Closed-loop stimulation modulates attention shifting in children.

Spontaneous fluctuations in attention can impede adaptation to changing goals and environments. Endogenous control over attentional shifts, referred to as attentional flexibility, is prone to disruption in children with attention deficit disorders. Here we studied in vivo intracranial recordings in children with epilepsy to identify a reproducible neural signature of attentional control that could predict and prevent impending lapses in real time. Machine learning classifiers were trained on intracranial signals while each child performed an attentional set-shifting task and predicted delays i

Motor inhibition and cognitive flexibility in eating disorder subtypes.

Anorexia Nervosa (AN) and Bulimia Nervosa (BN) are complex Eating Disorders (EDs). Even if are considered two different diagnostic categories, they share clinical relevant characteristics. The evaluation of neurocognitive functions, using standardized neuropsychological assessment, could be a interesting approach to better understand differences and similarities between diagnostic categories and clinical subtypes in EDs thus improving our knowledge of the pathophisiology of EDs spectrum. This study explored cognitive flexibility and motor inhibition in patients with AN considering both Restric

Guideline-Based Cochlear Implant Care: A Model for Structuring Delegated Technical Aftercare.

Cochlear implant (CI) care in Germany is largely standardized through the national CI clinical practice guideline, the CI White Paper, and established certification procedures. Lifelong aftercare is essential for maintaining long-term treatment outcomes. In particular, technical aftercare may, in accordance with current guidelines, be delegated by a cochlear implant providing department (CIVE) to qualified external cooperation partners (QEK). To preserve the CIVE's overall responsibility, a clearly structured framework for collaboration is required. The aim of this manuscript is to present a p

Capacity building through workforce training and community engagement for Surgical, Obstetric, Trauma, and Anaesthesia (SOTA) care: Case studies from South Asia.

South Asia faces a critical deficit in surgical, obstetric, trauma, and anaesthesia (SOTA) care, with over 1.6 billion people lacking access to safe and affordable surgical services. Addressing this inequity requires urgent workforce capacity building beyond traditional specialization. This study, conducted by the G4 Alliance Asia Working Group, employs a Multiple-Case Study design utilizing Cross-Case Synthesis to present six initiatives from Nepal, Bhutan, Pakistan, and India, mapped against a SOTA care workforce capacity building framework. We describe initiatives by training type (programm

Transcranial direct current stimulation combined with caffeine promotes executive function in healthy females.

BACKGROUND: Studies have demonstrated that both transcranial direct current stimulation (tDCS) and caffeine improve cognitive abilities through similar mechanisms. This study investigated the acute effects of tDCS combined with caffeine on executive functions. METHODS: Eighty females were randomly assigned to four groups (tDCS + caffeine, tDCS + placebo, sham tDCS + caffeine, and sham tDCS + placebo). Each participant completed two experimental sessions. In the first session, participants performed the Stroop, 3-back, and More-Odd Shifting tasks (T0). For the second session, participants inge

Performance and brain activity during the Wisconsin Card Sorting Test in adolescents with obsessive-compulsive disorder and adolescents with weight-restored anorexia nervosa.

Anorexia nervosa (AN) and obsessive-compulsive disorder (OCD) both show a peak age of onset during adolescence and share a number of phenotypic features, such as rigid rule-bound behavior and perseverative thinking. There is evidence of difficulties with set shifting or task switching in adults with each disorder, but evidence in adolescents is limited. Furthermore, no studies have previously directly compared AN and OCD on this cognitive process or examined comparative neural correlates. This study provides exploratory analyses to address this gap by measuring brain activity with functional m

On the interplay between state-dependent reconfigurations of global signal correlation and BOLD fluctuations: An fMRI study.

BACKGROUND: The dynamics of global, state-dependent reconfigurations in brain connectivity are yet unclear. We aimed at assessing reconfigurations of the global signal correlation coefficient (GSCORR), a measure of the connectivity between each voxel timeseries and the global signal, from resting-state to a stop-signal task. The secondary aim was to assess the relationship between GSCORR and blood-oxygen-level-dependent (BOLD) activations or deactivation across three different trial-conditions (GO, STOP-correct, and STOP-incorrect). METHODS: As primary analysis we computed whole-brain, voxel-

Virtual reality-based balance training system augmented with operant conditioning paradigm.

BACKGROUND: Stroke-related sensory and motor deficits often steal away the independent mobility and balance from stroke survivors. Often, this compels the stroke survivors to rely heavily on their non-paretic leg during weight shifting to execute activities of daily living (ADL), with reduced usage of the paretic leg. Increased reliance on non-paretic leg often leads to learned nonuse of the paretic leg. Therefore, it is necessary to measure the contribution of individual legs toward one's overall balance. In turn, techniques can be developed to condition the usage of both the legs during one'

Staying and shifting patterns across IGT trials distinguish children with externalizing disorders from controls.

The Iowa Gambling Task (IGT) is the most widely instrument used in the assessment of affective decision-making in several populations with frontal impairment. The standard performance measure on the IGT is obtained by calculating the difference between the advantageous and the disadvantageous choices. This standard score does not allows the assessment of the use of different strategies to deal with contingencies of gain and losses across the task. This study aims to compare the standard score method used in IGT with a method that analyses the patterns of staying and shifting among different de

Selective extra-dimensional set shifting deficit in a knock-in mouse model of Huntington's disease.

People with early-stage Huntington's disease have been found to have a specific deficit in performing an extra-dimensional shift. To date no evidence of this deficit has been identified in transgenic or knock-in rodent models of the disease. The aim of the present paper then, was to test whether homozygous knock-in mice derived from the Hdh(CAG(150)) mouse line were impaired in any of five 2-choice discrimination tasks (simple, compound, compound reversal, intra-dimensional shift and extra-dimensional shift), and whether these mice were impaired at recalling these tasks on the following day. Neurocognitive Profile of Avoidant/Restrictive Food Intake Disorder: Cognitive Flexibility, Delay Discounting, and Detail Orientation.

