Scaphoid Nonunion With Poor Prognostic Factors: The Role of the Free Medial Femoral Condyle Vascularized Bone Graft.

Background: To report on the results of free medial femoral condyle (MFC) vascularized bone graft for scaphoid nonunions with 1 or more poor prognostic factors. Methods: We have used the free MFC vascularized bone graft for scaphoid nonunions that have 1 or more factors associated with a poor prognosis. These were, a delay in presentation of over 5 years, a proximal pole nonunion, the presence of avascular necrosis (AVN), or previous nonunion surgery. We used this technique on 20 patients over a 4.5-year period. Results: Our overall union rate was 88.5% (17 of 19 patients), with 1 patient fail

Anterior Cervical Fusion with Stand-alone Trabecular Metal Cages to Treat Cervical Myelopathy Caused by Degenerative Disk Disease. Observations in 88 Cases with Minimum 12-month Follow-up.

BACKGROUND: Anterior cervical fusion (ACF) with autologous bone was reported > 50 years ago. The continuous development of materials with elastic properties close to that of the cortical bone improves induction of osteogenesis and simplifies the technique of interbody fusion. To determine the safety and efficiency of stand-alone trabecular metal (TM) (or porous tantalum) cages for ACF, we performed a retrospective analysis of 88 consecutive patients with one-level or two-level degenerative disk disease (DDD) causing cervical myelopathy treated by interbody fusion with stand-alone TM cages. [Scapho-trapezio-trapezoidal arthrodesis for scapho-trapezio-trapezoidal osteoarthritis].

Scapho-trapezio-trapezoid arthrodesis was originally performed for the treatment of scapho-lunate instability. However, only a few publications have described this technique for treatment of osteoarthritis of the scapho-trapezio-trapezoid (STT) joint. The purpose of this paper is to analyze the results of triscaphoid arthrodesis for STT osteoarthritis with a long-term follow-up. Thirteen cases of osteoarthritis of the STT joint in twelve patients, all treated by STT arthrodesis, were reviewed with an average follow-up of 60 months. Pain was classified according to Alnot's classification: eight

Angled BIO-RSA (bony-increased offset-reverse shoulder arthroplasty): a solution for the management of glenoid bone loss and erosion.

BACKGROUND: Glenoid deficiency and erosion (excessive retroversion/inclination) must be corrected in reverse shoulder arthroplasty (RSA) to avoid prosthetic notching or instability and to maximize function, range of motion, and prosthesis longevity. This study reports the results of RSA with an angled, autologous glenoid graft harvested from the humerus (angled BIO-RSA). METHODS: A trapezoidal bone graft, harvested from the humeral head and fixed with a long-post baseplate and screws, was used to compensate for residual glenoid bone loss/erosion. For simple to moderate (<25°) glenoid defects,

Validation of a technique for studying functional adaptation of the mouse ulna in response to mechanical loading.

Functional adaptation of the mouse ulna in response to artificial loading in vivo was assessed using a technique previously developed in the rat. Strain gauge recordings from the mouse ulnar midshaft during locomotion showed peak strains of 1680 muepsilon and maximum strain rates of 0.03 sec(-1). During falls from 20 cm these reached 2620 muepsilon and 0.10 sec(-1). Axial loads of 3.0 N and 4.3 N, applied through the olecranon and flexed carpus, engendered peak strains at the lateral ulnar midshaft of 2000 muepsilon and 3000 muepsilon, respectively. The left ulnae of 17, 17-week-old female CD1

Calvarial Vault Remodeling Technique for Lambdoid Craniosynostosis.

Premature fusion of the lambdoid suture is the most uncommon single-suture synostosis. It presents with a classic "windswept" appearance, a trapezoid-shaped head, and significant skull asymmetry notable for an ipsilateral mastoid bulge and contralateral frontal bossing. Because of the rarity of lambdoid synostosis, little is known about optimal techniques for its treatment. In particular, the proximity of the lambdoid suture to critical intracranial structures such as the superior sagittal and transverse sinuses represents a potential for significant intraoperative bleeding. Prior work has sho

Morphological and morphometric features of knee joint in wild boar (Sus scrofa).

