Case Report: Sarcoid-Like Reactions and Tertiary Lymphoid Structures Following Dual Checkpoint Inhibition in a Patient with Early-Stage Lung Adenocarcinoma.

Immune checkpoint inhibitor-induced sarcoid-like reactions and tertiary lymphoid structures (TLSs) are increasingly recognized but rarely reported in the same patient. We report a patient with lung adenocarcinoma who displayed sarcoid-like reactions in intrathoracic lymph nodes and tertiary lymphoid structures in surgical tumor after neoadjuvant therapy with nivolumab plus ipilimumab. Pathological examination revealed 50% residual tumor cells after treatment, and the CT evaluation of the primary tumor showed a stable disease. The patient experienced a recurrence eight months after surgery. To

A Standardized Analysis of Tertiary Lymphoid Structures in Human Melanoma: Disease Progression- and Tumor Site-Associated Changes With Germinal Center Alteration.

There is increasing evidence that tertiary lymphoid structures (TLS) control not only local adaptive B cell responses at melanoma tumor sites but also the cellular composition and function of other immune cells. In human melanoma, however, a comprehensive analysis of TLS phenotypes, density and spatial distribution at different disease stages is lacking. Here we used 7-color multiplex immunostaining of whole tissue sections from 103 human melanoma samples to characterize TLS phenotypes along the expression of established TLS-defining molecular and cellular components. TLS density and spatial d

Tertiary lymphoid structures with overlapping histopathologic features of cutaneous marginal zone lymphoma during neoadjuvant cemiplimab therapy are associated with antitumor response.

The development of immune checkpoint inhibitor (ICI) therapy with anti-CTLA-4 and anti-PD-1/L1 monoclonal antibodies has led to a paradigm shift in cancer therapy. ICI neoadjuvant therapy followed by surgery has become the standard of care for several advanced-stage cancers. The pathology associated with ICI therapy is vast and includes neoadjuvant-associated tissue reactions and activation of tertiary lymphoid structures (TLSs) at the site of the tumor bed and off-target immune-related adverse events. TLSs are thought to recapitulate lymph node function and may act as localized immune machine

Neoadjuvant chemotherapy interferes with tertiary lymphoid structure formation in Epstein-Barr virus-associated gastric cancer.

Epstein-Barr virus (EBV)-associated gastric cancer (EBVaGC) is a molecularly distinct, highly immunogenic gastric cancer subtype, yet the role of tertiary lymphoid structure (TLS) during therapy remains unclear. Using a large locally advanced EBVaGC cohort (144 samples), combined with spatial transcriptomics (11 samples) and a single-cell atlas (77 samples), we show that EBVaGC exhibits enhanced TLS formation compared with EBV-negative gastric cancer, with invasive-margin TLS (IM-TLS) predicting favorable prognosis. Neoadjuvant chemotherapy disrupts TLS formation and shows limited efficacy in

Research progress of tertiary lymphoid structure in hepatocellular carcinoma: from prognostic biomarkers to new strategies for immunotherapy.

Tertiary lymphoid structures (TLS) are important ectopic immune aggregates that develop within the tumor microenvironment (TME) of hepatocellular carcinoma (HCC) and have attracted increasing attention due to their potential clinical value. Recent studies have identified TLS as key players in prognosis assessment and immunotherapy response prediction in HCC, with their maturity and spatial distribution being critical determinants of clinical outcomes. TLS can be classified into intratumoral and peritumoral types, which exhibit distinct predictive values; the clinical significance of peritumora

Tertiary lymphoid structure topotypes in intrahepatic cholangiocarcinoma: spatial immunity, checkpoint-response associations, and therapeutic reprogramming.

