Case of hidden tooth: mesiodens fortuitously discovered on a cone-beam CT examination.

Mesiodens is the most common type of supernumerary tooth, located between the maxillary central incisors. A young man was referred by his orthodontist for management of a supernumerary tooth located in quadrant I, superposed to the bottom of the right maxillary sinus, distally orientated with the crown in contact with the apex of the palatal root of the maxillary first molar. The tooth was found on a panoramic radiography before starting his orthodontic treatment. To remove it and in order to study, its relationship to the anatomical structures a cone-beam CT examination was performed. This re

Orthodontic extrusion of subgingivally fractured incisor before restoration. A case report: 3-years follow-up.

Orthodontic forced eruption may be a suitable approach without risking the esthetic appearance in tooth fracture below the gingival attachment or alveolar bone crest. Extrusion of such teeth allows elevating the fracture line above the epithelial attachment and so the proper finishing margins can be prepared. Restoration after orthodontic eruption may present a more conservative treatment choice in young patients compared with the prosthetic restoration after extraction. This case describes a multidisciplinary approach using the orthodontic forced eruption facilitating the composite restoratio

Digital 3D-Printed Surgical Guide for Removal of a Separated Instrument at the Maxillary Sinus Floor and Apical Microsurgery.

This case report describes a maxillary first molar with chronic apical periodontitis (CAP) and a separated endodontic instrument migrated to the maxillary sinus floor, which was successfully managed with digital guide-assisted endodontic microsurgery. A 20-year-old female patient presented with long-standing occlusal pain and discomfort in the left maxillary first molar. Clinical examination revealed a temporary restoration, tenderness to percussion, and no gingival swelling. During root canal treatment, the mesiobuccal (MB2) canal was found to be calcified and non-negotiable, and a small segm

This study was undertaken to anatomically investigate the apical portion of root canal. Teeth used in this study were thirty maxillary central incisors, thirty maxillary lateral incisors and thirty maxillary cuspids extracted from patients between the ages of 11 and 73. All of the teeth had periodontal disease, but none had periapical disease. Resorption of the root surface could not be seen with the naked eye. The results obtained were as follows: 1. Deviations of the root apex and the apical foramen. Both of the root apex and apical foramen of the central incisors and cuspids were displaced

[Severe complication of a bonded mandibular lingual retainer].

Bonding a flexible spiral wire retainer to the lingual surfaces of all six anterior mandibular teeth is a commonly used type of retention. Complications are rare but can be serious enough to produce biologic damage. This article presents a serious complication of a lingual flexible spiral wire retainer. Four years after the orthodontic treatment, a 20-years-old man sought treatment for a broken flexible spiral wire retainer. The clinical examination showed about 35° of buccal root torque of that tooth. A cone-beam computed tomography image showed that the root and the apex of the tooth were al

Treatment of necrotic calcified tooth using intentional replantation procedure.

Introduction. If the teeth are impacted by a chronic irritant, the pulp space possibly will undergo calcific changes that may impede access opening during root canal treatment. In such cases that conventional endodontic treatment is impossible or impractical, intentional replantation may be considered as a last solution to preserve the tooth. Methods. After failing to perform conventional root canal therapy for a necrotic calcified right mandibular second premolar, the tooth was gently extracted. The root apex was resected and the root end cavity was prepared and filled with calcium enriched m

Working length variation during multiple-visit endodontic treatment: An observational study.

This study investigated variation in working length during multiple-visit endodontic treatment. Patients (N = 106) with pulpitis or pulp necrosis, with or without apical periodontitis, were included. During the first appointment, glide path was performed with ProGlider and shaping with ProTaper Next. Working length was detected four times. Working length was then re-recorded prior to filling during the second appointment and a 0.5 mm threshold was selected as the minimum clinically meaningful variation. A logistic regression model was used to evaluate the impact of tooth anatomy, preoperative

Reimplantation of an avulsed mature permanent tooth after 6 days: a 1-year follow-up.

Tooth avulsion in children and adolescents demands emergency treatment to increase the chances of successful reimplantation. The treatment prognosis depends on intrinsic and extrinsic factors, such as the extra-alveolar time until reimplantation, storage medium, handling, and periodontal ligament condition of the avulsed tooth as well as the patient's general health. This case report describes the treatment of an 11-year-old boy who suffered an avulsion of the maxillary right central incisor followed by delayed reimplantation after the tooth was stored in a dry medium for 6 days. Although the

CBCT assessment of alveolar bone wall morphology and its correlation with tooth angulation in the anterior mandible: a new classification for immediate implant placement.

PURPOSE: This study aimed to quantify alveolar bone morphology, demonstrate the relationship between tooth angulation and alveolar bone thickness, and introduce a new classification for anterior mandibular teeth related to immediate implant placement (IIP). METHODS: Cone-beam computed tomography (CBCT) images of 211 anterior mandibular teeth were analyzed in sagittal slices to measure the thickness of the facial alveolar bone crest (FAB1) and apex (FAB2), and the lingual alveolar bone crest (LAB1) and apex (LAB2). Tooth angulation was classified as 1°-10°, 11°-20°, and >20° according to the t

Heritability of teeth angulation and inclination in patients with overt malocclusion.

PURPOSE: To use 3-dimensional cone beam computed tomography (CBCT) images obtained on patients before treatment with dental malocclusion to calculate the heritability estimates of full-tooth angulation and inclination. A heritability estimate is a number between zero and one indicating the percentage of genetic factors involved in determining the phenotypic variation observed. METHOD: CBCT images of 111 patients (51 sibships) were analysed measuring both tooth angulation and inclination using the Anatomage Invivo5 software for central incisor to second molar. The long axis of each tooth was d

Root and root canal morphology in maxillary second molar with fused root from a native Chinese population.

INTRODUCTION: Root fusion is an anatomic variation in maxillary second molars (MSMs); however, the nature of this canal morphology as it relates to its root anatomy has not been fully clarified. The purpose of this study was to investigate the relationship between features of fused roots and root canal anatomy in MSMs using micro-computed tomographic imaging. METHODS: One hundred eighty-seven extracted MSMs were scanned with the μCT50 (Scanco Medical, Bassersdorf, Switzerland), and their root and canal morphology was classified and analyzed using the classifications proposed by Yang and Vertu

Clastic cells are absent around the root surface in pulp-exposed periapical periodontitis lesions in mice.

INTRODUCTION: Clastic cells, originating from the monocyte-macrophage lineage, resorb mineralized tissues. In periapical periodontitis, alveolar bone around the tooth apex becomes resorbed; however, the roots of the teeth are often left intact by yet unknown mechanisms. Here, we examined the status of clastic cells in a periapical periodontitis model in mice. METHODS: Periapical periodontitis was induced by performing pulp exposure on the maxillary first molar. The contralateral maxillary first molar was used as a control. The maxillae were harvested, fixed, and subjected to μCT scanning and
