Temporal headaches and associated symptoms relating to the styloid process and its attachments.

The styloid process is a slender spike-like bony process that is attached to the base of the skull that has been of interest to physicians for centuries. From this process is the attachment for five structures--three muscles and two ligaments are attached to it. Any of these soft tissues of the styloid process are prone to be torn due to trauma by way of detachment of the periosteum from the bone. These lesions may occur from auto accidents, falls, sports injuries, to prolonged medical or dental procedures requiring excessive mouth opening. The detachment of Sharpey's fibres results in the rel

Does the Deep Layer of the Deep Temporalis Fascia Really Exist?

PURPOSE: It has been widely accepted that a split of the deep temporal fascia occurs approximately 2 to 3 cm above the zygomatic arch and extends into the superficial and deep layers. The deep layer of the deep temporal fascia is between the superficial temporal fat pad and the temporal muscle. However, during procedures, the authors noted the absence of the deep layer of the deep temporal fascia between the superficial temporal fat pad and the temporal muscle. This prospective study was conducted to clarify the presence or absence of a deep layer of the deep temporal fascia. MATERIALS AND ME

The blood supply of the reverse temporalis muscle flap: anatomic study and clinical implications.

Although the reverse temporalis muscle flap has been used clinically, the exact vascular connection between the superficial and deep temporal vessels has not been clearly defined. The purpose of this study was to investigate the vascular territory of the reverse temporalis muscle supplied by the superficial temporal vessels. Six cadaver heads were studied using a colored lead oxide injection through the superficial temporal artery. The specimens were examined macroscopically and radiographically. The reverse temporalis muscle flap was then applied to a clinical case presenting with traumatic a

The Face – A Musculoskeletal Perspective. A literature review.

The individual appearance and facial expression are based on the musculoskeletal system of the face. The bones of the face contribute to the anterior portion of the skull. This region is also referred to as the facial skeleton or viscerocranium. The muscles of the face include all mimetic muscles innervated by the cranial nerve VII (facial nerve). Two masticatory muscles (masseter, temporalis) that are supplied by the motoric portion of the cranial nerve V3 (mandibular nerve) also contribute to the contour of the face. The mimetic muscles (also known as facial muscles or skin muscles) generall

Towards an integrative understanding of the human temporalis muscle: Anatomical, histological, and imaging evidence.

INTRODUCTION: Traditionally defined as attaching to the mandibular coronoid process, the temporalis muscle (TM) has instead been shown, through anatomical evidence, including dissection, histology, and imaging, to have more variable distal insertions relevant to mastication, deglutition, temporomandibular disorders, and facial reanimation. We have assembled this evidence and propose a classification based on insertion. METHODS: A systematic review of the medical literature was conducted using PubMed, Embase, Scopus, and LILACS to identify studies that reported distal TM insertions in humans b

The central nervous system adjusts muscle synergy structure and tightly controls rollator-supported transitions between sitting and standing.

BACKGROUND: Older individuals are at risk of falling. Assistive devices like rollators help to reduce that risk, especially by compensating for decreased leg muscle strength and balance problems. Paradoxically, rollators have been found to be a fall risk as well as being difficult to use. To investigate the causes, this study examines how different levels of rollator support (no assistance, light touch, and full support) and balance demands (standard lab floor, balance pads) affect movement coordination during standing up and sitting down movements. METHODS: Twenty young participants stood up

Strategic tunnelling of superficial temporal artery during bypass surgery for moyamoya disease.

BACKGROUND: The superficial temporal artery to middle cerebral artery bypass remains the favoured direct technique of cerebral revascularization in moyamoya angiopathy. We describe a novel technique of tunnelling the superficial temporal artery through the temporalis muscle. METHODS: The temporalis muscle is raised as a flap off the temporal bone. A triangular window is created on the temporalis fascia, and a tunnel is created for the superficial temporal artery to pass through the muscle belly. CONCLUSION: This innovative technique allows passage of the superficial temporal artery to the br

The electrical activity of the masticatory muscles in children with cleft lip and palate.

BACKGROUND: Information regarding masticatory muscle function in children with cleft lip and palate (CLP) is limited. As a consequence, research on masticatory muscle activity in cleft subjects is needed. AIM: To assess masticatory muscle activity in children surgically treated for CLP as well as identify the possible factors associated with this activity. DESIGN: The sample comprised 82 children with mixed dentition and Class I occlusions (25 children with unilateral CLP and 57 subjects with no cleft abnormalities). A DAB-Bluetooth Instrument (Zebris Medical GmbH, Germany) was used to take

EMG differences between weak and strong myogenous CMD patients and healthy controls.