Tertiary lymphoid structures (TLS) have evolved from descriptive histological findings into candidate biomarkers and targetable immune niches in solid tumors. In intrahepatic cholangiocarcinoma (ICC), however, similar lymphoid architectures may have different biological implications depending on their relationship to tumor nests, immune circuitry, and suppressive stroma. We define a "TLS topotype" as the multidimensional state of an individual TLS-centered region, including the TLS core, its perilesional neighborhood, and the nearest tumor-stroma interface. This state integrates anatomic topol

Effect of tumor draining lymph nodes in the formation and maturation of tertiary lymphoid structure in patients with lung adenocarcinoma.

BACKGROUND: Tertiary lymphoid structures (TLSs), ectopic lymphoid aggregates composed of immune cells, are associated with favorable clinical outcomes and increased efficacy of anti-tumor immunotherapies. However, the mechanisms underlying the formation and maturation of TLSs require further elucidation. The correlation between tumor immune microenvironment in tumor-draining lymph nodes (TDLNs) and TLSs remains inadequately studied. The study aimed to utilize multiplex immunofluorescence (mIF) to explore the relationship between TDLN and TLSs. METHODS: Tissue slides from 120 patients with lun

Identifying potential mechanism and targets for treatment of tertiary lymphoid structure in lupus nephritis based on bioinformatics analysis.

BACKGROUND: Tertiary lymphoid structure (TLS) is an ectopic lymphoid structure that develops in non-lymphoid structures. Some studies have shown that the TLS formed in autoimmune diseases, such as lupus nephropathy (LN), can cause damage to normal tissues and continuous disease progression. Nevertheless, there is still a lack of efficient treatments for TLS in LN. Thus, the study aims to identify potential targets for therapy of TLS in LN. METHODS: Mice datasets relative to TLS were obtained from Gene Expression Omnibus (GEO). The differentially expressed genes (DEGs) were identified from mic

Prognostic Significance of Tertiary Lymphoid Structure-Like Lymphocyte Aggregates in Routine Histopathological Examination in Bladder Cancer.

OBJECTIVE: To evaluate the effect of tertiary lymphoid structure (TLS)-like lymphocyte aggregates in radical cystectomy (RC) specimens on the survival outcomes of patients with bladder cancer. METHODS: We included 110 patients who underwent RC for urothelial carcinoma of the bladder. Hematoxylin and eosin-stained slides of RC specimens were evaluated for immune-related pathological results including the presence of TLS-like lymphocyte aggregates. Survival outcomes between groups were compared. The effects of various parameters on non-urinary tract recurrence-free survival (NUTRFS) were evalua

Cell-Targeted Inhibition of CaMK4 Suppresses Tertiary Lymphoid-like Structure Development in Lupus-Prone Mice.

Current treatment of lupus nephritis (LN) relies on broad immunosuppression and often fails to eradicate intrarenal immune niches that sustain inflammation. Tertiary lymphoid structures (TLS)-organized aggregates of immune cells forming in chronically inflamed non-lymphoid tissues-are increasingly recognized as drivers of local immune activation and tissue injury in LN. We previously showed that genetic CaMK4 deficiency suppresses autoimmunity and nephritis in lupus-prone mice. Here, we tested whether CaMK4 regulates renal TLS-like organization. Using kidneys from MRL/lpr mice that were CaMK4-

Spatiotemporal mapping of tertiary lymphoid structure heterogeneity shapes immune niches and clinical outcomes in intrahepatic cholangiocarcinoma.

Intrahepatic cholangiocarcinoma (iCCA) is a highly lethal malignancy with limited therapeutic options. The spatial architecture and functional diversity of tertiary lymphoid structures (TLSs) in iCCA remain unclear. Here, we present a multimodal spatial atlas of TLSs and identified intratumoral TLSs (iTLSs) as independent prognostic markers. Bulk proteomic profiling of 214 discovery and 155 validation cases identified a four-tier TLS-based tumor microenvironment classification system and supported development of a TLS-predictive random forest classifier. Imaging mass cytometry revealed that iT

Tertiary lymphoid structure can be a potential therapeutic target for head and neck squamous cell carcinoma (Review).

