ERCP-associated infected intrahepatic pancreatic pseudocyst.

INTRODUCTION: Pancreatic pseudocysts are abnormal mature collections of pancreatic fluid that can develop in association with acute or chronic pancreatitis. Here, we share the discovery of an infected hepatic subcapsular pseudocyst of the pancreas causing septic shock following endoscopic retrograde cholangiopancreatography (ERCP). PRESENTATION OF CASE: A 55-year-old woman with ethanol-related chronic pancreatitis and biliary stricture was transferred to the ICU for hypotension 8 hours following ERCP. Examination revealed mild right upper quadrant tenderness without sign of peritonitis. Labor

Population structure and characterization of viridans group streptococci (VGS) isolated from the upper respiratory tract of patients in the community.

A study was undertaken to examine the population structure of viridans group streptococci (VGS) isolated the upper respiratory tract of adult and paediatric patients within the community. VGS are common commensal bacterial inhabitants of the upper respiratory tract and valuable sentinel reporters of underlying antibiotic resistance (AR). Laboratory examination of the colonising VGS species may provide a valuable ecological description of the species isolated from the upper respiratory tract and their antibiotic susceptibility, including an estimation of the AR reservoir in this population. Molecular insights into lower respiratory tract microbiota reveal disease-specific biomarkers and shared microbial networks in asthma and COPD.

BACKGROUND: Lower respiratory tract infections (LRTIs) exacerbate chronic airway diseases, yet phenotype-specific microbial signatures are poorly defined. We applied broncho-alveolar lavage fluid (BALF) genomic sequencing to identify biomarkers for asthma and chronic obstructive pulmonary disease (COPD). METHODS: Between December 2023 and February 2025, 1-146 adults with suspected LRTI enrolled from the First Hospital of Jilin University underwent BALF next-generation sequencing. Patients were stratified by lung function, with the impaired pulmonary function group further divided into asthma,

Isolation of bacteria from cervical lymph nodes in patients with oral cancer.

Thirty patients with oral mucosal cancer were studied in relation to oral mucosal damage and bacterial translocation to the regional lymph nodes in the neck. All 30 patients (gingiva 11, tongue 13, cheek mucosa four, oral floor two) underwent extensive, clean-contaminated, head-and-neck surgery (including neck dissection) with free flap reconstruction. A total of 153 lymph nodes was harvested for microbial and histological examination. Viable bacteria were isolated from 70 lymph nodes (45.8%) from 25 patients (83.3%). Bacterial cells in the nodes were detected by scanning electron microscopy.

Povidone-Iodine Antimicrobial Activity In Vitro Against Periodontal Bacterial Pathogens.

Background Povidone-iodine (PV-I) is known to be active in vitro against periodontal bacterial pathogens, but previous studies most often used ≥5-minute contact times for PV-I testing and/or evaluated laboratory reference strains of bacterial species. This study further examined the antimicrobial effects of PV-I by using a 60-second in vitro treatment time for 10% and 5% PV-I on freshly recovered clinical isolates of subgingival biofilm bacteria from severe human periodontitis lesions. Methods Subgingival biofilm samples from 22 adults with severe periodontitis were mixed in vitro with 10% PV-

Phototargeting oral black-pigmented bacteria.

We have found that broadband light (380 to 520 nm) rapidly and selectively kills oral black-pigmented bacteria (BPB) in pure cultures and in dental plaque samples obtained from human subjects with chronic periodontitis. We hypothesize that this killing effect is a result of light excitation of their endogenous porphyrins. Cultures of Prevotella intermedia and P. nigrescens were killed by 4.2 J/cm2, whereas P. melaninogenica required 21 J/cm2. Exposure to light with a fluence of 42 J/cm2 produced 99% killing of P. gingivalis. High-performance liquid chromatography demonstrated the presence of v

"Streptococcus milleri" endocarditis caused by Streptococcus anginosus.

Unlike other viridans streptococci, members of the "Streptococcus milleri group" are often associated with abscess formation, but are only rare causes of infective endocarditis. Although it has been shown that almost all S. intermedius isolates and most S. constellatus isolates, but only 19% of S. anginosus isolates, were associated with abscess formation, no report has addressed the relative importance of the 3 species of the "S. milleri group" in infective endocarditis. During a 5-year period (April 1997 through March 2002), 6 cases of "S. milleri" endocarditis (out of 377 cases of infective

Microbial Detection in Community-Acquired Pleural Space Infections Using the BIOFIRE Joint Infection Panel and Individual Polymerase Chain Reaction Assays.

BACKGROUND: New insights into community-acquired pleural infection microbiology may enable development of more rapid and accessible diagnostics that can streamline treatment. This study systematically compared molecular approaches for pathogen detection in pleural fluid from patients with such infections. Considering the need for rapid results with minimum hands-on time and test complexity, off-label use of the cassette-based, random-access BIOFIRE Joint Infection Panel (BF-JIP, bioMérieux) with and without supplementary in-house polymerase chain reaction (PCR) assays was evaluated. METHODS:

Life-Threatening Orbital and Intracranial Sequelae of Odontogenic Sinusitis: 3 Complex Cases.

BACKGROUND Odontogenic sinusitis (ODS) is increasingly recognized as a distinct cause of secondary rhinosinusitis with an anaerobe-predominant microbiology and a propensity for severe extrasinus complications. Among serious sequelae are orbital and intracranial infections, which can be vision- or life-threatening. CASE REPORT We report 3 cases illustrating the spectrum of complicated ODS. The first patient, a 23-year-old man, developed bacterial meningitis and a brain abscess that required urgent endoscopic sinus surgery with dental extraction. Cultures identified Acinetobacter pittii in denta

Assessment of periradicular microbiota by DNA-DNA hybridization.

In the present study the "checkerboard" DNA-DNA hybridization technique was used to identify bacteria in periapical endodontic lesions of asymptomatic teeth. Thirty-four patients with root-filled teeth and apical periodontitis were divided into two groups, each containing 17 patients. In Group 1, a marginal incision was performed during surgery to expose the lesion, and in Group 2, a submarginal incision was applied. The gingiva and mucosa were swabbed with an 0.2% chlorhexidine gluconate solution prior to surgery. Bacterial DNA was identified in all samples from the two groups using 40 differ

Auto-aggregation in Streptococcus intermedius is driven by the Pel polysaccharide.

The Streptococcus milleri group (SMG), comprising Streptococcus intermedius, anginosus, and constellatus, can asymptomatically colonize various mucosal sites of healthy individuals. These bacteria are opportunistic pathogens that cause different types of infections across various anatomical sites. Although the pathogenic mechanisms leading to infections are not well defined in the SMG, auto-aggregation is a key driver of biofilm adhesion and cohesion in many Streptococci and Staphylococci. Here, we identify a S. intermedius gene cluster with significant homology to the Bacillus cereus and Pseu
