Cerebral abscess secondary to post-esophageal dilation bacteremia: A case report with a pathophysiological association to consider.

BACKGROUND: Accidental caustic ingestion is a pediatric emergency that may result in severe esophageal injury and long-term complications, including strictures requiring repeated dilation. Cerebral abscess is a rare but potentially fatal condition in children and has only sporadically been reported following esophageal dilation. CASE PRESENTATION: We report the case of a 3-year-old girl who developed an esophageal stricture following accidental ingestion of sodium hydroxide. After multiple esophageal dilations, the patient presented with electrolyte abnormalities and neurological symptoms. Ne

Lemierre-like syndrome presenting with extensive intracranial and central venous thrombosis without pulmonary embolism: illustrative case.

BACKGROUND: Lemierre-like syndrome is a rare septic thrombotic disorder caused by non-Fusobacterium pathogens. Presentations with predominantly intracranial involvement and extensive central venous thrombosis are particularly uncommon, and optimal management remains unclear. OBSERVATIONS: A 75-year-old female presented with impaired consciousness, fever, and right exophthalmos with conjunctival injection following mild sore throat. Contrast-enhanced CT demonstrated extensive thrombosis involving the superior vena cava, bilateral internal jugular veins, bilateral transverse-sigmoid sinuses, ca

Lemierre Syndrome: A Rare Complication of Infectious Mononucleosis.

Lemierre syndrome is a rare condition that arises from septic thrombophlebitis most commonly in the internal jugular vein. Common precipitating factors include recent oropharyngeal, tonsillar, or dental infection or chronic bacteremia. While modern antibiotic usage has decreased mortality rates, challenges persist in accurately diagnosing the condition at the time of patient presentation due to a lack of awareness of this rare disorder. Here, we present the case of a previously healthy 20-year-old female patient who was diagnosed with infectious mononucleosis approximately two weeks before her

Volatile profiling distinguishes Streptococcus pyogenes from other respiratory streptococcal species.

Sore throat is one of the most common complaints encountered in the ambulatory clinical setting. Rapid, culture-independent diagnostic techniques that do not rely on pharyngeal swabs would be highly valuable as a point-of-care strategy to guide outpatient antibiotic treatment. Despite the promise of this approach, efforts to detect volatiles during oropharyngeal infection have yet been limited. In our research study, we sought to evaluate for specific bacterial volatile organic compounds (VOC) biomarkers in isolated cultures in vitro, in order to establish proof-of-concept prior to initial cli

Case Study of an Atypical Presentation of a Large Undiagnosed Empyema in the Setting of Hyperglycemia.

A 58-year-old man with a history of type 2 diabetes mellitus was hospitalized for hyperglycemia. During a review of systems, the patient reported additional symptoms of mild, right-sided pleuritic chest pain. Further investigation revealed dullness to percussion with decreased breath sounds and slight tachycardia on physical exam. Additional workup revealed a 16 cm pleural effusion on CT, despite no fever, no leukocytosis, and mild clinical presentation. Thoracentesis findings revealed empyema, and laboratory analysis identified Streptococcus intermedius. Chest tubes were placed and antibiotic

Streptococcus anginosus group brain abscesses and subdural empyemas: exploratory compartment-specific phenotypes and discharge outcomes in a single-center retrospective cohort study.

BACKGROUND: Intracranial suppuration caused by the Streptococcus anginosus group (SAG) includes brain abscess and subdural empyema, but compartment-specific phenotypes and short-term outcomes remain incompletely described. We characterized clinical, radiological, and microbiological features and explored factors associated with poor discharge outcome. METHODS: We conducted a single-center retrospective cohort study of patients with imaging-confirmed intracranial suppuration and SAG identified by culture and/or metagenomic next-generation sequencing from October 2020 to October 2025. Pathogenic bacterial species associated with endodontic infection evade innate immune control by disabling neutrophils.

Endodontic infections, in which oral bacteria access the tooth pulp chamber, are common and do not resolve once established. To investigate the effects of these infections on the innate immune response, we established a mouse subcutaneous chamber model, where a mixture of four oral pathogens commonly associated with these infections (endodontic pathogens [EP]), i.e., Fusobacterium nucleatum, Streptococcus intermedius, Parvimonas micra, and Prevotella intermedia, was inoculated into subcutaneously implanted titanium chambers. Cells that infiltrated the chamber after these infections were primar

Bacteria and their toxins tamed for immunotherapy.