OBJECTIVE: Neurocognitive deficits are well-documented across psychiatric disorders and may indicate novel treatment targets. However, empirical data on the neurocognitive profile of avoidant/restrictive food intake disorder (ARFID) are lacking. To address this gap, this study examined three core neurocognitive domains altered in other eating disorders-cognitive flexibility, delay discounting, and detail orientation-in adults with ARFID and healthy controls (HCs). METHOD: Adults (18-45 years) with ARFID (n = 49) and HCs (n = 21) completed computerized tasks assessing cognitive flexibility (Ca

Simulating category learning and set shifting deficits in patients weight-restored from anorexia nervosa.

OBJECTIVE: To examine set shifting in a group of women previously diagnosed with anorexia nervosa who are now weight-restored (AN-WR) and then apply a biologically based computational model (Competition between Verbal and Implicit Systems [COVIS]) to simulate the pattern of category learning and set shifting performances observed. METHOD: Nineteen AN-WR women and 35 control women (CW) were administered an explicit category learning task that required rule acquisition and then a set shift following a rule change. COVIS was first fit to the behavioral results of the controls and then parameters

Cognition in trichotillomania: a meta-analysis.

OBJECTIVE: Trichotillomania (TTM) is a mental health disorder characterized by repetitive urges to pull out one's hair. Cognitive deficits have been reported in people with TTM compared to controls; however, the current literature is sparse and inconclusive about affected domains. We aimed to synthesize research on cognitive functioning in TTM and investigate which cognitive domains are impaired. METHODS: After preregistration on the International Prospective Register of Systematic Reviews (PROSPERO), we conducted a comprehensive literature search for papers examining cognition in people with

The disengage deficit in hemispatial neglect is restricted to between-object shifts and is abolished by prism adaptation.

We sought to determine the effects of prism adaptation on peripherally cued visual attention shifting in patients with spatial neglect, using a task devised by Egly et al. (J Exp Psychol Gen 123:161-177, 1994) based on the classic Posner paradigm. This task allowed a comparison of "within-object" versus "between-object" attention shifts. A display was presented containing two parallel outline rectangles, and subjects were asked to make rapid responses to a target, which would appear at one end of one of the rectangles. The target location was pre-cued with 75% validity: on invalid trials atten

Adapted to explore: reinforcement learning in Autistic Spectrum Conditions.

Recent studies have recorded a tendency of individuals with Autism Spectrum Conditions (ASC) to continually change their choices in repeated choice tasks. In the current study we examine if this finding implies that ASC individuals have a cognitive style that facilitates exploration and discovery. Six decision tasks were administered to adolescents with ASC and matched controls. Significant differences in shifting between choice options appeared in the Iowa Gambling task (Bechara, Damasio, Damasio, & Anderson, 1994). A formal cognitive modeling analysis demonstrated that for about half of the

Uterine Tonus Assessment by Midwives versus Patient self-assessment in the active management of the third stage of labor (UTAMP): study protocol for a randomized controlled trial.

BACKGROUND: Postpartum hemorrhage (PPH) is the leading cause of maternal mortality worldwide and accounts for one third of maternal deaths in low-income and middle-income countries. PPH can be prevented by active management of the third stage of labor (AMTSL), a series of steps recommended by the World Health Organization to be performed by skilled birth attendants (SBAs). Task shifting in the AMTSL step of uterotonic drugs administration to community health workers, traditional birth attendants and self-administration has been investigated as a strategy to increase access to quality obstetric

Rumination impairs the control of stimulus-induced retrieval of irrelevant information, but not attention, control, or response selection in general.

The aim of the study was to throw more light on the relationship between rumination and cognitive-control processes. Seventy-eight adults were assessed with respect to rumination tendencies by means of the LEIDS-r before performing a Stroop task, an event-file task assessing the automatic retrieval of irrelevant information, an attentional set-shifting task, and the Attentional Network Task, which provided scores for alerting, orienting, and executive control functioning. The size of the Stroop effect and irrelevant retrieval in the event-five task were positively correlated with the tendency

Task-sharing anaesthesia in conflict zones: a cross-sectional study of safety gaps and systemic failures in Wad Medani, Sudan.

BACKGROUND: In Sudan, where physician anaesthesiologists are critically scarce (0.47/100,000 population), non-physician anaesthesia providers (NPAPs) deliver most perioperative care, a situation exacerbated by conflict-driven health system collapse. Despite global evidence supporting task sharing and task shifting, NPAP safety outcomes in fragile settings remain unquantified. This study aimed to quantify NPAP anaesthesia delivery, compare supervised and unsupervised safety outcomes, and identify systemic gaps in conflict-affected Sudan. METHODS: We conducted a cross-sectional clinical audit o

Integration of a task strengthening strategy for hypertension management into HIV care in Nigeria: Baseline characteristics of participants in a cluster randomized controlled trial.

BACKGROUND: Improved access to antiretroviral therapy (ART) has increased survival among people living with HIV (PLWH) but also the burden of cardiovascular risk factors like hypertension. While the task-strengthening strategy for hypertension control (TASSH) is a viable integrated care model, evidence of its clinical impact in Africa is limited. This report presents baseline findings from a trial evaluating 2 implementation strategies for integrating TASSH into HIV care across primary health centers (PHCs) in Lagos, Nigeria. METHODS: A total of 3,504 PLWH on antiretroviral therapy in 30 PHCs
