Reconstruction of recalcitrant pressure ulcers with latissimus dorsi muscle-splitting free flaps.

BACKGROUND: Reconstruction of recalcitrant pressure ulcers is very challenging because all available local tissues have been exhausted. Although occasionally suggested as reconstructive options in some reports, free flaps are still not favored for pressure ulcers because of the less available recipient vessels in buttock area and the need for position change. Here, we describe our experience with latissimus dorsi muscle-splitting free flaps harvested in prone position for recalcitrant pressure ulcers. METHODS: Between January 2012 and January 2020, 10 patients of recalcitrant pressure ulcers

Enhancement of External Rotation after Latissimus Dorsi Tendon Transfer (LDTT): A Cadaveric Study.

Background and objectives: Massive rotator cuff tears compromise shoulder mobility function and cannot be directly repaired. Latissimus dorsi tendon transfer (LDTT) is a therapeutic alternative suitable for the treatment of rotator cuff tears that helps to restore external shoulder rotation. Cadaver models have been used for studying the effects of LDTT and procedural variations, but, to the best of our knowledge, none of them have been validated. The aim of our study was to validate a novel cadaver model while verifying the effects of LDTT on external rotation. Materials and Methods: Two grou

MRI in migraineurs: are there abnormalities in the area where the myofascial trigger points are palpable and in volume measurements?

INTRODUCTION: Patients with migraine may present a higher quantity of myofascial trigger points (MTrP) and alterations in the cervical muscles when compared to non-migraineurs. The magnetic resonance imaging (MRI) is a robust method for the study of human soft tissues and could be useful to investigate these points. OBJECTIVES: To identify the presence of MTrP in the descending fibers of the trapezius muscle in women with migraine and to quantify the muscle volume by MRI, correlating it with the headache characteristics. METHODS: A cross-sectional analytic study was conducted among 14 women,

Comparison between dry needling plus manual therapy with manual therapy alone on pain and function in overhead athletes with scapular dyskinesia: A randomized clinical trial.

Muscles' trigger points can induce scapular dyskinesia (SD) which interferes with overhead athletes' professional training. We aimed to evaluate effects of dry needling (DN) alone and plus manual therapy (MT) on pain and function of overhead athletes with SD. 40 overhead athletes (15 male, 25 female) aged 18-45 with at least 3 points Numeric Rating Scale (NRS) pain intensity during training were recruited and randomly allocated to the treatment group: MT followed by DN on trigger points of Subscapularis, Pectoralis minor, Serratus anterior, upper and lower Trapezius muscles; or the control gro

Traumatic Tear of the Latissimus Dorsi Myotendinous Junction: Case Report of a CrossFit-Related Injury.

A case of a latissimus dorsi myotendinous junction strain in an avid CrossFit athlete is presented. The patient developed acute onset right axillary burning and swelling and subsequent palpable pop with weakness while performing a "muscle up." Magnetic resonance imaging examination demonstrated a high-grade tear of the right latissimus dorsi myotendinous junction approximately 9 cm proximal to its intact humeral insertion. There were no other injuries to the adjacent shoulder girdle structures. Isolated strain of the latissimus dorsi myotendinous junction is a very rare injury with a scarcity

Effects of different working postures and pointing devices on energy expenditure, upper limb and back muscles activity, task performance and perception.

This study simultaneously compared four working postures (sitting on an office chair, on a ball, standing and pedalling on an ergocycle) and five pointing device configurations (traditional and slanted mouse placed next to and in front of the keyboard, and central pointing device) in relation to sedentary behaviour and musculoskeletal disorders. Pedalling on an ergocycle was the only posture that exceeded the sedentary behaviour threshold of 1.5 METs. This posture resulted in the highest task completion time and was least favoured by participants. Regardless of the pointing device configuratio

Different ways to balance the spine in sitting: Muscle activity in specific postures differs between individuals with and without a history of back pain in sitting.

BACKGROUND: Previous research explored muscle activity in four distinct sitting postures with fine-wire electromyography, and found that lumbar multifidus muscle activity increased incrementally between sitting with flat thoracolumbar and lumbar regions, long thoracolumbar lordosis, or short lordosis confined to the lumbar region. This study used similar methods to explore whether people with a history of low back pain provoked by prolonged sitting used different patterns of trunk muscle activity in specific postures. METHODS: Fine-wire electromyography electrodes were inserted into the right

Division of the spinal accessory nerve in the anterior triangle: a prospective clinical study.

