Trapeziometacarpal osteoarthritis: Arthroscopic hemitrapeziectomy using the Thenar Portal.

BACKGROUND: Although the standard 1Radial and 1Ulnar portals allow for visualisation of most of the trapezium articular surface, the proximity of these two portals can often make arthroscopic triangulation and visualisation of the most lateral capsule and joint surface challenging. Despite its already reported advantages in improving visualisation, there is little literature reporting the clinical experience with the Thenar Portal. The purpose of this study is to describe potential complications and the short-term clinical-radiographic outcomes of arthroscopic hemitrapeziectomy using the Thena

Arthroscopic interposition arthroplasty of the trapeziometacarpal joint.

PURPOSE: In carpometacarpal (CMC) arthritis of the thumb, the use of interposition techniques (polylactic acid, pyrocarbon, dacron) has been increasing recently. These techniques are most often combined with open or arthroscopic complete or partial trapeziectomy. This article reports the results at one year of the arthroscopic interposition of an absorbable implant performed without trapeziectomy. METHODS: Our series included 25 patients aged 60.5 years on average, presenting with osteoarthritis of the trapeziometacarpal joint that had been medically treated for 18.5 months on average. All pa

The carpal bones in Poland syndrome.

OBJECTIVE: Classical Poland syndrome is represented by unilateral aplasia of the sternocostal head of the pectoralis major muscle and ipsilateral simple syndactyly and brachydactyly. Various classifications of the severity of hand involvement have been proposed. Since its initial description, numerous studies have been made of the bony, soft tissue, organ, and hematological disturbances. However, carpal bone involvement has been largely overlooked. The purpose of this study was to evaluate the carpal bones in patients with Poland syndrome from a local (Manitoba) cohort as well as those from th

Radial versus dorsal plating for trapeziometacarpal arthrodesis: a comparative biomechanical study.

Trapeziometacarpal arthrodesis is used for the treatment of advanced arthritis. Insufficient stabilization of the joint may lead to nonunion or hardware problems after arthrodesis. The purpose of this study was to compare the biomechanical properties of dorsal versus radial plate fixation of the trapeziometacarpal joint in ten pairs of fresh-frozen cadaveric hands. The biomechanical performance of each group was measured for stiffness in extension and flexion and load to failure using cantilever bending tests. The stiffness in extension was lower in the dorsally positioned group than in the ra

Use of the Swanson silicone trapezium implant for treatment of primary osteoarthritis : long-term results.

BACKGROUND: Instability of the prosthesis and silicone-induced synovitis have led most surgeons to abandon use of the Swanson trapezium implant for the treatment of primary osteoarthritis. However, the literature contains little information on the results of long-term follow-up. The present study was conducted to establish the long-term results and to highlight the problems associated with the implant. METHODS: Thirty-five patients (forty-five implants) of our initial forty-five patients (fifty-seven implants) were available for clinical review. The mean duration of follow-up was 13.8 years.

Clinical outcome comparison of suture button suspensionplasty versus trapeziectomy with ligament reconstruction and tendon interposition in patients with carpometacarpal joint arthritis: a systematic review of direct comparison studies.

INTRODUCTION: Trapeziectomy nowadays is predominantly done with Ligament Reconstruction and Tendon Interposition (LRTI) or Suture Button Suspensionplasty (SBS). This article is intended to highlight comparison between SBS and trapeziectomy with LRTI through direct comparison evidence. MATERIAL AND METHOD: The LRTI technique involves the removal of the diseased trapezium bone and creating suspension through the use of flexor carpi radialis tendon, while SBS stabilize the metacarpal base by suspending the first metacarpal bone. The record screening process of this systematic and meta-analysis w

Flexor carpi radialis tendinopathy after the hammock procedure for trapeziometacarpal arthritis.

PURPOSE: Suspension arthroplasty using the abductor pollicis longus (APL) tendon is widely performed for advanced trapeziometacarpal (TMC) arthritis. The hammock method involves wrapping the APL tendon around the flexor carpi radialis (FCR) tendon for suspension. Although outcomes are generally favorable, FCR tendon-related complications-such as tendinitis and rupture-have been reported, though detailed analyses remain limited. METHODS: In this retrospective study, 21 hands in 21 patients who underwent the hammock procedure for primary TMC arthritis were evaluated, with a follow-up of more th

Does thumb metacarpophalangeal arthrodesis increase trapeziometacarpal loading?

INTRODUCTION: The combination of a trapeziometacarpal prosthesis and a metacarpophalangeal arthrodesis of the thumb was formally contraindicated by JY de la Caffinière. This is a landmark notion, but no theoretical, experimental or clinical study has confirmed it. Combined indications for arthrodesis and trapeziometacarpal prosthesis are rare. Isolated arthrodesis is more common. In both cases, it is important to avoid overloading the trapeziometacarpal joint. AIM: The aim of this study is to verify whether metacarpophalangeal arthrodesis really does increase the load on the trapeziometacarpa

Revisiting the Critical Pathology of Primary Thumb Trapeziometacarpal Osteoarthritis: Volar Beak Ligament Degeneration.

PURPOSE: Volar and dorsal capsular contributions to trapeziometacarpal joint stability have been extensively studied in laboratory models. We investigated the less-understood relationship between trapeziometacarpal joint osteoarthritis and capsuloligamentous integrity in human thumbs. METHODS: Eighty thumb trapeziometacarpal joints from two distinct groups of 20 anatomical donors were prospectively specified for dissection through either a dorsal (20 men, 20 women; mean age 83 (72-100) years) or volar (24 men, 16 women; mean age 90 (76-100) years) arthrotomy. What is the most important mechanical factor involved in trapeziometacarpal osteoarthritis development? A sensitivity analysis based on biomechanical modelling.

Few studies consider the variability of the model parameters. This study aimed to perform a sensitivity analysis of a trapeziometacarpal joint model, by performing 675 finite element simulations built from the combination of different morphologies, joint passive stiffness, and grip strategies to estimate the joint pressure. Pressure variability was significantly more affected by morphology than grip strategy and joint passive stiffness. The effect of morphology and grip strategy on joint pressure was significant. A significant correlation between the trapezium dorso-volar curvature and the joi
