Ultrasound of the peripheral nerves in systemic vasculitic neuropathies.

INTRODUCTION: Ultrasound of the peripheral nerves (PNUS) can be used to visualize nerve pathologies in polyneuropathies (PNP). The aim of this study was to investigate, whether PNUS provides additional information in patients with proven systemic vasculitic neuropathies (VN). MATERIAL AND METHODS: Systematic ultrasound measurements of several peripheral nerves, the vagal nerve and the 6th cervical nerve root were performed in 14 patients and 22 healthy controls. Nerve conduction studies of the corresponding nerves were undertaken. Finally, the measured results were compared to a study populat

Tarsal tunnel syndrome: electrophysiological study.

Tarsal tunnel syndrome (TTS) is a rare compression neuropathy of the posterior tibial nerve. Typical symptoms are burning pain and paresthesia in the toes and along the sole of the foot. The presence of Tinel's sign and objective sensory loss in the territory of any of the terminal branches of the posterior tibial nerve are diagnostically helpful. The terminal latency and sensory nerve conduction velocity in medial and lateral plantar nerves were studied in 20 normal controls and 21 cases of TTS in 17 patients. Prolonged terminal latency was observed in 11 cases, with TTS, while sensory nerve

Tarsal tunnel syndrome caused by strain of an anomalous muscle: an MRI-specific diagnosis.

Tarsal tunnel syndrome (TTS) is a compression neuropathy of the posterior tibial nerve in the ankle, commonly caused by a mass lesion such as ganglion, tumor, fracture, or post-traumatic fibrosis. Although anomalous muscle is relatively common in the ankle, there has been only one report to date implicating it in TTS. This case demonstrates MR findings of a strained flexor digitorum accessorius longus muscle as an unusual cause of TTS.

A Complication of Posterior Malleolar Fracture Fixation.

We present a case of tibial nerve impingement by an anteroposterior screw inserted for stabilization of a posterior malleolar fracture. This specific complication has not previously been described in published studies, although numerous reports have described various forms of peripheral nerve entrapment. We discuss the merits of fixation of these fractures using a posterolateral approach.

Predictive Outcome Modeling of Preoperative Clinical Symptoms and Electrodiagnostic Data in Tarsal Tunnel Surgery.

Background  The relationship between tarsal tunnel syndrome (TTS), electrodiagnostic (Edx) findings, and surgical outcome is unknown. Analysis of TTS surgical release outcome patient satisfaction and comparison to Edx nerve conduction studies (NCSs) is important to improve outcome prediction when deciding who would benefit from TTS release. Methods  Retrospective study of 90 patients over 7 years that had tarsal tunnel (TT) release surgery with outcome rating and preoperative tibial NCS. Overall, 64 patients met study inclusion criteria with enough NCS data to be classified into one of the fol

Health effects among refrigeration repair workers exposed to fluorocarbons.

Refrigeration repair workers may be intermittently exposed to fluorocarbons and their thermal decomposition products. A case of peripheral neuropathy (distal axonopathy) in a commercial refrigeration repairman prompted an epidemiological investigation of the health of refrigeration repair workers. No additional cases of peripheral neuropathy were identified among the 27 refrigeration repair workers studied. A reference group of 14 non-refrigeration repair workers was also studied. No differences were noted between groups for the ulnar (motor and sensory), median (motor and sensory), peroneal,

Charcot arthropathy of the knee managed with total knee arthroplasty.

Charcot joint arthropathy is a progressive degenerative condition where neuropathy causes joint destruction, deformity and instability. Although most frequent in the foot and ankle, knee involvement occurs occasionally with rapid, disabling progression. Diagnosis is often challenging due to overlap with septic and inflammatory arthritis. We report a woman in her forties with post-spinal anaesthesia neuropathy who developed progressive left knee pain, deformity and instability over 5 years. Radiographs and MRI confirmed Charcot arthropathy. She successfully underwent total knee arthroplasty (TK

Tarsal tunnel syndrome: still more opinions than evidence. Status of the art.

