-operative CT images. An explorative laparotomy associating extensive small bowel resection with endarterectom
