Predicting splenic abnormality in Hodgkin disease using volume response to epinephrine administration.

The change in relative splenic volume following epinephrine administration was used to determine splenic abnormality noninvasively. Selective splenic imaging was accomplished with 99mTc-labeled heat-treated red blood cells, an LFOV gamma camera, a 30 degrees bilateral rotating slant hole collimator, and bilateral slant hole tomographic software. Relative splenic volumes were plotted over time, and correlated with clinical and histologic data. Eight patients with Hodgkin disease and other lymphomas were examined. Volume response to epinephrine appears to be significantly less in abnormal spleen

Increased susceptibility to infection in sickle cell disease: defects of opsonization and of splenic function.

Patients with sickle cell disease (SCD) have an increased susceptibility to bacterial infections and die more frequently from infection than any other cause. Their sera do not promote the phagocytosis of pneumococci normally, apparently because of an inability to utilize the alternate pathway for C3 fixation as a means of opsonization. The splenic dysfunction which exists in SCD may result in depressed synthesis of alternate pathway factor(s), as well as in decreased phagocytic clearance of bacteria.

Splenic Arterial Embolization in the Treatment of Severe Portal Hypertension Due to Pancreatic Diseases: The Primary Experience in 14 Patients.

OBJECTIVE: This retrospective study reports our experience using splenic arterial particle embolization and coil embolization for the treatment of sinistral portal hypertension (SPH) in patients with and without gastric bleeding. METHODS: From August 2009 to May 2012, 14 patients with SPH due to pancreatic disease were diagnosed and treated with splenic arterial embolization. Two different embolization strategies were applied; either combined distal splenic bed particle embolization and proximal splenic artery coil embolization in the same procedure for acute hemorrhage (1-step) or interval s

Covered Stent Placement for a Splenic Artery Pseudoaneurysm in a Patient With Inflammatory Vascular Disease.

Splenic artery pseudoaneurysms (SAPs) are potentially life-threatening vascular lesions with a high risk of rupture that may be associated with inflammatory vascular pathologies. While transcatheter embolization is the most common endovascular treatment for SAPs, covered stent placement preserves arterial patency and splenic perfusion but is limited by anatomic constraints, such as inadequate landing zones and excessive vessel tortuosity. This case report discusses the clinical and technical considerations in successfully stenting a SAP in a patient with a history of systemic lupus erythematos

Renal and Splenic Infarctions: Unmasking Rheumatic Mitral Valve Disease in a Young Adult.

Systemic thromboembolism is an uncommon but clinically significant complication of rheumatic heart disease (RHD), most often seen in association with infective endocarditis. We describe a 39-year-old woman with no known cardiovascular comorbidities who presented with acute left-sided abdominal pain. Imaging revealed concurrent renal and splenic infarctions. Further evaluation uncovered paroxysmal atrial fibrillation, severe mitral regurgitation, and moderate to severe mitral stenosis with marked left atrial enlargement. Transthoracic and transesophageal echocardiography confirmed the diagnosis

True Spontaneous Splenic Rupture Mimicking Peptic Ulcer Disease: A Case Report.

INTRODUCTION: Spontaneous splenic rupture (SSR) is an uncommon and potentially life-threatening cause of acute abdominal pain, often linked to underlying pathology. Because its presentation can mimic more common conditions, diagnosis is frequently delayed. We report a case from a resource-limited setting where SSR initially mimicked peptic ulcer disease, underscoring diagnostic challenges. CASE PRESENTATION: A 57-year-old Ethiopian woman with chronic reflux presented after 5 days of severe epigastric pain radiating to the left shoulder, with nausea and vomiting but no trauma or fever. Initial

Splenic filtration of red blood cells in physiology, malaria and sickle cell disease.

