Hierarchical ventilator-free days with win method in acute respiratory distress syndrome treatment.

BACKGROUND: Ventilator-free days (VFDs), a composite endpoint combining survival and mechanical ventilation duration, are increasingly used in acute respiratory distress syndrome (ARDS) trials to capture patient-centred outcomes beyond mortality. However, its interpretation and analysis are challenged by methodological limitations of conventional statistical methods. This study aims to investigate the interpretability and statistical performance of the hierarchical VFDs compared with the conventional versions for ARDS-related trials. METHODS: We applied a hierarchical composite endpoint frame

Theory for the mixed alkali effect in glasses.

The mixed alkali or mixed mobile ion effect in glasses manifests itself by strong nonlinear variations of ionic transport properties upon mixing of different types of mobile ions. We develop a theory for this effect based on thermally activated hopping transport in disordered site energy landscapes that consistently incorporates the statistical-mechanical and kinetic aspects of a mobile ion mixture. This includes a consideration of the joint probability density of site energy states, generalized Fermi distributions for mean site occupations, and cross terms in the current response described by

Diurnal differences in cerebral microvascular blood flow and oxygen delivery across brain regions in awake mice.

SIGNIFICANCE: Elucidating the diurnal differences in cerebral hemodynamics is essential for both advancing our understanding of brain function and improving therapeutic strategies for neurological disorders. However, it remains unclear how the inactive and active diurnal phases influence the microvascular-scale distributions of cerebral blood flow and oxygenation and whether these distributions exhibit brain-region-specific variations. AIM: We aim to characterize the time-of-day variations in cerebral microvascular blood flow and oxygenation across brain regions in awake mice. APPROACH: We u

Inter-patient image registration algorithms to disentangle regional dose bioeffects.

Radiation therapy (RT) technological advances call for a comprehensive reconsideration of the definition of dose features leading to radiation induced morbidity (RIM). In this context, the voxel-based approach (VBA) to dose distribution analysis in RT offers a radically new philosophy to evaluate local dose response patterns, as an alternative to dose-volume-histograms for identifying dose sensitive regions of normal tissue. The VBA relies on mapping patient dose distributions into a single reference case anatomy which serves as anchor for local dosimetric evaluations. The inter-patient elasti

Shift of the distribution of age in patients with otosclerosis.

Morphological and biochemical investigations have shown evidence of an association between measles virus and otosclerosis. Epidemiological analysis of age and gender distributions in the 1960s and 1970s revealed a higher incidence of otosclerosis in women, the average age of onset of clinical disturbances and need for surgery being between 15 and 40 years. In the late 1960s and early 1970s a campaign to vaccinate children against measles was started in Germany. The aim of our study was to evaluate whether this campaign has had any influence on the distributions of the age and gender of patient

Genetic Susceptibility to Autism Spectrum Disorders: Folate Pathway Polymorphisms, Mediterranean Diet and Evolutionary Insights.

Autism spectrum disorders (ASD) represent a complex medical and social issue with a multifactorial etiology, often correlated with the host genetic background, particularly with the folate metabolic pathway. This study aimed to investigate the association between MTHFR (C677T and A1298C) and MTRR (A66G) polymorphisms and ASD susceptibility in an Apulian cohort. We analyzed 73 patients diagnosed with ASD and 84 healthy controls recruited at the University Hospital Policlinico, Bari. Genotyping was performed using a Real-time/FRET method. Statistical analysis evaluated genotypic/allelic distribu

Estimation of multiple ordered ROC curves using placement values.

In many diagnostic accuracy studies, a priori orders may be available on multiple receiver operating characteristic curves. For example, being closer to delivery, fetal ultrasound measures in the third trimester should be no less accurate than those in the second trimester in predicting small-for-gestational-age births. Such an a priori order should be incorporated in estimating receiver operating characteristic curves and associated summary accuracy statistics, as it can potentially improve statistical efficiency of these estimates. Early work in the literature has mainly taken an indirect ap

A comparison of population size estimators under the truncated count model with and without allowance for contaminations.

The purpose of the study is to estimate the population size under a homogeneous truncated count model and under model contaminations via the Horvitz-Thompson approach on the basis of a count capture-recapture experiment. The proposed estimator is based on a mixture of zero-truncated Poisson distributions. The benefit of using the proposed model is statistical inference of the long-tailed or skewed distributions and the concavity of the likelihood function with strong results available on the nonparametric maximum likelihood estimator (NPMLE). The results of comparisons, for finding the appropr

Mixture proportional hazards cure model with latent variables.

