Partial liquid ventilation versus conventional mechanical ventilation with high PEEP and moderate tidal volume in acute respiratory failure in piglets.

This prospective randomized pilot study aimed to test the hypotheses that partial liquid ventilation combined with a high positive end-expiratory pressure (PEEP) and a moderate tidal volume results in improved gas exchange and lung mechanics without negative hemodynamic influences compared with conventional mechanical ventilation in acute lung injury in piglets. Acute lung injury was induced in 12 piglets weighing 9.0 +/- 2.4 kg by repeated i.v. injections of oleic acid and repeated lung lavages. Thereafter, the animals were randomly assigned either to partial liquid ventilation (n = 6) or con

Discrepancy in applied tidal volumes between t-piece and lung level during delivery room stabilization - a case report.

We present data of a preterm infant at 28 4/7 weeks gestation with inadequate response to non-invasive ventilation after birth who was intubated five minutes after delivery. During non-invasive stabilization, we noted a discrepancy between the displayed tidal volumes on the respiratory function monitor (RFM) and the real-time electrical impedance tomography (EIT) measurements. While expiratory tidal volumes on the RFM were in the recommended target of 4-6 mL/kg, EIT showed only minimal tidal volumes (corresponding to approximately 1-3 mL/kg), which was more consistent with the clinical impress

Impact of delivered tidal volume on the occurrence of intraventricular haemorrhage in preterm infants during positive pressure ventilation in the delivery room.

BACKGROUND AND OBJECTIVES: Delivery of inadvertent high tidal volume (VT) during positive pressure ventilation (PPV) in the delivery room is common. High VT delivery during PPV has been associated with haemodynamic brain injury in animal models. We examined if VT delivery during PPV at birth is associated with brain injury in preterm infants <29 weeks' gestation. METHODS: A flow-sensor was placed between the mask and the ventilation device. VT values were compared with recently described reference ranges for VT in spontaneously breathing preterm infants at birth. Infants were divided into two

No change in the regional distribution of tidal volume during lateral posture in mechanically ventilated patients assessed by electrical impedance tomography.

We assessed the distribution of regional lung ventilation during moderate and steep lateral posture using electrical impedance tomography (EIT) in mechanically ventilated patients. Seven patients were placed on a kinetic treatment table. An elastic belt containing 16 electrodes was placed around the chest and was connected to the EIT device. Patients were moved to left and right lateral positions in a stepwise (10 degrees ) mode up to 60 degrees. EIT images [arbitrary units (AU)] were generated and scanned for assessment of relative ventilation distribution changes [tidal volume (V(T))]. A cal

Effect of two tidal volumes on oxygenation and respiratory system mechanics during the early stage of adult respiratory distress syndrome.

PURPOSE: To study the effect of two tidal volumes on gas exchange, lung mechanics, and hemodynamics in 12 patients with acute respiratory distress syndrome (ARDS) within the first 72 hours of mechanical ventilation. METHODS: Tidal volume (VT) was increased by 40% from the initial value at fixed positive end-expiratory pressure (PEEP) and matched minute ventilation by adjusting the respiratory rate (RR) of the ventilator. Initial VT and RR were 592 +/- 42 mL and 19 +/- 1 min-1, respectively. High VT amounted to 825 +/- 54 mL with a RR of 12 +/- 1 min-1. RESULTS: We found that at high VT (1) t

Low tidal volume ventilation facilitates spontaneous increase in bronchoconstriction and air trapping that can be resolved by deep inspiration and bronchodilator.

Prolonged absence of deep inspiration (DI) increases airway resistance. The underlying mechanism is not entirely clear. We hypothesize that DI prohibition allows basal airway smooth muscle (ASM) tone to narrow and close airways over time, resulting in elevation of airway and lung resistance, as well as air trapping. We further hypothesize that DI or pharmacological bronchodilators can prevent or alleviate the resistance increase and air trapping. Physiological respiration was simulated in ex vivo sheep lungs. Lung resistance, elastance, and volume were measured using small tidal volume (120 mL

A Novel Prototype Neonatal Resuscitator That Controls Tidal Volume and Ventilation Rate: A Comparative Study of Mask Ventilation in a Newborn Manikin.

The objective of this randomized controlled manikin trial was to examine tidal volume (VT) delivery and ventilation rate during mask positive pressure ventilation (PPV) with five different devices, including a volume-controlled prototype Next Step™ device for neonatal resuscitation. We hypothesized that VT and rate would be closest to target with the Next Step™. Twenty-five Neonatal Resuscitation Program providers provided mask PPV to a newborn manikin (simulated weight 1 kg) in a randomized order with a self-inflating bag (SIB), a disposable T-piece, a non-disposable T-piece, a stand-alone re

Postoperative Reduction in Postvoid Residual Urine Volume May Be Associated With Bladder Capacity Decrease Rather Than an Increase in Tidal Voided Urine Volume in Patients With Pelvic Organ Prolapse.

AIM: More than half of patients with pelvic organ prolapse (POP) complain of lower urinary tract symptoms (LUTS), especially voiding difficulty. Surgical approaches such as laparoscopic sacrocolpopexy (LSC) and transvaginal mesh surgery (TVM) often result in improved voiding function in POP cases. The present study primarily aimed to evaluate postoperative longitudinal changes in bladder-related parameters after POP repair rather than determine superiority between surgical techniques. METHODS: We retrospectively evaluated 216 patients with POP treated with LSC or TVM at our hospital. LSC was

A New Computational Method for Detecting Leak Flow and Tidal Volume Waveforms During Spontaneous or Mandatory Breathing Assisted with Nasopharyngeal Ventilation.