Tarsal tunnel syndrome is an entrapment neuropathy of the posterior tibial nerve or its terminal branches within its fibro-osseous tunnel beneath the flexor retinaculum on the medial side of the ankle. The condition is frequently underdiagnosed leading to controversies regarding its epidemiology and to an intense debate in the literature. With the advent of nerve imaging techniques, the diagnostic confirmation and the etiological identification have become more accurate. However, management of this entrapment neuropathy remains challenging because of many intervention strategies but limited ro

Huge Intraneural Ganglion Cyst of Tibial Nerve in a 78-Year-Old Male Patient With Gonarthrosis: A Case Report and Review of the Literature.

Intraneural ganglion cysts (IGCs) are mucinous cysts located within peripheral nerves, often associated with an articular nerve branch and the adjacent synovial joint capsule. These cysts, while rare, can occur in various nerves, with the tibial nerve being an infrequent site. Tibial nerve IGCs are rare pathologies. We present a case of a tibial nerve IGC in a 78-year-old male patient with pre-existing grade III gonarthrosis. Furthermore, we performed a brief review of the existing literature for tibial nerve IGCs. To our knowledge, we present the second case of an IGC in a patient with known

Leprosy affects the tibial nerves diffusely from the middle of the thigh to the sole of the foot, including skip lesions.

This study investigated where leprosy affects the posterior tibial nerve and whether neurolysis is beneficial. Nine patients with bilateral posterior tibial leprous neuropathy with no sensorimotor recovery were studied. Preoperative sensory-muscle and nerve conduction velocity testing revealed the tarsal tunnel to be the site of a severe lesion in all cases. During surgery, the most proximal site of the nerve lesion was detected by electrically stimulating the spinal roots from the second lumbar nerve to the fourth sacral nerve, evoking efferent mixed nerve compound action potentials that were

Zig-Zag Skin Incision for Treatment of Tarsal Tunnel Syndrome.

BACKGROUND: Tarsal tunnel syndrome (TTS) is a common entrapment neuropathy of the posterior tibial nerve. Surgery can be performed less invasively under local anesthesia. We adopted zig-zag skin incision to prevent postoperative wound complications. METHODS: Between July 2022 and June 2023, we operated on 19 legs of 14 consecutive TTS patients (5 males, 11 females; average age 73.3 years). We made a 2- to 3-cm zig-zag skin incision on the tarsal tunnel. After posterior tibial nerve decompression by posterior tibial artery (PTA) transposition, the subcutaneous layer was tightly sutured with 4-

MR imaging in tarsal tunnel syndrome.

Magnetic resonance imaging was used to demonstrate the normal anatomy of the tarsal tunnel in two volunteers and to evaluate 33 feet in 27 patients with tarsal tunnel syndrome. The tarsal tunnel is a fibroosseous channel extending from the ankle to the midfoot, through which the medial tendons and the posterior tibial neurovascular bundle pass. Tarsal tunnel syndrome is a compression neuropathy of the posterior tibial nerve or one of its branches and may be caused by a variety of pathologic lesions. Magnetic resonance imaging demonstrated a mass lesion in five feet, dilated veins or varicositi

A rare case of an accessory flexor hallucis longus causing tarsal tunnel syndrome.

Tarsal tunnel syndrome (TTS) is a rare entrapment neuropathy of the tibial nerve within the fibro-osseous tarsal tunnel for which multiple etiologies, including trauma, congenital foot abnormalities and space occupying lesions, have been described. We present an unusual case of TTS caused by an accessory Flexor Hallucis Longus (FHL) tendon. Surgical excision led to a complete resolution of symptoms and improved the quality of life of our patient.

Radioguided Occult Lesion Localization in Deep Schwannomas of the Peripheral Nerves: Results of a Preliminary Case Series.

BACKGROUND: The detection of small deep schwannomas of the peripheral nerves has been increasing since the the use of precise neuroimaging techniques has become more widespread; however, although nonpalpable lesions can be well defined by images, it is often difficult to identify them during the surgical procedure. The authors report seven cases of nonpalpable small deep schwannomas surgically treated after their identification using the radioguided occult lesion localization (ROLL) technique. METHODS: Seven men, whose ages ranged from 34 to 70 years (mean 52 years), presented with symptomati
