The authors study the different parameters of the stapedius reflex on a 55 patients group with retrocochlear and non tumoral diagnostic is assess by electrophysiologic (BERA, Electrocochleography) and neuroradiologic explorations (TAC), Meato-cisternography, NMR). We realise an comparative study between the results of this group and the results of 2 other groups (98 acoustic neuromas and 104 endocochlear diseases). Using the variables auditory threshold, speech audiometry, SISI, ABLB, stapedius reflex, Carhart-test, Békésy audiometry and calorization three factors could be extracted: 1. The quality of peripheral speech-coding in combination with central processing; 2. the degree of damage of the inner ear (threshold, distance between hearing-threshold and reflex-threshold, SISI and calorization); 3. a retrocochlear factor. SISI and ABLB are both tests concerning the inner ear, but must be regarded separately as it is to be supposed that these two findings are caused be two different pathologic processes.

The literature relating to the effects of otological surgery on tinnitus is reviewed. The results of such surgery are often unpredictable with respect to postoperative tinnitus, and ablative surgery may well make the tinnitus worse. The concept of somatosounds (tinnitus arising from outside the auditory pathway) is presented, and the management of some conditions, including patulous Eustachian tube and palatal myoclonus, is discussed.

Stapedius reflexes were tested in 21 deaf patients with the Vienna cochlear implant using intra- or extracochlear electrodes. Sinusoidal bursts of 125, 500, 1,000 and 2,000 Hz were used as stimuli presented in a randomized sequence. The probe tone of an impedance bridge has a direct effect on the results of tympanometry. The frequency of the probe tone should be close to the resonant frequency of the middle ear. Subjective and objective hearing tests are used for evaluation of hearing impairments. Objective methods include impedance measurement with tympanometry and stapedius reflex measurement, otoacoustic emissions (OAE), and auditory evoked potentials (AEP). Combined with statistical analysis, the introduction of auditory steady state responses (ASSR) has enabled objective hearing tests in newborn hearing screening and automated hearing threshold assessment. A novel technique for the diagnosis and management of middle ear myoclonus is described. A patient with middle ear myoclonus underwent a trans-canal microscopic middle ear exploration with injection of botulinum toxin into the stapedius and tensor tympani muscles. We report two patients with pure word deafness (PWD) with tumour in the III ventricle region with obstructive hydrocephalus. Unilateral congenital absence of the stapes suprastructure occurred in association with displaced attachment of the stapedial tendon on an abbreviated incudal lenticular process. The stapedial reflex is routinely used both in hearing evaluation and to distinguish cochlear from retrocochlear lesions. The present study was carried out on seven healthy volunteers in the hope of obtaining basic information about the physiological variables of stapedial reflex using hypoxic hypoxia.
