Most carcinoid tumors of the rectum are confined to the submucosa with a size less than 1-2 cm and are usually suitable for local excision, as metastasis to the regional nodes is limited. AIM: To assess the morphological impact of transanal endoscopic surgery on the sphincter apparatus using the modified Starck classification.
METHODS: A prospective, observational study of 118 consecutive patients undergoing Transanal Endoscopic Operation/Transanal Endoscopic Microsurgery (TEO/TEM) from March 2013 to May 2014 was performed. All the patients underwent an endoanal ultrasound prior to surgery and one and four months postoperatively in order to measure sphincter width, identify sphincter defects and to quantify them in terms of the level, depth and size of the affected anal canal.

BACKGROUND AND AIMS: Advanced rectal lesions (ARLs), including large laterally spreading lesions (LSLs), villous or high-grade dysplastic adenomas, and early rectal cancers, constitute a critical transitional category between benign adenoma and invasive carcinoma. Historically, radical surgery was liberally employed for these lesions, often resulting in significant morbidity and permanent stoma formation. Contemporary endoscopic and transanal techniques permit curative resection while preserving anorectal function, but careful assessment is mandatory to avoid undertreatment of covert cancers.

BACKGROUND: The aim of this study was to compare the short-term outcome after Transanal Endoscopic Microsurgery (TEM) and Transanal Minimally Invasive Surgery (TAMIS) for intraluminal rectal lesions.
METHODS: Retrospective analysis of a prospectively maintained database of all TEM and TAMIS procedures performed at a single institution by one surgeon between March 2009 and September 2017 was conducted. Primary outcome was operating time. Secondary outcomes were blood loss, pathological outcome, length of hospital stay, 30-day readmission and mortality.

INTRODUCTION: Villous adenomas are dubiously benign lesions, which are difficult to interpret because of their malignant potential. Distal villous adenomas present with bleeding or mucus discharge. Giant adenomas are not amenable for endoscopic or transanal resection. Only few isolated cases have been reported by laparoscopic resection. We present our case of a circumferential giant villous adenoma of the rectum managed successfully by laparoscopic ultra-low anterior resection with colo-anal anastomosis with a review of literature in regard to their malignant potential.

INTRODUCTION: Rectourethral fistula is an uncommon pathology, usually iatrogenic, occurring as a complication of surgical or ablative treatments for prostate or rectal cancer.
