Ventriculoperitoneal (VP) shunt migration causing an acute scrotum: a case report of Doppler evaluation.

The diagnosis and management of a scrotal complication of a ventriculoperitoneal shunt simulating testicular torsion in a neonate is outlined. Radiologic evaluation determined the location of the shunt tube and adequate blood flow to the testicle was confirmed by use of the Doppler stethoscope, allowing for definitive therapy by extraperitoneal shortening of the shunt catheter. Surgery to the scrotum was, thus, avoided.

Automatic pediatric congenital heart disease classification based on heart sound signal.

Congenital heart diseases (CHD) are the most common birth defects, and the early diagnosis of CHD is crucial for CHD therapy. However, there are relatively few studies on intelligent auscultation for pediatric CHD, due to the fact that effective cooperation of the patient is required for the acquisition of useable heart sounds by electronic stethoscopes, yet the quality of heart sounds in pediatric is poor compared to adults due to the factors such as crying and breath sounds. This paper presents a novel pediatric CHD intelligent auscultation method based on electronic stethoscope. Firstly, a

Technology for enhancing chest auscultation in clinical simulation.

The ability to use an acoustic stethoscope to detect lung and/or heart sounds, and then to then communicate one's interpretation of those sounds is an essential skill for many medical professionals. Interpretation of lung and heart sounds, in the context of history and other examination findings, often aids the differential diagnosis. Bedside assessment of changing auscultation findings may also guide treatment. Learning lung and heart auscultation skills typically involves listening to pre-recorded normal and adventitious sounds, often followed by laboratory instruction to guide stethoscope p

Acoustic transmission in normal human hips: structural testing of joint symmetry.

An acoustical technique has been developed for the measurement of structural symmetry of the hip joints. A mild vibratory force was applied to the sacrum and sound signals were picked up at both hips by a pair of microphones installed in two stethoscopes. These stethoscope-microphone assembles were calibrated to achieve a difference in relative sensitivity of less than 0.2 dB. The relative transmission of sound signals was analysed and compared between both hips by a dual-channel signal analyser. Twenty-seven healthy adults, 20 healthy pre-school children and 19 normal neonates were tested. Re

MYSTETH-home-based heart monitoring.

INTRODUCTION: The MySteth is an intelligent medical tool designed for cardiac disease screening, utilizing either a stethoscope or smartphone to record heart sounds. Normal heart sounds in healthy individuals consist of "lub" and "dub" noises, while murmurs-additional sounds during heartbeats-can indicate cardiac anomalies such as valve dysfunctions and rapid blood flow, categorized as systolic or diastolic. METHOD: MySteth was developed and tested using heart sounds recorded via smartphone and digital stethoscope. For ensuring the clinical validity of the data, all heart sound samples were m

Voice reduction in cardiac auscultation sounds with reference signals measured from vocal resonators.

This study proposes the use of vocal resonators to enhance cardiac auscultation signals and evaluates their performance for voice-noise suppression. Data were collected using two electronic stethoscopes while each study subject was talking. One collected auscultation signal from the chest while the other collected voice signals from one of the three voice resonators (cheek, back of the neck, and shoulder). The spectral subtraction method was applied to the signals. Both objective and subjective metrics were used to evaluate the quality of enhanced signals and to investigate the most effective

Legacy Building: The Experience of Heartbeat Recordings for Bereaved Caregivers in Pediatrics.

The death of a child is a devastating event that can lead to chronic sorrow and great stress among parents and caregivers. Legacy-building and memory-making experiences for anticipatory grief and bereavement have become increasingly popular in pediatric hospitals, including the use of heartbeat recordings. This intervention created by Brian Schreck at Cincinnati Children's Hospital Medical Center involves audio recording the patient's heartbeat or other respiratory sounds with a digital stethoscope to construct and preserve the patient's legacy, as well as to act as a therapeutic tool.

Necrotic testicle with increased blood flow on Doppler ultrasonic examination.

A twelve-year-old male with a five-day history of scrotal swelling and pain had increased blood flow on ultrasonic examination. A testicular flow and scan indicated an ischemic testicle. Surgical exploration revealed complete torsion of the spermatic cord. In chronic cases of torsion, the Doppler stethoscope may give a false negative result because of reactive hyperemia.

Ultrasound stethoscopy.

Miniaturization and digital techniques have resulted in the development of high-resolution, battery-powered personal ultrasound devices with excellent grey-scale and color blood flow imaging capabilities. These devices are appropriately called "ultrasound stethoscopes" and are practical to use. They extend our physical perception during a clinical examination by "seeing the invisible pathology" and allow the user to address specific clinical problems anywhere at the point-of-care. Murmurs and abnormal precordial movements can be directly related to cardiac structural, functional, and flow abno

Adventitious Pulmonary Sound Detection: Leveraging SHAP Explanations and Gradient Boosting Insights.

Pulmonary illnesses are annually reported as highly prevalent. Patient outcomes can, however, be improved through the aid of automated processes for early diagnoses. This study aims to develop an automated method with a comprehensive explanation for diagnosing underlying adventitious sounds in respiratory diseases. For this purpose, stethoscope audio signal recordings are initially segmented to merely include either normal or adventitious sounds such as wheezing and crackling. A comprehensive feature set representing the temporal and spectral dynamics in the respiratory events is extracted to

Analysis of breath sounds in normal and asthmatic children and adults using computer digitized airway phonopneumography (CDAP).

Analysis of breath sounds using the stethoscope is a major part of physicians evaluation of their patients. However, the use of a stethoscope is often inadequate to give quantitative measurements of the clinical state of the individual. In this study a modification of a previously described computer analysis of breath sounds was used to measure sound intensity levels in both normal and asthmatic children who, in most cases, were unable to perform pulmonary function. The intensity levels were derived using a microcomputer-based program that digitizes audio signals and calculates energy values a

Artificial Intelligence Models for Pediatric Lung Sound Analysis: Systematic Review and Meta-Analysis.

BACKGROUND: Pediatric respiratory diseases, including asthma and pneumonia, are major causes of morbidity and mortality in children. Auscultation of lung sounds is a key diagnostic tool but is prone to subjective variability. The integration of artificial intelligence (AI) and machine learning (ML) with electronic stethoscopes offers a promising approach for automated and objective lung sound. OBJECTIVE: This systematic review and meta-analysis assess the performance of ML models in pediatric lung sound analysis. The study evaluates the methodologies, model performance, and database character

A machine learning approach to the development and prospective evaluation of a pediatric lung sound classification model.

Auscultation, a cost-effective and non-invasive part of physical examination, is essential to diagnose pediatric respiratory disorders. Electronic stethoscopes allow transmission, storage, and analysis of lung sounds. We aimed to develop a machine learning model to classify pediatric respiratory sounds. Lung sounds were digitally recorded during routine physical examinations at a pediatric pulmonology outpatient clinic from July to November 2019 and labeled as normal, crackles, or wheezing. Ensemble support vector machine models were trained and evaluated for four classification tasks (normal

Cardiac auscultation: a glorious past--and it does have a future!

Cardiac auscultation remains an important part of clinical medicine. The standard acoustic stethoscope, which has been useful for more than a century, cannot process, store, and play back sounds or provide visual display, and teaching is hindered because there is no means to distribute the same sounds simultaneously to more than one listener. Modern portable and inexpensive tools are now available to provide, through digital electronic means, better sound quality with visual display and the ability to replay sounds of interest at either full or half speed with no loss of frequency representati

Digital phonocardiography of cardiac arrhythmias in dogs - Preliminary experiences.

Electronic stethoscopes and digital phonocardiograms (DPCGs) can be applied when diagnosing cardiac murmurs, but their use for cardiac arrhythmias is not described in veterinary medicine. Data of 10 dogs are presented in this preliminary study, demonstrating the applicability of these techniques. Although the number of artefacts and the amount of baseline noise produced by the two digitising systems used did not differ, the Welch Allyn Meditron system or similar ones capable of simultaneous recording of electrocardiograms (ECGs) and DPCGs provide a better option for clinical research and educa

Deep Learning for Heart Sound Abnormality of Infants: Proof-of-Concept Study of 1D and 2D Representations.

Introduction: Advanced identification and intervention for Congenital Heart Defects (CHDs) in pediatric populations are crucial, as approximately 1% of neonates worldwide present with these conditions. Traditional methods of diagnosing CHDs often rely on stethoscope auscultation, which heavily depends on the clinician's expertise and may lead to the oversight of subtle acoustic indicators. Objectives: This study introduces an innovative deep-learning framework designed for the early diagnosis of congenital heart disease. It utilizes time-series data obtained from cardiac auditory signals captu

The infant-doctor relationship: an examination of infants' distress reactions in the presence of a doctor.

Fear of doctors is a common source of distress among infants; however, the underlying sources of this distress are unknown. To investigate the doctor-infant relationship, the behaviors of 61 healthy infants (176-617 days old) were observed in a simulated examination room. Their behaviors and electrocardiograms were recorded. Two groups of infants were analyzed: those who cried and those who did not. When an experimenter dressed in the doctor's attire entered the room, all 9 infants who were crying (14.8% of all infants) stopped crying, all infants gazed at the experimenter, and their mean hear