In the study, it was aimed to reveal the structure of the knee joint anatomically and morphometrically in the wild pig known as the ancestor of the domestic pig. In the study, the left and right knee joints of mature 10 wild boars were used as material. For this purpose, the materials were fixed with 10% formaldehyde solution. Macroanatomical and morphometrical examinations of bones, menisci and ligaments involved in joint formation were performed after fixation. Trochlea and condyles of femur were examined, and various measurements were made. Trochlear labia were found to be the same size, an

The role of structural viscoelasticity in deformable porous media with incompressible constituents: Applications in biomechanics.

The main goal of this work is to clarify and quantify, by means of mathematical analysis, the role of structural viscoelasticity in the biomechanical response of deformable porous media with incompressible constituents to sudden changes in external applied loads. Models of deformable porous media with incompressible constituents are often utilized to describe the behavior of biological tissues, such as cartilages, bones and engineered tissue scaffolds, where viscoelastic properties may change with age, disease or by design. Here, for the first time, we show that the fluid velocity within the m

Endoscopic endonasal approach to the maxillary strut: anatomical review and case series.

OBJECTIVES/HYPOTHESIS: The maxillary strut is the bone that separates the foramen rotundum and superior orbital fissure. Tumors involving the lateral wall of the sphenoid sinus, posterior ethmoid, or posterior maxillary sinus may invade this region. The authors detail the anatomy of the strut and present a case series that emphasizes the importance and utility of this useful landmark during an endoscopic endonasal approach to lesions in this region. STUDY DESIGN: Cadaveric dissections and retrospective case series. METHODS: Endoscopic endonasal dissections were performed on six formalin-fixe

A Complex Case of Clino-Syndactyly with Fourth Metacarpal Aplasia.

Syndactyly is the most common congenital malformation of the hand, leading to the fusion of the digits and frequently affecting the ring and middle fingers. The incidence is 1 out of 2500 children, predominantly occurring in boys and Caucasians. Clinically, the malformation may present as a soft tissue or bony fusion, resulting in the union of the fingers characterised as complete or incomplete. This fusion may involve the phalanges but may also extend to the carpal/tarsal bones, even to the metacarpal or metatarsal level, rarely to the distal end of the forearm and lower leg. The malformation

Serrated needle design facilitates precise round window membrane perforation.

The round window membrane (RWM) has become the preferred route, over cochleostomy, for the introduction of cochlear implant electrodes as it minimizes inner ear trauma. However, in the absence of a tool designed for creating precise perforation, current practices lead to tearing of the RWM and significant intracochlear pressure fluctuations. On the basis of RWM mechanical properties, we have designed a multi-serrated needle to create consistent holes without membrane tearing or damaging inner ear structures. Four and eight-serrated needles were designed and produced with wire electrical discha

The carpometacarpal stress view radiograph in the evaluation of trapeziometacarpal joint laxity.

PURPOSE: Hypermobility at the thumb carpometacarpal (CMC) joint has been proposed as an explanation for abnormal loading and subsequent development of osteoarthritis. Radiographic evaluation of this joint is difficult owing to the obliquity of the articulation. We modified a previously described technique to obtain a bilateral stress radiograph of the thumbs to measure CMC joint laxity. The purpose of this study was to present the details of this modified technique and evaluate its reproducibility in a group of volunteer subjects. We hypothesized that this technique would be reliable and repro

Scaphotrapeziotrapezoid Fusion for Chemotherapy-induced Capitate Avascular Necrosis.

The capitate's retrograde vascular supply and limited intraosseous anastomoses predispose its proximal pole to avascular necrosis, although this remains rare. Although typically linked to trauma, connective tissue disorders, or corticosteroid use, avascular necrosis may also arise secondary to chemotherapy. We present the case of an 18-year-old woman with a history of childhood leukemia who developed capitate avascular necrosis. This case emphasizes the importance of considering avascular necrosis in chemotherapy-exposed patients presenting with atraumatic wrist pain and evaluates outcomes of

Glycopeptide bone penetration in patients with septic pseudoarthrosis of the tibia.

BACKGROUND AND OBJECTIVE: In the treatment of bone infections, a major determinant of the clinical response is the active drug concentration at the infected site. Because of the high prevalence of meticillin (methicillin)-resistant staphylococci and enterococci, glycopeptides are widely used for the treatment of bone and joint infections, but data on their penetration into human bone are lacking. The aim of our study was to measure vancomycin and teicoplanin concentrations in infected human bone under steady-state conditions and verify their relationship with inflammatory markers, patient demo
