Role of traumatic subdural fluid collection in developing process of chronic subdural hematoma.

We investigated the role of traumatic subdural fluid collection in the genesis of chronic subdural hematoma (CSDH) in 43 consecutive patients with traumatic subdural fluid collection. Traumatic subdural fluid collection was found in 43 (6%) of 715 patients who underwent CT scans because of head injury. Twenty-four of the 43 patients were more than 65 years of age and this problem occurred in 30% of the 79 head-injured patients over 65 years of age. In 20 (Group 1) of the 43 patients CSDH subsequently developed. During the same period, 30 other patients were treated for CSDH. Twenty-four (Group

Bilateral subdural hematomas in an adult with osteogenesis imperfecta.

Osteogenesis imperfecta (OI) is a rare inherited disorder characterized by disruption of normal collagen formation, brittle bones, and low bone density, predisposing to multiple bone fractures and wide-spectrum symptomatology. Extradural and subdural hematomas and spinal cord injuries have been reported but mainly in children with OI. We cared for a 60-year old woman with OI and bilateral nontraumatic subdural hematomas possibly caused by spontaneous intracranial hypotension.

Neuroradiographic abnormalities in congenital cytomegalovirus infection.

To determine the spectrum of neuroradiographic abnormalities associated with congenital cytomegalovirus (CMV) infection, CT brain scans of 15 infants with symptomatic infection were reviewed. The initial CT scans were abnormal in 13 patients. Abnormalities included intracranial calcifications, cortical atrophy, ventricular enlargement, subdural effusions, porencephaly and polycystic encephalomalacia. Intracranial calcifications were present in 33% of the infants. In addition, three of the 15 infants developed progressive hydrocephalus which required ventriculoperitoneal shunt placement. These

Bacterial meningitis in infancy and childhood in Lusaka (One year prospective sturdy).

In approximately 10,000 admissions in a 12 months period, at University Teaching Hospital, Lusaka 85 cases of meningitis were recorded. The signs and symptoms in these patients do not greatly differ from other similar studies in Africa. The commonest organism isolated was pneumococcus. There was high mortality rate which was to a large extent due to parents not bringing their children to hospital early enough for medical treatment to be instituted. This is borne out by the fact that 50% of the children with meningitis died within the first 24 hours. The C.S.F. protein and peripheral white bloo

Multiple etiologies of secondary headaches associated with arachnoid cyst, cerebrospinal fluid hypovolemia, and nontraumatic chronic subdural hematoma in an adolescent: A case report.

BACKGROUND: Diagnosing the cause of headaches can be challenging. Even if intracranial lesions are found in a patient, careful assessment is essential for diagnosis, and treatment strategies will differ for each etiology. CASE DESCRIPTION: A 16-year-old boy presented with sudden-onset headache which had lasted for 2 days. His headache was aggravated in the orthostatic position. The patient denied recent head trauma. He had been diagnosed with an arachnoid cyst (AC) in his right middle cranial fossa. Computed tomography (CT) revealed bilateral subdural effusions and slit-like lateral ventricle

[Clinical study of chronic subdural hematoma and hygroma in two hospitals for elderly patients with dementia].

The authors have investigated the incidence of chronic subdural hematoma and hygroma (CSH) in two hospitals for elderly patients with dementia syndrome over a period of three years and nine months. All observed patients had been in the hospital for 2 weeks or more and had had brain computed tomography (CT) on admission. The total of 613 patients included 212 males and 401 females. Based on the CT findings observed in patients on admission, the authors found 47 patients with CSH. This was equivalent to 7.7% of all patients observed. After admission, almost all patients had brain CT twice per ye

Insertion of a ventriculo-biliary shunt in a 3-year-old complicated by subdural biliary hygromas: a case report and review of the literature.

INTRODUCTION: Hydrocephalus is commonly treated by diverting cerebrospinal fluid (CSF) from the ventricles through a shunt system to a distal site, such as the peritoneum, right atrium, or pleural space. When CSF diversion to these sites is contraindicated, the gallbladder is an alternative terminus. Ventriculo-biliary shunts, in which the distal tip of the shunt catheter is placed into the gallbladder through the cystic duct, are one such alternative. There are a paucity case reports describing the adverse effects of these shunts. CASE PRESENTATION: Here, we describe the case of a 3-year-old

Treatment of post-dural puncture headache with bilateral greater occipital nerve block.

(Headache 2010;50:869-881) Epidural blood patch is an effective treatment with a low complication rate. It is also an invasive method that can cause permanent neurological sequelae such as early and late back pain, radiculopathy, spinal-subdural hematoma, spinal-epiarachnoid hematoma, intrathecal hematoma, arachnoiditis, and infection. We report a case in which a postdural puncture headache resolved within 2 minutes of a greater occipital nerve block, a minimally invasive and easy procedure with a low complication rate. This case report suggests that a greater occipital nerve block may be a su

[Third cranial nerve palsy due to intracranial hypotension syndrome].