PURPOSE OF REVIEW: The human spleen clears the blood from circulating microorganisms and red blood cells (RBCs) displaying alterations. This review analyzes how generic mechanisms by which the spleen senses RBC, such pitting, trapping and erythrophagocytosis, impact the pathogenesis of two major spleen-related diseases, malaria and sickle cell disease (SCD). RECENT FINDINGS: Scintigraphy, functional histology, comparison of circulating and splenic RBC, ex-vivo perfusion of human spleens and in-silico modeling enable relevant exploration of how the spleen retains and processes RBC in health an

Splenic uptake on FDG PET/CT correlates with Kikuchi-Fujimoto disease severity.

Kikuchi-Fujimoto disease (KFD) is usually self-limiting, but prolonged systemic symptoms often result in frequent hospital visits, long admission durations, or missed workdays. We investigated the role of fluorine-18 fluoro-2-deoxy-D-glucose (18F-FDG) positron emission tomography/computed tomography (PET/CT) in assessing KFD severity. We reviewed the records of 31 adult patients with pathologically confirmed KFD who underwent 18F-FDG PET/CT between November 2007 and April 2018 at a tertiary-care referral hospital. Disease severity was assessed using criteria based on clinical manifestations of

Cytomegalovirus Disease in a Patient With Granulomatosis With Polyangiitis Who Also Has Splenic Necrosis.

Cytomegalovirus infection, which can occur as a result of reactivation due to immunosuppressive treatment in patients with granulomatosis with polyangiitis, is a serious condition that should be kept in mind because of its fatal course. In this article, we report a 49-year-old male patient with a diagnosis of granulomatosis with polyangiitis who developed a life-threatening colonic ulcer due to cytomegalovirus colitis and a shrunken spleen with irregular contours that was detected on abdominal computed tomography. This is a rare case of cytomegalovirus disease in a patient with granulomatosis

Massive Splenic Infarction in a Child With Sickle Cell Disease on Chronic Transfusion Therapy.

Massive splenic infarction (MSI) is a rare complication of sickle cell disease, as the spleen generally atrophies within the first few years of life. We report a case of MSI in a 12-year-old boy with homozygous sickle cell anemia (Hb SS) whose chronic transfusion therapy resulted in hypersplenism. The occurrence of a complicated MSI in our patient should perhaps further encourage elective splenectomy in such patients, despite known potential perioperative complications and postsplenectomy risks of infection and thrombosis.

Atraumatic splenic rupture as a complication of acute exacerbation of chronic pancreatitis, an unusual disease.

We report the case of a patient with acute exacerbation of chronic pancreatitis and he suffered an atraumatic splenic rupture. Splenic rupture not associated with trauma is a rare entity that can occurs in normal spleen (spontaneous) or damaged spleen (pathological). This entity may be associated with local inflammatory processes, such as pancreatitis. Ultrasound is a non-invasive technique which is used in unstable patients. CT is useful for making a diagnosis of extension in patients with hemodynamic stability. Atraumatic splenic rupture as a complication of chronic pancreatitis is an unusua

Imaging characteristics of focal splenic and hepatic lesions in type 1 Gaucher disease.

In Gaucher disease (GD) imaging of liver and spleen is part of routine follow-up of GD patients. Focal lesions in both liver and spleen are frequently reported at radiological examinations. These lesions often represent benign accumulations of Gaucher cells, so-called "gaucheroma", but malignancies, especially hepatocellular carcinoma, are more frequently found in GD as well. We report the imaging characteristics of all focal lesions in liver and spleen in the Dutch GD cohort. Of the 95 GD1 patients, 40% had focal splenic and/or hepatic lesions, associated with more severe GD. Lesions identifi

Splenic circumference at midpregnancy as a predictor of hemoglobin Bart's disease among fetuses at risk.

OBJECTIVE: To determine the accuracy of splenic circumference in predicting hemoglobin (Hb) Bart's disease among fetuses at risk at midpregnancy. MATERIALS AND METHODS: Women with singleton pregnancies with a fetal risk of Hb Bart's disease were enrolled in the study at 18-22 weeks of gestation. All underwent splenic circumference measurement before cordocentesis for fetal blood analysis. The final diagnosis used as a gold standard was based on fetal Hb typing using high-performance liquid chromatography. RESULTS: Among all the pregnancies, the prevalence of Hb Bart's disease was 26.1% (87/3