Nasopharyngeal ventilation (NPV) is a common technique used to support breathing, particularly when a patient's respiration is inadequate, such as under sedation. It involves delivering oxygen through an endotracheal tube positioned above the glottis. Accurate tidal volume measurement is crucial for anesthesiologists, with the gold standard being a pneumotachograph. However, due to leakage from the mouth or mask, this method has limitations when applied to NPV. This study introduces a computational model that calculates respiratory flow in real time by accounting for leak flow. Results show th

Intracerebroventricular administration of the angiotensin II receptor antagonist saralasin reduces respiratory rate and tidal volume variability in freely moving Wistar rats.

The possible importance of intra-individual variations in respiratory rate and tidal volume has recently gained interest in psychiatric research, as a result of the observations that patients with panic disorder or premenstrual dysphoric disorder display enhanced respiratory variability as compared to controls. Although the role of brain neurotransmitters in the regulation of breathing has been extensively studied, as yet data on the central regulation of respiratory variability is sparse. Prompted by previous studies indicating that angiotensin II (ANG II) may influence ventilation as well as

Association Between Dead Space to Tidal Volume Ratio and Duration of Respiratory Support After Extubation in Children With Congenital Heart Disease.

BACKGROUND: Children with cardiac disease liberated from mechanical ventilation often receive noninvasive respiratory support (NRS) postextubation via high-flow nasal cannula, CPAP, or noninvasive ventilation. Predicting the type and duration of postextubation NRS can be challenging due to a lack of objective tools to guide decision-making. The dead space to tidal volume ratio (VD/VT) is a potential tool to guide this decision. We hypothesized that an elevated VD/VT would be associated with longer duration and higher level of NRS following extubation in children with cardiac disease. METHODS:

High-frequency low-tidal volume ventilation improves procedural and long-term clinical outcomes in persistent atrial fibrillation ablation: Prospective multicenter registry.

BACKGROUND: High-frequency, low-tidal volume (HFLTV) ventilation increases the efficacy and efficiency of radiofrequency catheter ablation (RFCA) of paroxysmal atrial fibrillation. Whether those benefits can be extrapolated to RFCA of persistent atrial fibrillation (PeAF) is undetermined. OBJECTIVE: The purpose of this study was to evaluate whether using HFLTV ventilation during RFCA in patients with PeAF is associated with improved procedural and long-term clinical outcomes compared to standard ventilation (SV). METHODS: In this prospective multicenter registry (REAL-AF), patients who had u

Validity of tidal breathing flow volume loops in diagnosing obstructive sleep apnea in young children with adenotonsillar hypertrophy: a preliminary study.

Tidal breathing flow volume loops (TBFVL) can indicate the site/severity of upper airway obstruction (UAO). The authors did a pilot study to determine 1) the correlation between TBFVL and obstructive sleep apnea (OSA) as well as its severity and 2) the validity of TBFVL in determining OSA and desaturation during sleep in young children with a denotonsillar hypertrophy (ATH). A cross sectional analytical study was performed in 10 patients with ATH (age 4.2 +/- 0.4 yrs; 40% female) at King Chulalongkorn Memorial Hospital during January-June 2004. All had polysomnography and TBFVL performed durin

Respiratory motor recovery after unilateral spinal cord injury: eliminating crossed phrenic activity decreases tidal volume and increases contralateral respiratory motor output.

By 2 months after unilateral cervical spinal cord injury (SCI), respiratory motor output resumes in the previously quiescent phrenic nerve. This activity is derived from bulbospinal pathways that cross the spinal midline caudal to the lesion (crossed phrenic pathways). To determine whether crossed phrenic pathways contribute to tidal volume in spinally injured rats, spontaneous breathing was measured in anesthetized C2 hemisected rats at 2 months after injury with an intact ipsilateral phrenic nerve, or with ipsilateral phrenicotomy performed at the time of the SCI (i.e., crossed phrenic pathw

Agreement and Reliability of a Newly Developed Tidal Volume Monitoring Device in Bag-Valve Ventilation and Intubation Scenarios: A Simulation-Based Study.

Background: Manual bag-valve ventilation is a fundamental intervention in prehospital and emergency care but remains highly operator-related and frequently associated with inappropriate tidal volume delivery. Excessive or insufficient ventilation may adversely affect hemodynamics and clinical outcomes. Although real-time tidal volume monitoring has the potential to improve ventilation quality, its measurement performance across different airway conditions has not been sufficiently evaluated. Methods: This simulation-based repeated-measures study evaluated the measurement agreement and reliabil

Mechanical ventilation with high tidal volumes attenuates myocardial dysfunction by decreasing cardiac edema in a rat model of LPS-induced peritonitis.

BACKGROUND: Injurious mechanical ventilation (MV) may augment organ injury remote from the lungs. During sepsis, myocardial dysfunction is common and increased endothelial activation and permeability can cause myocardial edema, which may, among other factors, hamper myocardial function. We investigated the effects of MV with injuriously high tidal volumes on the myocardium in an animal model of sepsis. METHODS: Normal rats and intraperitoneal (i.p.) lipopolysaccharide (LPS)-treated rats were ventilated with low (6 ml/kg) and high (19 ml/kg) tidal volumes (Vt) under general anesthesia. Non-ven

Effects of smaller tidal volumes during basic life support ventilation in patients with respiratory arrest: good ventilation, less risk?

OBJECTIVE: When ventilating an unintubated patient in cardiac or respiratory arrest, smaller tidal volumes of 500 ml instead of 800-1200 ml may be beneficial to decrease peak airway pressure, and to minimise stomach inflation. The purpose was to determine the effects of small (approximately 500 ml) versus large (approximately 1000 ml) tidal volumes given with paediatric versus adult self-inflatable bags and approximately 50% oxygen on respiratory parameters in patients during simulated basic life support ventilation. METHODS: While undergoing induction of anaesthesia, patients were randomised
