BACKGROUND: Transfer from the intensive care unit to an acute care unit is reportedly a stressful time for adult patients and their families. However, little has been done to understand these transition experiences and needs in paediatric patients and their parents/caregivers.
AIM: To describe parent/caregiver perceptions of their child's transition from the paediatric general or cardiac intensive care unit to an acute care unit.
STUDY DESIGN: Using a descriptive electronic survey-based design, parents/caregivers reported their perceptions of the quality of their child's transition from a pa

Adolescents and young adults with chronic kidney disease (CKD), particularly those with genetic kidney diseases, face unique challenges as they transition from paediatric to adult nephrology care. This period is marked not only by changes in healthcare providers but also by significant developmental, psychosocial and medical complexities. In response, the European Renal Association Working Group on Genes and Kidney and the European Society for Paediatric Nephrology Working Group on Inherited Kidney Diseases have collaborated to develop practical advice for healthcare professionals involved in

The complexity of transition of pediatric patients to adult care is well recognized, with a multidisciplinary approach widely agreed to be essential. Despite extensive existing literature in this area, practical guidance as to the management of specific medical aspects and how to address these with patients and families is lacking, with little accessible literature to help guide practitioners. Many paediatric endocrine conditions have a genetic or epigenetic aetiology. Both presentation and evolution of their specific health issues may be unfamiliar to adult endocrinologists, few of whom have

Recent changes in the global health financing resulted in a steep reduction in donor support for HIV programmes worldwide, threatening years of progress in reducing infections and mortality. These developments require existing HIV programmes to rethink their priorities. Advanced HIV disease (AHD) is a condition present in approximately 30% of people living with HIV (PLHIV) presenting to HIV care programmes worldwide. These individuals are defined by their low immune status, having a CD4 cell count ≤200 cells/mm3 or World Health Organization (WHO) Clinical Stage 3 or 4 AIDS-defining illness-and

INTRODUCTION: "Comfort care" is a holistic approach for patients with terminal conditions, such as cancer, who are not expected to recover. It focuses on managing pain, among other end-of-life symptoms, and providing support to both the patient and their family during the dying process, which can last unpredictably from hours to days. End-of-life care concepts are often shaped by personal judgment and culture and thus lack a single consensus. This can lead to ambiguity in the term "comfort care" itself as well as create confusion in medical communication and treatment, making the transition to

As many as 90% of individuals with childhood onset diseases now live into adulthood. Due to this increase in lifespan, many, such as those with childhood onset neuromuscular disorders (NMDs), need health care transition to adult primary and specialty care providers. Despite growing recognition of the need for transition, only 17% of youth with special healthcare needs are receiving assistance with health care transition. One primary model of transition is the six core elements of transition as defined by the National Alliance to Advance Adolescent Health in their "Got Transition" which serves

AIM: Medical advancements will lead to more children with long-term illnesses and/or disabilities undergoing the transition to adult care. Previous studies show that many young adults are unprepared for this transition, and might suffer from loss of follow-up. This study aimed to investigate the post-transfer experiences of the transition among young adults with long-term illnesses and/or disabilities.
METHODS: A qualitative descriptive design was used. Three semi-structured focus group interviews were conducted with 15 participants (18-25 years of age) recruited via patient organisations foc

Continuation of opioids at transitions of care increases the risk of long-term opioid use and related harm. To our knowledge, no study has examined the implementability of opioid deprescribing interventions at transitions of care. Our scoping review aimed to identify the type of opioid deprescribing interventions employed at transitions of care and assess the implementability of tested interventions. Nine electronic databases were searched on 15 May 2023 for English-language studies of adults transitioning between care settings, where opioid deprescribing interventions targeting patients, clin

Through advances in long-term ventilation, the number of children with chronic respiratory insufficiency reaching adult age has increased tremendously. Therefore, transition of children from pediatric to adult care has become inevitable. Transition is necessary for medicolegal reasons, to increase autonomy of the young patients and because of change in the disease as a result of increasing age. Transition bears the risks of uncertainty of patients and parents, loss of the medical home or even loss of complete medical care. Good structural conditions, professional preparation of patient and par

Technological advances and the globalization of knowledge have led to a considerable increase in the number of patients with chronic gastrointestinal disease who transition from pediatric to adult care during one of the most vulnerable life stages: adolescence. The Transition Working Group of the Gastroenterology Committee of the Sociedad Argentina de Pediatría conducted an exhaustive literature search and summoned leading specialists in the most frequent chronic pathologies from all over the country to unify criteria based on evidence and experience. As a result, a series of recommendations a

PURPOSE: Here, we review issues regarding the transition from pediatric to adult-focused health care for individuals with osteogenesis imperfecta (OI).
RECENT FINDINGS: The clinical consequences of OI change during the lifespan. Fracture rates are lower in adults than in children with OI, whereas other manifestations are typically becoming more prominent in adults. The evidence base for the transition to adult health care in OI is thin, as the literature on the topic is limited to qualitative investigations on a small number of participants. A few tools to help with transition, such as a prog

Open spina bifida (OSB) is a common neural tube defect. Medical and surgical care involves addressing the baseline orthopedic, urologic, and neurological dysfunction as well as the changes or declines that may occur as the patient ages. Given the complexity of this disease, coordinated, multidisciplinary care involving specialists in neurosurgery, orthopedics, urology, rehabilitation and physical medicine, pediatrics, and psychology is necessary to establish and optimize baseline function. Traditionally in the US, pediatric multispecialty spina bifida clinics have provided the patient with a c

OBJECTIVE: The symptoms of juvenile idiopathic arthritis (JIA) and the necessity for continuous treatment may persist in adulthood. Therefore, patients with JIA need to be appropriately transferred to adult care. We aimed to analyse the timing of the patients' transition to adult care, and patients' self-management skills with the process and the quality of the transition.
METHOD: This study included 161 Finnish participants of the population-based Nordic JIA cohort who attended a 17 year follow-up appointment. Special attention was paid to the three groups: those referred by the paediatric r
