Isolated Left Atrial Cardiac Tamponade Caused by Pleural Effusion.

A localized left atrial tamponade caused by left side pleural effusion is a rare finding that leads to hemodynamic instability. Here, we describe left atrial systolic and diastolic collapse resulting from left pleural effusion. An increase in intrapleural pressure by a pleural effusion can compress the pericardial space and lead to impaired cardiac filling and tamponade physiology. Here, we present a case of a 79-year old African American female who presented with shortness of breath and dry cough for a duration of one week. Chest radiograph and CT scan of the chest showed left pleural effusio

The process and safety of removing chest tubes 4 to 12 hours after robotic pulmonary lobectomy and segmentectomy.

OBJECTIVE: Chest tubes cause pain and morbidity. METHODS: This is a quality initiative study and review of patients who underwent robotic pulmonary resection by 1 surgeon (R.J.C.). The goal was to remove chest tubes within 4 to 12 hours after robotic segmentectomy and lobectomy. Primary outcome was removal without the need for reinsertion, thoracentesis, or any morbidity due to early removal of the chest tube. Secondary outcomes were symptomatic pneumothorax, pleural effusion, chylothorax, subcutaneous emphysema, and chest tube reinsertion or thoracentesis within 60 days of surgery. RESULTS:

CT demonstration of empyema necessitatis.

Empyema necessitatis is a collection of fluid that has, by direct extension from the pleural cavity, penetrated the thoracic wall to form a mass in the extrapleural soft tissues of the chest wall. This is a rare condition and usually is associated with tuberculous effusion but may be associated with malignancy, blastomycosis, actinomycosis, and may even follow thoracentesis.

Congenital chylothorax presenting as hydrops fetalis. A case report.

A case of congenital chylothorax presented as polyhydramnios and massive fetal edema. The diagnosis was suspected prior to delivery on the basis of ultrasonography. Ultrasound was helpful in localizing the fluid for postnatal thoracentesis. Congenital chylothorax is part of the differential diagnosis of nonimmune hydrops fetalis.

Ruptured giant lateral thoracic meningocele associated with intracranial hypotension syndrome in neurofibromatosis type 1: case report with long-term follow-up.

Lateral thoracic meningocele is a rare clinical entity typically associated with neurofibromatosis type 1 (NF1), which can progressively enlarge under the cerebrospinal fluid (CSF) pressure. Despite its rarity, these lesions may rupture or be associated with intracranial hypotension syndrome (IHS). We herein describe a 50-year-old woman presenting with a 2-year history of dyspnea, positional headaches, and dizziness. Imaging revealed a giant lateral thoracic meningocele with right-sided pleural effusion. Following thoracentesis with evacuation of 480 ml effusion (later confirmed as CSF), her h

Topics in Lung Disease: Noninfectious Pleural Effusions.

Noninfectious pleural effusions arise from an imbalance between fluid entering and leaving the pleural space. Effusions are classified as transudative or exudative, based on protein and lactate dehydrogenase levels. Common symptoms are dyspnea, cough, and chest pain. Physical examination findings may include dullness to percussion and decreased breath sounds. Transudates are associated with conditions such as congestive heart failure and cirrhosis, whereas exudates are linked to pneumonia, malignancy, pulmonary embolism, and gastrointestinal disorders. Diagnosis relies on chest imaging and ima

Diagnostic utility of eosinophils in the pleural fluid.

This study was conducted to assess the prevalence of eosinophilia in 358 consecutive samples of pleural fluid (all cases corresponded to first thoracentesis), to review the cause of eosinophilic pleural effusions, and to determine whether the presence of eosinophils increases the likelihood of nonmalignant underlying disorders. Eosinophilic pleural effusions were identified in 45 patients (12.6%): malignant underlying conditions were diagnosed in 11 patients (24.4% with eosinophilic effusions) and benign aetiologies were found in 27 patients. Benign aetiologies included uncomplicated paraneumo

Confirmed Case of Longstanding Respiratory Francisella tularensis holarctica Infection: Nebraska, 2022.

A male patient with distant history of extensive rabbit contact and pulmonary nodules for 6 years developed empyema. Francisella tularensis holarctica was isolated from thoracentesis fluid. Retrospective immunohistochemical examination of a pulmonary nodule, biopsied 3 years prior, was immunoreactive for F. tularensis. These findings suggest the potential for chronic tularemia.

