Neonatal and Maternal Outcomes of Pregnancies following Stillbirth.

OBJECTIVE: Prior stillbirth increases risk of subsequent stillbirth but has unclear effect on subsequent liveborn pregnancies. We examined associations between prior stillbirth, adverse neonatal outcomes, and maternal morbidity in subsequent liveborn pregnancies. STUDY DESIGN: This is a secondary analysis of a large, National Institutes of Health-funded retrospective cohort study of parturients who delivered a singleton infant at a tertiary-care hospital from January 2002 to March 2013 and had a past medical/obstetric history of diabetic, and/or hypertensive disorders, and/or pregnancy with f

Stillbirth And Faith: When Belief And Death Collide.

Mothers who had experienced stillbirth (n = 436) were asked about their "faith base" and whether it was "helpful to their healing." Only 178 (24.5%) indicated their faith base was "helpful;" there was also a wide range of other responses including those who indicated their faith was "lost" following their baby's death. Findings are discussed in terms of what participants found helpful from their pastor and/or church community.

Modifiable risk factors for stillbirth: a literature review.

A stillbirth is defined as an infant born weighing 500 g and/or more or at a gestational age of 24 weeks who shows no signs of life. Having a stillborn baby has a wide range of consequences that can affect parents, family and the healthcare professionals involved. Several risk factors have been associated with an increased risk of stillbirth: including maternal medical factors, maternal characteristics, fetal factors, sociodemographic factors and behavioral factors. The aim of this work is to review the literature on risk factors that have a behavioral component. The main behaviors modulating

Placental examination in nonmacerated stillbirth versus neonatal mortality.

AIM: To retrospectively statistically compare clinical and placental phenotypes of nonmacerated fetuses and live-born perinatal deaths in 3rd trimester pregnancies. METHODS: Twenty-five clinical and 47 placental phenotypes were statistically compared among 93 cases of nonmacerated (intrapartum, or recent antepartum death) 3rd trimester fetal deaths (Group 1), 118 3rd trimester neonatal deaths (Group 2) and 4285 cases without perinatal mortality (Group 3). RESULTS: Sixteen clinical and placental phenotypes were statistically significantly different between Group 3 and the two groups of perina

Toxoplasmosis-associated abortion and stillbirth in Tehran, Iran.

OBJECTIVES: This study was aimed to evaluate the role of toxoplasmosis in etiology of abortion and stillbirth based on molecular and serological techniques. MATERIAL AND METHODS: A total of 110 pregnant women with abortion and stillbirth were enrolled as the case group, and 110 pregnant women with normal delivery were enrolled as the control group. Serological and molecular detections of Toxoplasma gondii were assessed by ELISA and PCR methods. RESULTS: The seroprevalence of IgG was 25.5% in the case group (26.8% in abortion and 21.4% in stillbirth) and 26.4% in the control group. IgM seropo

A triple risk model for unexplained late stillbirth.

BACKGROUND: The triple risk model for sudden infant death syndrome (SIDS) has been useful in understanding its pathogenesis. Risk factors for late stillbirth are well established, especially relating to maternal and fetal wellbeing. DISCUSSION: We propose a similar triple risk model for unexplained late stillbirth. The model proposed by us results from the interplay of three groups of factors: (1) maternal factors (such as maternal age, obesity, smoking), (2) fetal and placental factors (such as intrauterine growth retardation, placental insufficiency), and (3) a stressor (such as venocaval c

Anaemia and stillbirth in Kassala Hospital, Eastern Sudan.

BACKGROUND: Globally, the reduction of stillbirths is a high priority, especially in view of the targets set by the Millennium Development Goals. It is crucial that health policy-makers and programme managers are aware of the epidemiology of stillbirths. OBJECTIVES: This was a case-control study conducted in Kassala hospital in Eastern Sudan to investigate the prevalence and risk factors for stillbirth. Cases were women who delivered stillbirths; two consecutive women who delivered a live-born neonate per case were used as controls. RESULTS: Among 1342 singleton deliveries, there were 44 sti

Placenta and umbilical cord abnormalities seen with stillbirth.

