Trauma-informed palliative care: A systematic scoping review of evidence sources describing concepts relevant to an emerging field of practice.

BACKGROUND: Trauma-informed palliative care aims to improve end-of-life experiences by recognising and responding to the presence of psychological trauma. While many practitioners support the approach, they also acknowledge the need to build a stronger evidence base. AIM: To: (1) understand the methodology and defining features of existing research; (2) describe how the impact of psychological trauma at the end-of-life is understood; and (3) describe the nature of, and challenges to, palliative care approaches that recognise and respond to such trauma. DESIGN: A systematic scoping review was

Response shift as a common mechanism linking quality of life gains and reduced aggressive care in early palliative care trials.

In this commentary, I address the association of quality of life and palliative care objective outcomes to Calman's gap and response shift. One can surmise that palliative care benefits are related to response shifts in quality of life which have a direct influence on choices of care at the end of life.

Palliative Care for Dementia.

With the growing care needs for the older population at the end of their lives, there has been a substantial increase in attention to the management of the patient with dementia in hospice and palliative care services. This article reviews issues in access to care and the optimal management of the patient with dementia, particularly in the context of neuropsychiatric complexities. Special issues such as delirium, cachexia, behavioral symptoms, and pain management are addressed. Future challenges in research such as the development of better prognostic models are noted as well as the importance

Palliative and end-of-life nursing care in Saudi Arabia: A systematic review of nursing practices, challenges, and patient-family outcomes.

BACKGROUND: Palliative and end-of-life (EOL) care is gaining increasing importance in Saudi Arabia due to the rising burden of chronic and life-limiting illnesses. Nurses play a central role in delivering comprehensive, culturally appropriate palliative care; however, their practices are influenced by educational preparation, institutional support, and sociocultural and religious contexts. To date, evidence on palliative nursing care in Saudi Arabia remains fragmented and insufficiently synthesized. AIM: This systematic review aimed to synthesize existing evidence on palliative and EOL nursin

The role of music therapy in palliative and end-of-life care: a narrative review of psychosocial outcomes.

BACKGROUND/OBJECTIVES: Music therapy has emerged as a valuable component of palliative and end-of-life care, addressing the physical, emotional, social, and spiritual needs of patients and their families. Despite growing clinical adoption, the mechanisms and breadth of psychosocial outcomes remain inconsistently synthesized. This narrative review aims to synthesize peer-reviewed evidence on music therapy's psychosocial outcomes in palliative settings, including mechanisms, populations, modalities, and evidence gaps. METHODS: A systematic search was conducted in PubMed and the University of Al

Palliative care or supportive care?

Palliative care is the active holistic (physical, psychological, social and spiritual) care of people who experience health-related suffering due to severe illness and those close to them, aiming to improve their quality of life. There are three levels of palliative care: palliative care approach, generalist palliative care, and specialist palliative care. Traditionally associated with end-of-life care, this article highlights the evolution of specialist palliative care, now deemed to be applicable throughout the course of an illness. Terminologies surrounding 'palliative care' are also consid

Effectiveness of End-of-Life Care in the Community in the Asian Context: A Systematic Review.

Asian countries are prioritizing community and home end-of-life care (EoLC) as populations age and care preferences shift. Asian community-based EoLC's effectiveness is questionable due to cultural, institutional, and healthcare system challenges that affect access and quality. This systematic review seeks components. This makes EoLC effective in Asia. The disparities in care models, how they combine with healthcare institutions, and cultural and family variables are examined. Systematic review (PROSPERO ID: CRD42025644698). The electronic databases PubMed, CINAHL, Scopus, and ScienceDirect we

From research to reality: A review of three clinical problems in the last days of life.

All of us will one day die. For most of us, death will be anticipated, usually following a period of ill health. The opportunity to anticipate and manage clinical conditions associated with the agonal process is an essential part of advance care planning. Guidelines exist for the palliation of most symptomatic events at the end of life, although many recommendations are based on low-quality evidence or consensus. Furthermore, when potentially practice-changing data emerge, there is an inevitable lag time before clinical practice changes. In this paper, we shall discuss the management of three

Addressing Challenges With Sedation in End-of-Life Care.

In 2009, Quill and colleagues stipulated that there are three types of sedation practices at the end of life: ordinary sedation, proportionate palliative sedation (PPS), and palliative sedation to unconsciousness (PSU). Of the three, PPS and PSU are described as "last-resort options" to relieve refractory symptoms, and PSU as the most ethically controversial type that "should be quite rare." Unfortunately, little is known about actual sedation practices at the end of life in the United States. This may be due in part to a lack of conceptual clarity about sedation in end-of-life care. We argue

Consensus-based guidelines for the provision of palliative and end-of-life care for people living with epidermolysis bullosa.

BACKGROUND: Inherited epidermolysis bullosa (EB) is a cluster of rare, genetic skin and mucosal fragility disorders with multi-system and secondary effects, in which blistering and erosions occur in response to friction/mechanical trauma. Considering the incurable and potentially life-limiting nature of the condition and the challenges posed by its symptoms, a palliative approach to EB-related care is necessary. However, knowledge and experience related to the provision of EB palliative care is minimal. Evidence-based, best care guidelines are needed to establish a base of knowledge for practi

[Is it necessary to legislate euthanasia?].

There are no specific articles on the end of life in French law. Thus an act of euthanasia can be qualified as murder, murder with premeditation or non-assistance to a person in danger. Recent events and debates have raised the question of enacting new legislation to deal with this problem. Two contrary positions could be considered: either create a special offence or explicitly authorize acts of euthanasia. There are major objections to both these propositions. The first one would require taking account of various situations, --unbearable suffering, loss of dignity, and precise requests,--tha

The connections of physical and psychosocial symptoms among patients with terminal illnesses: A network analysis.

BACKGROUND: Patients with terminal illnesses have different symptoms. The goal of palliative care is to alleviate the suffering induced by these symptoms. Available research on symptoms has focused on their prevalence or identification of clusters. Little is known about the central symptoms contributing to the maintenance of the whole system of symptoms or the mechanism of influence between symptoms, particularly between physical and psychosocial symptoms. AIM: The study used network analysis to explore symptom clusters among patients with terminal illnesses and identify the central symptoms,

Palliative pharmacological sedation for terminally ill adults.

BACKGROUND: Terminally ill people experience a variety of symptoms in the last hours and days of life, including delirium, agitation, anxiety, terminal restlessness, dyspnoea, pain, vomiting, and psychological and physical distress. In the terminal phase of life, these symptoms may become refractory, and unable to be controlled by supportive and palliative therapies specifically targeted to these symptoms. Palliative sedation therapy is one potential solution to providing relief from these refractory symptoms. Sedation in terminally ill people is intended to provide relief from refractory symp

Development and external validity of a nurse-led intervention program to improve palliative care and quality of dying and death in intensive care unit.

INTRODUCTION: In Japan, end-of-life care in intensive care units (ICU) remains inconsistent and often suboptimal, contributing to variability in the quality of dying and death. This study aimed to develop a culturally appropriate nurse-led intervention program to improve the quality of dying and death in Japanese ICU. METHODS: Using a structured framework for intervention development, we conducted a systematic and scoping review to identify modifiable factors and effective strategies for end-of-life care in ICUs. Eight evidence-based strategies were synthesized into four core components: symp