Tertiary lymphoid structure (TLS) is a kind of ectopic lymphoid tissues that form in chronic inflammation or tumor microenvironment (TME) and is structurally similar to secondary lymphoid organs such as lymph nodes, containing B cell follicles, T cell regions, and mature dendritic cells. It was reported that TLS is associated with improved prognosis and higher response rates to immunotherapy in a variety of tumors. However, the mechanism of TLS and its clinical significance in head and neck squamous cell carcinoma (HNSCC) remains unclear. In the present study, the latest advances of biological

Single-Cell RNA Sequencing + TCR Sequencing Reveal the Proportion and Characteristics of Dual TCR T Cells in Tertiary Lymphoid Structures in Pemphigus.

The origin, characteristics, and the effects/mechanisms of lymphocytes within tertiary lymphoid structures (TLSs) in pemphigus pathological skin tissue as well as their role in pemphigus pathogenesis remain unclear. Using single-cell RNA-sequencing and TCR-sequencing analysis, this study revealed that pemphigus TLSs (denoted as P_TLS) contain a higher proportion of dual TCR T cells than chronic idiopathic erythroderma and healthy control skin tissues. Notably, P_TLSs exhibit a unique enrichment of clonally expanded dual TCR CXCL13+CD4+ T cells, whereas the proportions of dual TCR regulatory T

Analyzing the associations between tertiary lymphoid structures and postoperative prognosis, along with immunotherapy response in gastric cancer: findings from pooled cohort studies.

BACKGROUND: The clinical significance of tertiary lymphoid structure (TLS) in gastric cancer (GC) was uncertain. METHODS: A systematic search was performed in public databases for eligible studies as of April 2, 2023. Meta-analyses were performed to interrogate the associations between TLS levels and prognosis and immunotherapy response of GC. Bioinformatic analyses based on the nine-gene signature of TLS were further conducted to capture the biological underpinnings. RESULTS: Eleven studies containing 4224 GC cases were enrolled in the meta-analysis. TLS levels positively correlated with sm

SSR4 Sustains Tertiary Lymphoid Structures by Regulation Quality Control of N-linked Glycosylation During B-cell Differentiation Into Plasmacyte in Colorectal Cancer.

Intratumoral tertiary lymphoid structure (TLS) fosters B-cell differentiation and antibody production, yet the intrinsic programs sustaining these functions remain unclear. By integrating spatial transcriptomics and single-cell RNA sequencing of colorectal cancer, we delineate a B-cell trajectory enriched for endoplasmic reticulum protein processing, autophagy, NF-κB signaling, and N-linked glycosylation, highlighting SSR4 as a progressively induced TRAP-complex component. SSR4 is further upregulated in B cells from anti-PD-1 responders. B-cell-specific Ssr4 deletion leads to peripheral B-cell

A High Density of Impaired Tertiary Lymphoid Structures Is Associated with Higher Tumor Stage and Recurrence in Non-Muscle-Invasive Bladder Cancer.

Tertiary lymphoid structures (TLS) have emerged as critical immune niches within the tumor microenvironment across various cancers. However, their structural organization and functional roles in non-muscle-invasive bladder cancer (NMIBC) remain poorly characterized. In this study, we comprehensively characterized TLS in NMIBC using en bloc resected primary diagnostic specimens, which enabled high-resolution spatial mapping and quantitative histological assessment across a large cohort of primary tumors (97 patients). Past failures and new horizons: the nuances of tertiary lymphoid structures in high-grade serous ovarian cancer may contribute to immunotherapy effectiveness.

Tertiary lymphoid structures (TLS) are ectopic lymphoid aggregates that allow for optimal B-T cell crosstalk in solid tumors. Further, TLS are found in many solid tumors and are associated with improved patient survival and superior immunotherapeutic response, ultimately reflecting their potential as new therapeutic targets. Despite the prognostic benefit of TLS, women with high-grade serous ovarian cancer (HGSOC) are insensitive to immunotherapies. We have summarized recent work on TLS in patients with HGSOC, ultimately demonstrating how tumor microenvironment factors and therapeutics shape t