Placental lesions identified in cases of stillbirth are of clinical interest and are frequently invoked as having a causal role. However, most of the placental changes found in stillbirth are also seen in liveborn pregnancies, and are of uncertain pathogenetic significance. Much of the literature addressing placental lesions found in stillbirth is descriptive, listing cases of interest without adequate controls. Further, lesions are described qualitatively, often with inadequate description of examination and sampling protocols. In this manuscript we describe the placental characteristics that

Caffeine intake during pregnancy, late miscarriage and stillbirth.

Caffeine is a commonly consumed drug during pregnancy with the potential to affect the developing fetus. Findings from previous studies have shown inconsistent results. We recruited a cohort of 2,643 pregnant women, aged 18-45 years, attending two UK maternity units between 8 and 12 weeks gestation from September 2003 to June 2006. We used a validated tool to assess caffeine intake at different stages of pregnancy and related this to late miscarriage and stillbirth, adjusting for confounders, including salivary cotinine as a biomarker of smoking status. There was a strong association between c

Maternal age and temporal effects on stillbirth rates.

Stillbirth rates among single and multiple births show markedly decreasing temporal trends. In addition, several studies have demonstrated that the stillbirth rates are dependent on maternal age, in general, showing a U- or J-shaped association with maternal age. In this study, the temporal trends in and the effect of maternal age on the stillbirth rate were considered simultaneously. Our goal was to split the variation into temporal trends and maternal age effects. We applied two-dimensional analysis of variance because no linear association between maternal age and stillbirth rate can be ass

Young maternal age and risk of intrapartum stillbirth.

OBJECTIVES: To determine the risk of intrapartum stillbirth among teen mothers. METHODS: The Missouri maternally linked data containing births from 1978 to 1997 were analyzed. The study group (teen mothers) was sub-divided into younger (<15 years) and older (15-19 years) teenagers. Women aged 20-24 were the referent category. We used Kaplan-Meier product-limit estimator to calculate the cumulative probability of death for each group and the Cox Proportional Hazards Regression models to obtain adjusted hazards ratios. RESULTS: The rate of antepartum and intrapartum stillbirth among teenagers

Increased Stillbirth Rates and Exposure to Environmental Risk Factors for Stillbirth in Counties with Higher Social Vulnerability: United States, 2015-2018.

INTRODUCTION: Exposure to unfavorable environmental conditions during pregnancy, such as extreme heat and air pollution, has been linked to increased risk of stillbirth, defined as fetal mortality at or after 20 weeks' gestation, however no studies have examined its association with social vulnerability. We examined associations between county-level stillbirth rates, environmental risk factors for stillbirth, and social vulnerability in the United States. METHODS: This ecologic study linked county-level data from three nationwide datasets on stillbirths (National Vital Statistics System), env

Sharing experiences to improve bereavement support and clinical care after stillbirth: report of the 7th annual meeting of the International Stillbirth Alliance.

Stillbirth remains a global health challenge which is greatly affected by social and economic inequality, particularly the availability and quality of maternity care. The International Stillbirth Alliance (ISA) exists to raise awareness of stillbirth and to promote global collaboration in the prevention of stillbirth and provision of appropriate care for parents whose baby is stillborn. The focus of this ISA conference was to share experiences to improve bereavement support and clinical care. These issues, relevant throughout the globe, are not discrete but closely interrelated, with both simi

Racialized Economic Segregation and Disparities in the Risk of Stillbirth.

BACKGROUND: While there is growing evidence of the negative impact of neighborhood segregation on maternal and infant health, the evidence for stillbirth is limited. METHODS: A retrospective cohort study of live births and stillbirths for 2007-2020 was conducted in greater Houston, TX, to examine the associations of racialized economic segregation, assessed using the Index of Concentrations at the Extremes (ICE), with stillbirth. Here, ICE contrasted census tract-level racial and economic privilege with disadvantage (i.e., high-income non-Hispanic White persons vs. low-income Persons of Color