Electromyographic and clinical characteristics of 42 myogenous craniomandibular disorder (CMD) patients were related to 40 healthy control subjects. Surface EMG recordings were obtained from the masseter and the temporal muscles. The clinical examination included active maximum mouth opening, endfeel distance, active laterotrusion and lateral endfeel distance, dental abrasion, the presence of a lateral slide (RCP-ICP) and the amount of static pain. Orthopantomographic X-rays were available from 32 CMD patients for measuring the condylar and ramus asymmetry. Compared to the control group lower

Ankylosing spondylitis presenting with enthesitis at an uncommon site and fever of unknown origin.

A 58-year-old man presented with a 2-month history of arthralgia and bilateral temporal region pain, and a 1-month history of fever. He had had refractory neck pain since his 20s. Reduced cervical and lumbar mobility was observed. Radiographs of cervical and thoracic vertebrae disclosed syndesmophytes. Pelvic radiographs showed sclerosis in the right sacroiliac joint and ankylosis in the left sacroiliac joint. MRI with contrast enhancement showed enthesitis in the upper extremities and enhancement in the bilateral temporal muscle, which indicated enthesitis of temporal muscle. He was diagnosed

[Motor nerves of the face. Surgical and radiologic anatomy of facial paralysis and their surgical repair].

Motor innervation of the face depends on the facial nerve for the mobility of the face, on the mandibular nerve, third branch of the trigeminal nerve, which gives the motor innervation of the masticator muscles, and the hypoglossal nerve for the tongue. In case of facial paralysis, the most common palliative surgical techniques are the lengthening temporalis myoplasty (the temporal is innervated by the mandibular nerve) and the hypoglossal-facial anastomosis. The aim of this work is to describe the surgical anatomy of these three nerves and the radiologic anatomy of the facial nerve inside the

ATP-sensitive muscle afferents activate spinal trigeminal neurons with meningeal afferent input in rat - pathophysiological implications for tension-type headache.

BACKGROUND: Tension-type headache and other primary headaches may be triggered or aggravated by disorders of pericranial muscles, which is possibly due to convergent or collateral afferent input from meningeal and muscular receptive areas. In rodent models high extracellular concentrations of ATP caused muscle nociception and central sensitization of second order neurons. In a rat model of meningeal nociception we asked if spinal trigeminal activity induced by ATP can be modulated by local anaesthesia of distinct muscles. METHODS: Ongoing activity was recorded from spinal trigeminal neurons w

Hemodynamic monitoring in the human temporalis muscle using near-infrared spectroscopy.

Objective. Altered temporal muscle perfusion is implicated in several painful disorders afflicting orofacial and head regions, including temporomandibular joint dysfunctions, bruxism, and headache. Knowledge about the regulation of blood supply to the temporalis muscle is limited, due to methodological difficulties. The study aimed to test the feasibility of near-infrared spectroscopy (NIRS) monitoring of the human temporal muscle.Approach. Twenty-four healthy subjects were monitored with a 2-channel NIRS: amuscleprobe placed over the temporal muscle and abrainprobe placed on the forehead. A s

Temporalis Muscle Dissection Techniques in the Pterional Approach: Quantitative Impact on Operative Corridor and Surgical Freedom.

OBJECTIVE: To compare the effects of 3 temporalis muscle dissection techniques-myocutaneous (MC), interfascial (IF), and subfascial (SF)-on surgical freedom during the pterional approach. METHODS: Eight cadaveric heads were dissected bilaterally (16 sides). The MC, IF, and SF techniques were compared in terms of temporal lobe surface area, surgical freedom volume, and horizontal and vertical angles of attack at 8 predefined target points. Measurements were obtained using Cartesian coordinates acquired with a frameless stereotactic navigation system and analyzed using mathematical modeling. R

[The subperiosteal subtemporal approach in the surgical treatment of craniostenosis. Apropos of 9 cases].

The authors describe the approach consisting of direct subperiosteal, subtemporal cleavage based on existing anatomical continuity between the subtemporal plane and the cranial periosteum in young children. This allows for downward cleavage up to the level of the malar processes, for dissecting both orbits as well as the external columns. Frontoorbital remodeling which is needed for management of craniostenosis may be readily carried out with the improved field of vision. It will suffice, then, to depress the scalp and to suture the skin to obtain perfect repositioning of the temporal muscle,
