Evaluation and nonsurgical management of rotator cuff calcific tendinopathy.

Rotator cuff calcific tendinopathy is a common finding that accounts for about 7% of patients with shoulder pain. There are numerous theories on the pathogenesis of rotator cuff calcific tendinopathy. The diagnosis is confirmed with radiography, MRI or ultrasound. There are numerous conservative treatment options available and most patients can be managed successfully without surgical intervention. Nonsteroidal anti-inflammatory drugs and multiple modalities are often used to manage pain and inflammation; physical therapy can help improve scapular mechanics and decrease dynamic impingement; ul

Clinical applications of platelet-rich plasma in patellar tendinopathy.

Platelet-rich plasma (PRP), a blood derivative with high concentrations of platelets, has been found to have high levels of autologous growth factors (GFs), such as transforming growth factor-β (TGF-β), platelet-derived growth factor (PDGF), fibroblastic growth factor (FGF), vascular endothelial growth factor (VEGF), and epidermal growth factor (EGF). These GFs and other biological active proteins of PRP can promote tissue healing through the regulation of fibrosis and angiogenesis. Moreover, PRP is considered to be safe due to its autologous nature and long-term usage without any reported maj

Results of proximal medial gastrocnemius release for Achilles tendinopathy.

BACKGROUND: Most patients with Achilles tendinopathy (AT) are treated successfully with physiotherapy involving eccentric calf training. In some patients, gastrocnemius contracture persists and there are reports of improvement following gastrocnemius release. We present the first series of patients to have proximal medial gastrocnemius release (PMGR) for AT. METHOD: 16 PMGRs (12 patients) were performed at our institution over a 2-year period. Nine patients (10 PMGRs) were available for follow-up. The mean age of patients was 45 years (range, 25-63 years), with 5 female and 4 male subjects. T

Gender and eccentric training in Achilles mid-portion tendinopathy.

The role of gender in Achilles tendinopathy is yet to be determined. We hypothesized that female patients respond the same as males to 12 weeks of painful eccentric training. A total number of 75 consecutive mid-portion patients with Achilles tendinopathy (25 females, 38 males) were enrolled in a cohort study with 63 being analyzed after 12 weeks according to their gender for tendon and paratendon microcirculatory mapping. Outcome was determined by pain on visual analogue scale, VISA-A score, Foot Ankle Outcome Score (FAOS), tendon and paratendon capillary blood flow, oxygen saturation, and po

Kinematic analysis of runners with achilles mid-portion tendinopathy.

BACKGROUND: Despite anecdotal evidence linking overpronation to the onset of Achilles tendinopathy (AT), there is little conclusive evidence of a particular movement pattern of the lower extremity associated with this injury. Therefore, the objective of the present study was to observe differences in the kinematic profiles of healthy runners (CON) and runners with mid-portion Achilles tendinopathy (ATG). MATERIALS AND METHODS: In this cross-sectional analysis, 48 male height and weight matched subjects were invited to participate: 27 with mid-portion Achilles tendon pain and 21 asymptomatic c

Minimally invasive paratenon release for non-insertional Achilles tendinopathy.

BACKGROUND: Non-insertional Achilles tendinopathy is an increasing, perhaps secondary to an increase in sporting activities. Traditional open surgical procedures have been associated with a significant rate of complications. We describe a minimally invasive technique involving the release of the adherent paratenon with a peritendinous infusion of corticosteroids and present the results in a cohort of patients. MATERIALS AND METHODS: Twenty-nine tendons (26 patients) with non-insertional Achilles tendinopathy were treated with the above technique. Assessments were made prospectively using an a

Exploring the knowledge base of ChatGPT in lateral elbow tendinopathy.

BACKGROUND: Large language models are a type of artificial intelligence that can understand language and generate responses to text inputs. This presents potential within healthcare to improve triage of common conditions with established care pathways, such as lateral elbow tendinopathy (LET). However, its application to clinical scenarios requires evaluation. METHODS: Four questions regarding LET investigation and management were posed to ChatGPT-3.5, which was asked to provide five evidence sources. Five clinical scenarios were posed to the model, simulating consultations with typical and r

[Transarterial microembolization in tendinopathies : Overview and current state of research].

BACKGROUND: Chronic tendinopathies are common causes of persistent musculoskeletal pain. In refractory cases, pathological hypervascularization and neo-innervation play a key role in pain generation. Transarterial microembolization (TAME) aims to reduce this abnormal vascularity using a minimally invasive approach. PURPOSE: This review summarizes current evidence regarding the efficacy and safety of TAME in selected tendinopathies. MATERIALS AND METHODS: A narrative review of recent literature on TAME for adhesive capsulitis, medial and lateral epicondylitis, Achilles tendinopathy, and plant

Current perspectives on the etiology and pathogenesis of tendinopathy - a literature review.

OBJECTIVE: Aim: Tendinopathy is a pain-dysfunction syndrome of tendons resulting from an imbalance between the processes operating within the tissue and the external factors to which it is exposed. Contemporary concepts of pathogenesis indicate the overlap of many mechanisms: from mechanical overload leading to abnormal matrix remodeling, through inflammatory processes, to intratendinous compression causing hypoxia and pathological vascularization. The aim of this study was to review information regarding the current state of knowledge about the pathogenesis and mechanisms driving tendinopathi

Role of the tendon circadian clock in tendinopathy and implications for therapeutics.

Tendinopathy or tendon overuse injury is a major clinical problem for individuals and has a significant socio-economic cost. Its pathophysiology is not yet fully understood and involves multiple factors, including mechanical, cellular and molecular factors. The circadian rhythm has been shown to be a major regulator of extracellular matrix (ECM) homeostasis in several connective tissues, including tendon. Very recently, the human patellar tendon has been established as a peripheral clock tissue that exhibits dampening with chronic tendinopathy, and this has important translational and clinical

Gluteal tendinopathy masterclass: Refuting the myths and engaging with the evidence.

INTRODUCTION: Gluteal tendinopathy is a prevalent and impactful condition. For many decades clinical practice has been guided by 'myths' that developed around clinical beliefs. It's time to examine these myths and engage with the substantial evidence base. PURPOSE: This masterclass aims to: (i) engage with the evidence to refute the most common myths associated with gluteal tendinopathy and trochanteric pain, and (ii) provide clinicians with guidance around contemporary, evidence-based clinical practice. IMPLICATIONS: It is time to put aside traditional beliefs that focused on anti-inflammat

Effectiveness of Kinesiotherapy in the Treatment of Achilles Tendinopathy-A Narrative Review.

This narrative review of kinesiotherapy methods in the treatment of Achilles tendinopathy (AT) encompassed a diverse range of studies, including athletes and untrained people, healthy or injured, undergoing kinesiotherapy treatments. Most experimental studies (86%) reported results related to pain perception, 27% to the range of motion, and 27% to biomechanical assessment. However, the studies showed notable heterogeneity in the outcomes associated with the interventions, and, in this review of kinesiotherapy protocols for AT, a prominent observation emerged regarding their efficacy, suggestin

Role of orthobiologics in managing patellar tendinopathy: A narrative review.

UNLABELLED: Patellar tendinopathy is a relatively common cause of anterior knee pain in athletes. It is predominantly seen in sports involving jumping, running, abrupt change in direction like basketball, volleyball, soccer, sprinting and jumping. The main pathophysiology is considered to be repetitive microtrauma at the inferior pole of patella along with a poor healing response. Although eccentric exercises have shown to be beneficial, the improvement is often slow and may result in a less than satisfactory outcome. There is a growing interest of orthobiologics, mainly platelet-rich plasma (