Bacterial toxins share the ability to enter host cells to target various intracellular proteins and to modulate host immune responses. Over the last 20 years, toxins and their mutated variants, as well as live attenuated bacteria, have been exploited for vaccination and immunotherapy of various infectious, malignant and autoimmune diseases. The ability of Bordetella pertussis adenylate cyclase toxin to translocate its adenylate cyclase domain across the host cell membrane, as well as the pathways of intracellular trafficking of Bacillus anthracis lethal and edema toxins, Shigella dysenteriae s

Orbital Abscess With Vision Threatening Optic Neuropathy Due to Odontogenic Sinusitis in Adolescence: A Case Report and Literature Review.

Orbital abscesses secondary to odontogenic infections are rare but can lead to serious complications, including compressive optic neuropathy and permanent vision loss, if not diagnosed and treated promptly. We present the case of a 13-year-old child with a radiologically confirmed orbital abscess associated with a recent odontogenic infection. The patient initially presented with a one-week history of right eyelid swelling and fever. The symptoms began after treatment for dental caries, during which the patient was prescribed a three-day course of oral antibiotics. Despite emergency pulp extir

Pulmonary granulomas and Mendelson syndrome in an immunocompromised patient.

Granuloma formation is generally correlated with infection. Pulmonary granulomas caused by foreign bodies aspiration are uncommon. The clinical and radiologic features of such cases often lack specificity, which makes it difficult to distinguish from malignancy. Aspiration is usually not considered in the differential diagnosis of patients presenting with mass-like abnormalities on chest imaging. Occult aspiration history, diverse clinical manifestations, atypical imaging findings, and limited availability of pathogen detection techniques make the precise diagnosis a substantial challenge. Milleri group streptococcus--a stepchild in the viridans family.

The purpose of this investigation was to provide a comprehensive review of the pathogenic role and spectrum of disease of milleri group streptococci, with special attention to bloodstream invasion and to possible differential roles among the three species. All consecutive isolates of milleri group streptococci from any anatomic source, during a 37-month period, in a tertiary care teaching hospital in Tel-Aviv, Israel, were thoroughly investigated. Identification to the species level was performed by an automated system.Streptococcus anginosus constituted 82% of the 245 patient-unique isolates

Multiple brain abscesses with good prognosis in an infant with cyanotic congenital heart disease: a case report.

BACKGROUND: Brain abscesses are relatively rare, but they are a potentially life-threatening condition. Predictive factors for poor outcome are a young age and the presence of multiple abscesses. We report a case of a 15-month-old girl with cyanotic congenital heart disease who developed multiple brain abscesses caused by Streptococcus intermedius. The patient was treated with a combination of surgical aspiration and antimicrobial therapy without apparent neurological sequelae. To the best of our knowledge, this is the youngest such patient to have been reported in the literature. We explore t

Surgical Management of a Pediatric Infratentorial Subdural Empyema as a Complication of Parapharyngeal Abscess.

Infratentorial empyema is a rare medical emergency typically presenting secondary to a middle ear infection. Nonspecific symptoms, limited access to radiological facilities, and imaging artifacts render this pathology prone to misdiagnosis and delayed intervention. An 11-year-old girl presented to the emergency department with a high fever, cervicalgia, and a two-week history of frontal headache. Computed tomography revealed parapharyngeal abscess and polysinusitis. Pus drained from the parapharyngeal abscess showed Staphylococcus capitis and Streptococcus intermedius. Bilateral tubo-ovarian abscesses four years after total abdominal hysterectomy.

BACKGROUND: Pelvic inflammatory disease (PID) is a common gynecologic disorder. One known complication of PID is tubo-ovarian abscess (TOA) formation. The predominant theory on TOA formation postulates that an ascending infection from the cervix through the uterus to the fallopian tubes and ovaries results in abscess formation. Other theories include seeding via a hematogenous infection, diverticular disease, and appendicitis. CASE: A 39-year-old female patient with abdominal pain was referred to our institution and was found to have a pelvic mass. After a thorough evaluation, surgical explor

Long-term bacterial exposure can trigger nonsuppurative destructive cholangitis associated with multifocal epithelial inflammation.

Bacterial infection has become a focus of attention in the pathogenesis of primary biliary cirrhosis (PBC). We earlier reported that the bacterial lipoteichoic acid was detected at the sites of inflammation around damaged bile ducts in the livers of PBC, and PBC patients' sera showed high titers against streptococcal histone-like protein. Here, we investigated whether chronic bacterial exposure could trigger PBC-like epithelial cell damage in normal mouse. BALB/c mice were repeatedly inoculated with various bacteria for 8 weeks. At 1 week (Group 1) and 3, 4, or 20 months (long term; Group 2) a