A mixture proportional hazards cure model with latent variables is proposed. The proposed model assesses the effects of the observed and latent risk factors on the hazards of uncured subjects and the cure rate through a proportional hazards model and a logistic model, respectively. Factor analysis is employed to measure the latent variables through correlated multiple indicators. Maximum likelihood estimation is performed through a Gaussian quadratic technique that approximates the integration over the latent variables. A piecewise constant function is used for the unspecified baseline hazard

Copula selection models for non-Gaussian outcomes that are missing not at random.

Missing not at random (MNAR) data pose key challenges for statistical inference because the substantive model of interest is typically not identifiable without imposing further (eg, distributional) assumptions. Selection models have been routinely used for handling MNAR by jointly modeling the outcome and selection variables and typically assuming that these follow a bivariate normal distribution. Recent studies have advocated parametric selection approaches, for example, estimated by multiple imputation and maximum likelihood, that are more robust to departures from the normality assumption c

The importance of distribution-choice in modeling substance use data: a comparison of negative binomial, beta binomial, and zero-inflated distributions.

BACKGROUND: It is important to correctly understand the associations among addiction to multiple drugs and between co-occurring substance use and psychiatric disorders. Substance-specific outcomes (e.g. number of days used cannabis) have distributional characteristics which range widely depending on the substance and the sample being evaluated. OBJECTIVES: We recommend a four-part strategy for determining the appropriate distribution for modeling substance use data. We demonstrate this strategy by comparing the model fit and resulting inferences from applying four different distributions to m

Segregation analysis of idiopathic talipes equinovarus in a Texan population.

"Idiopathic" talipes equinovarus (ITEV) is a nonsyndromal congenital anomaly of one or both feet. Casting and surgery are often necessary to obtain correct foot alignment. In spite of treatment, residual deformities of the feet occur and calf muscles may be hypoplastic. The cause of ITEV is unknown but genetic factors have been postulated. Complex segregation analysis was performed on 173 ITEV families including 93 Caucasian and 48 Hispanic families. The recessive mixed model was the best fitting model and no differences were found based on ethnicity. These results confirm previous studies sug

Left ventricular shape variation in asymptomatic populations: the Multi-Ethnic Study of Atherosclerosis.

BACKGROUND: Although left ventricular cardiac geometric indices such as size and sphericity characterize adverse remodeling and have prognostic value in symptomatic patients, little is known of shape distributions in subclinical populations. We sought to quantify shape variation across a large number of asymptomatic volunteers, and examine differences among sub-cohorts. METHODS: An atlas was constructed comprising 1,991 cardiovascular magnetic resonance (CMR) cases contributed from the Multi-Ethnic Study of Atherosclerosis baseline examination. Issues in biomedical statistics: statistical inference.

The first step in making inferences under the frequentist system of statistical logic is to propose a null hypothesis. An experiment is then performed, or a set of observations made. The resulting data are subjected to statistical analysis to determine whether the null hypothesis should be rejected or not. If it is, then some alternative hypothesis must have been entertained. In biomedical work, the alternative hypothesis should usually be non-specific and it follows that the statistical test of the null hypothesis should be interpreted in a two-sided fashion. The decision to reject or accept

LGTM: Gaussian process modulated neural topic modeling for longitudinal microbiome.

Longitudinal microbiome data are key to understanding the dynamics of microbial communities and their relationships with the host and environment. However, analysis of such data is challenging due to high dimensionality, compositionality, irregular sampling and temporal dependencies on external covariates. Existing analytical approaches typically address only subsets of these challenges, limiting their ability to yield biologically interpretable insights. We introduce LGTM, a probabilistic modeling framework that combines flexible non-linear longitudinal modeling with interpretable topic-based

Robust Heterogeneity Adjustment for Gaussian Graphical Model With Latent Variables.

Graphical models serve as fundamental tools for encoding conditional dependence structures in multivariate biological data, with latent variable Gaussian graphical models playing a pivotal role in capturing complex dependencies in the presence of unobserved confounding variables. However, practical implementations often face two critical challenges: systematic heterogeneity arising from unobserved subpopulations (e.g., tumor subtypes, cell clusters, or patient stratifications) and outliers (e.g., technical artifacts or rare phenotypic variations), both of which can substantially distort the un

Statistical tests for latent class in censored data due to detection limit.

Measures of substance concentration in urine, serum or other biological matrices often have an assay limit of detection. When concentration levels fall below the limit, the exact measures cannot be obtained. Instead, the measures are censored as only partial information that the levels are under the limit is known. Assuming the concentration levels are from a single population with a normal distribution or follow a normal distribution after some transformation, Tobit regression models, or censored normal regression models, are the standard approach for analyzing such data. However, in practice