The anatomical relation between the spinal accessory nerve and internal jugular vein is well documented, but other variants of the nerve, including the contributions of the cervical plexus to supply motor fibres to the trapezius, are less well known. We have previously described an anatomical variant in which the spinal accessory nerve divided before entering the sternocleidomastoid, and the inferior trunk passed directly under it to supply the trapezius. We now present a prospective study of 133 neck dissections (excluding radical dissections) in which a meticulous search was made for the var

Electromyographic Analysis of Paraspinal Muscles of Scoliosis Patients Using Machine Learning Approaches.

A large number of studies have used electromyography (EMG) to measure the paraspinal muscle activity of adolescents with idiopathic scoliosis. However, investigations on the features of these muscles are very limited even though the information is useful for evaluating the effectiveness of various types of interventions, such as scoliosis-specific exercises. The aim of this cross-sectional study is to investigate the characteristics of participants with imbalanced muscle activity and the relationships among 13 features (physical features and EMG signal value). A total of 106 participants (69%

The Effects of Isometric Fatigue on Trunk Muscle Stiffness: Implications for Shear-Wave Elastography Measurements.

Muscle stiffness has been implicated as a possible factor in low back pain risk. There are few studies on the effects of isometric fatigue on the shear modulus of trunk muscles. This study aimed to investigate the effects of trunk isometric fatigue on the passive and active (during low and high-level contractions) shear moduli of the erector spinae (ES) and superficial and deep multifidus (MF) muscles. We assessed passive and active shear modulus using shear-wave elastography in healthy young participants (n = 22; 11 males, 11 females), before and after an isometric trunk extension fatigue pro

Reliability of bioimpedance spectroscopy in the upper trapezius muscle.

OBJECTIVE: Localized bioimpedance spectroscopy (BIS) is a non-invasive technique for assessing muscle tissue properties. This study examined the test-retest reliability of BIS measurements on the upper trapezius muscle and the effects of body positioning and electrode reapplication. APPROACH: Ten healthy adults (ages 18-30) with no history of neck or shoulder pain participated in the study. BIS measurements were collected over two days across three body positions (prone, seated, seated with neck bent) and with different electrode applications. Key impedance parameters were extracted from Cole

Intramuscular Cavernous Hemangioma of the Latissimus Dorsi Muscle: A Rare Presentation of Chronic Musculoskeletal Pain - Case Report.

BACKGROUND Hemangiomas comprise 7% of benign soft tissue tumors, and only 0.8% of hemangiomas are intramuscular; 26% of intramuscular hemangiomas comprise the cavernous histologic variant. Intramuscular cavernous hemangiomas are benign soft tissue tumors of unknown pathophysiology. They are most commonly described in the extremities, with few reports involving the thorax. CASE REPORT A 53-year-old Chinese woman - previously healthy with no clinically significant comorbidities - presented with a chronic, painful swelling of the right lateral chest wall. Magnetic resonance imaging revealed a lob

Contralateral Lower Trapezius to Triceps Transfer for Restoring Elbow Extension in Patients with Brachial Plexus Palsy - A Technical Note.

Restoration of elbow extension is seldom considered in the reconstructive plan for patients with extensive brachial plexus injury because of the scarcity of the nerve and tendon transfer options. However, restoring elbow extension could increase the outreach of hand and give better control of the elbow flexion. We propose a new technique for restoring elbow extension using the readily available contralateral lower trapezius (CLT). By extending the CLT with a fascia lata graft and transferring it to the triceps, we were able to restore elbow extension. In all four patients who underwent this pr

Effect of scapular posterior tilting exercise on scapular muscle activities in men and women with a rounded shoulder posture.

Round-shoulder posture (RSP) is a common postural condition, characterized by protraction, downward rotation, anterior tilting and internal rotation of the scapula. RSP can lead to shoulder dysfunction. Different methods have been proposed for rehabilitating and correcting the altered posture in RSP including stretching, strengthening exercises, and shoulder brace or taping. However, the findings are controversial and studies are ongoing to develop more effective method. The present study is aimed at investigating the effects of scapular posterior tilting (SPT) exercise in different support po

Absence of the sternocleidomastoid muscle in a patient that underwent neck dissection for squamous cell carcinoma of the tongue.

The congenital absence of a skeletal muscle is a rare cause of congenital muscular torticollis, and the condition is associated with various unusual anatomical structures. We describe a rare case of congenital absence of the sternocleidomastoid muscle associated with squamous cell carcinoma of the tongue in a patient who underwent neck dissection. In this case, both the external jugular vein and the spinal accessory nerve were absent. However, we found that branches of the C3 nerve extended from the cervical plexus to the trapezius muscle and seemed to be acting as trapezius muscle motor nerve