INTRODUCTION: Intracranial hypotension syndrome (IHS) is clinically defined by orthostatic headache due to cerebrospinal fluid (CSF) hypovolemia. It is frequently associated with lower cranial nerve (CN) paresis, especially the 6th one. However, neuropathy of the 3rd CN is rare. CASE REPORT: A 43 year old man presented with orthostatic headache and vertical diplopia of gradual onset. Clinical examination detected incomplete, pupil sparing left third CN palsy. Magnetic resonance imaging (MRI) of the brain and spine demonstrated typical abnormalities that confirmed the clinical suspicion of IHS

Does hearing loss after spinal anesthesia differ between young and elderly patients?

UNLABELLED: Fifty male patients scheduled for inguinal hernia repair with spinal anesthesia were included in this study. The patients were divided into two groups: 25 patients aged 30 yr or younger (Group Y) and 25 patients aged 60 yr or older (Group E). We performed subarachnoidal injection at the L3-4 interspace by using a 25-gauge Quincke needle with the patient in the sitting position, and 3 mL of 0.5% isobaric bupivacaine was administered. Patients were evaluated by pure tone audiometry (LdB [low frequencies], 125-500 Hz; SdB [speech frequencies], 500-2000 Hz; HdB [high frequencies], 2000

Intracranial complications of frontal sinusitis.

Frontal sinus infections can spread to the intracranial space so fast that the clinical situation often becomes far advanced before a complication is recognized. Retrograde septic thrombophlebitis is the most common pathway of extension. A review of recent experiences with fulminating frontal sinusitis and its intracranial complications such as subdural empyema, brain abscess, epidural abscess, and meningitis is presented. Neurologic features of intracranial invasion are interpreted. Good results have been achieved by immediate and aggressive surgical and medical measures.

Rupture of middle fossa arachnoid cyst simulating new-onset migraine with aura.

Arachnoid cysts represent a common, innocent, finding in routine neuroimaging of headache patients. We present the first report of symptomatic migraine with aura caused by the spontaneous rupture of a middle fossa arachnoid cyst into the subdural space. Brain imaging enabled an accurate diagnosis and, subsequently, adequate surgical management.

Minicraniotomy versus bur holes for evacuation of chronic subdural collections in infants-a preliminary single-institution experience.

OBJECT: Various surgical interventions have been described to evacuate chronic subdural collections (CSCs) of infancy. These include transfontanel percutaneous aspiration, subdural drains, placement of bur hole(s) with or without a subdural drain, and shunting. Shunt placement typically provides good long-term success (resolution of the subdural fluid), but comes with well-known early and late complications. Recently, the authors have used a mini-osteoplastic craniotomy technique with the goal of definitively treating these children with a single surgery while avoiding the many issues associat

Neurological cause of late postpartum seizures.

A 32-year-old primigravida showed signs of pre-eclampsia before delivery of a healthy boy at term. The CSF-space was accidentally punctured during epidural anaesthesia in labour. One day later hypertension was noted and the patient had a single generalized fit. For the next three weeks she had postural headaches, fluctuating hypertension, intermittent hearing loss and double-vision. On the 22nd day of postpartum, the patient had the first of a series of partial and later generalized seizures, followed by hemiparesis, alteration of consciousness, and finally slow recovery with corticosteroid th

Surgical management of bacterial meningitis.

A variety of associated lesions may require the neurosurgeon's assistance in the management of bacterial meningitis. As treatment of this infection of the central nervous system proceeds, the surgeon will have to decide about the concurrent or subsequent operative treatment of congenital dysraphic states, paraneural infections, compound fractures or penetrating wounds of thecranium or spine, or infected bypass shunts for cerebrospinal fluid (CSF). In patients with intractable meningitic infections the surgeon may have to insert a ventricular drainage-irrigation system to permit adequate perfus

[Post-operative complications peculiar to lumboperitoneal shunt: possible consequences due to side leakage of CSF from around the inserted spinal tube into the lumbar epidural space].

This is a report on a few possible consequences of occasional side leakage of CSF into the lumbar epidural space. In this report, we refer to such CSF side leakage from around the inserted spinal tube which occurred during the early stages following our lumboperitoneal shunt operations in two of our patients. This leakage can lead to over-drainage of CSF, and such complications as post-operative orthostatic headache, subdural effusion, and/or subdural hematoma can also result. The diagnosis of these complications can be verified if CSF components in the lumbar epidural space are visible in T2

Meningitis; a review of one year's cases from an active children's hospital service.

Fifty-six cases of bacterial meningitides, other than tuberculous, observed in a period of one year on an active children's hospital medical service were reviewed. All age groups were involved but most commonly infected were patients in the first year of life. There was no inordinate incidence of infection with any one kind of organism in any specific age bracket, except possibly for infection with enteric organisms which in the present series occurred preponderantly in infants less than six months old. Meningococcus and H. influenza type B were the most common organisms; they were the infecti