Pneumothorax Ex Vacuo.

An 84-year-old woman with pleural effusion developed anterior chest pain during thoracentesis, and post-procedural chest radiography revealed hydropneumothorax, initially suggesting iatrogenic pneumothorax. However, comparison of pre- and post-procedure imaging showed ipsilateral hemithoracic volume loss with tracheal deviation toward the procedure side, supporting pneumothorax ex vacuo, managed conservatively without intervention.

Diagnosis and Treatment of Pleural Effusion. Recommendations of the Spanish Society of Pulmonology and Thoracic Surgery. Update 2022.

Pleural effusion (PE) is a common yet complex disease that requires specialized, multidisciplinary management. Recent advances, novel diagnostic techniques, and innovative patient-centered therapeutic proposals have prompted an update of the current guidelines. This document provides recommendations and protocols based on a critical review of the literature on the epidemiology, etiology, diagnosis, prognosis, and new therapeutic options in PE, and addresses some cost-effectiveness issues related to the main types of PE.

Characteristics and clinical implications of pleural effusions after lung transplantation: A retrospective analysis of 195 thoracocenteses in 113 patients.

Despite advances in lung transplantation (LTx), morbidity, and mortality are high. We hypothesized that pleural effusions requiring thoracocentesis lead to poor outcomes after LTx. We performed a single-center retrospective analysis of thoracocenteses after initial hospital discharge in LTx patients between March 2008 and September 2020 to identify risk factors, etiologies, and outcomes. Of the 1223 patients included, 113 patients (9.2%) required a total of 195 thoracocenteses. The cumulative incidence of thoracocentesis was 10.6% at 1 year and 14.2% at 5 years after transplantation. We observ

Clinical presentation and improvised management of neonatal pneumothorax in the setting of a low-resource country: Conakry, Guinea.

Two neonates were presented at the Neonatology Department of the Institute of Child Nutrition and Health in Conakry, Guinea, with tension pneumothoraces as confirmed by chest X-ray. They were initially managed with needle thoracentesis but required continuous thoracic drainage. Due to scarce resources in the public health sector, no prepacked and dedicated pleural drainage systems were available as is the case in many developing countries. Therefore, we fabricated an improvised underwater seal drain out of a plastic infusion bottle and a Heimlich valve out of a vicryl fingerstall. Both devices

Indwelling pleural catheters for benign pleural effusions: what is the evidence?

PURPOSE OF REVIEW: Benign pleural effusions are common and usually managed by treating the underlying disease process. In some patients, these effusions may be refractory to medical management. Indwelling pleural catheters, used extensively for malignant pleural effusions, are increasingly used in benign pleural effusions not responding to medical therapy. This review focuses on current data regarding their safety and effectiveness in effusions due to congestive heart failure, hepatic hydrothorax, end-stage renal disease, and chylothorax. RECENT FINDINGS: Indwelling pleural catheters are effe

Recurrent Malignant Pleural Effusion Management: Evaluating Quality Gaps, Outcomes, Costs, and Adherence to Guideline-Consistent Care.

BACKGROUND: Guidelines recommend early definitive management (indwelling pleural catheter, pleurodesis, decortication, or combination approach) for patients with symptomatic, rapidly recurrent malignant pleural effusion (MPE). This study evaluates health care utilization among patients who received early definitive management (EDM), defined as definitive management at first episode of rapidly recurrent MPE, compared with patients who do not (no early definitive management [NEDM]). RESEARCH QUESTION: What percentage of patients with rapidly recurrent symptomatic MPE receive guideline-consisten

A Simple Method for Differentiating Complicated Parapneumonic Effusion/Empyema from Parapneumonic Effusion Using the Split Pleura Sign and the Amount of Pleural Effusion on Thoracic CT.

BACKGROUND: Pleural separation, the "split pleura" sign, has been reported in patients with empyema. However, the diagnostic yield of the split pleura sign for complicated parapneumonic effusion (CPPE)/empyema and its utility for differentiating CPPE/empyema from parapneumonic effusion (PPE) remains unclear. This differentiation is important because CPPE/empyema patients need thoracic drainage. In this regard, the aim of this study was to develop a simple method to distinguish CPPE/empyema from PPE using computed tomography (CT) focusing on the split pleura sign, fluid attenuation values (HU:
