[HYpertension due to sympathetic hyperactivity after severe head injury].

We present the case of a 34-year-old patient with severe head trauma and severe brain involvement. The patient deteriorated progressively and required several reinterventions for bleeding and brain herniation. We were consulted by neurosurgery due to irregular blood pressure control despite treatment with calcium antagonists. The patient had associated diaphoresis, tachypnoea and tachycardia, together with persistent high-grade fever with no microbiological evidence. Laboratory tests showed normal kidney function, with no proteinuria and no signs of left ventricular hypertrophy to suggest prev

Association of Cerebrospinal Fluid Biomarkers of Central Nervous System Injury With Neurocognitive and Brain Imaging Outcomes in Children Receiving Chemotherapy for Acute Lymphoblastic Leukemia.

IMPORTANCE: Little is known about treatment-related neurotoxic mechanisms in children with acute lymphoblastic leukemia (ALL) treated with chemotherapy only. OBJECTIVE: To examine concentration of cerebrospinal fluid (CSF) biomarkers of brain injury at ALL diagnosis and during cancer therapy and to evaluate associations with long-term neurocognitive and neuroimaging outcomes and relevant genetic polymorphisms. DESIGN, SETTING, AND PARTICIPANTS: This prospective cohort study included 235 patients with ALL who received a chemotherapy-only protocol. Patients provided CSF samples after diagnosis

Powered Exoskeletons for Walking Assistance in Persons with Central Nervous System Injuries: A Narrative Review.

Individuals with central nervous system injuries are a large and apparently rapidly expanding population-as suggested by 2013 statistics from the American Heart Association. Increasing survival rates and lifespans emphasize the need to improve the quality of life for this population. In persons with central nervous system injuries, mobility limitations are among the most important factors contributing to reduced life satisfaction. Decreased mobility and subsequently reduced overall activity levels also contribute to lower levels of physical health. Braces to assist walking are options for grea

Environmental enrichment decreases asphyxia-induced neurobehavioral developmental delay in neonatal rats.

Perinatal asphyxia during delivery produces long-term disability and represents a major problem in neonatal and pediatric care. Numerous neuroprotective approaches have been described to decrease the effects of perinatal asphyxia. Enriched environment is a popular strategy to counteract nervous system injuries. The aim of the present study was to investigate whether enriched environment is able to decrease the asphyxia-induced neurobehavioral developmental delay in neonatal rats. Asphyxia was induced in ready-to-deliver mothers by removing the pups by caesarian section after 15 min of asphyxia

Intercostal vessels and nerves are at risk for injury during supracostal percutaneous nephrostolithotomy.

PURPOSE: We characterized key intercostal anatomical relationships relevant to supracostal percutaneous nephrostolithotomy using anatomical dissection. MATERIALS AND METHODS: We performed 20 cadaveric dissections of the 11th intercostal space to characterize key anatomical relationships relevant to percutaneous upper pole renal access. Specific data recorded included intercostal rib distance at the lateral border of the paraspinous muscles, and the mid scapular and posterior axillary lines. We also recorded the distance between the 12th and 11th ribs to the intercostal nerve, artery and vein

Can no-fault compensation of impaired infants alleviate the malpractice crisis in obstetrics?

In 1987, Virginia initiated no-fault compensation for birth-related neurological injuries in an attempt to ensure the availability of malpractice insurance for the state's obstetricians. This paper explores some possible causes for the refusal of Virginia's insurers to write malpractice coverage for obstetricians and analyzes the ability of the act to resolve the medical malpractice crisis in obstetrics. It also examines the effect of this limited no-fault compensation scheme on obstetricians' incentives and on the welfare of neurologically damaged children.

The Role of Lipids, Lipid Metabolism and Ectopic Lipid Accumulation in Axon Growth, Regeneration and Repair after CNS Injury and Disease.

Axons in the adult mammalian nervous system can extend over formidable distances, up to one meter or more in humans. During development, axonal and dendritic growth requires continuous addition of new membrane. Of the three major kinds of membrane lipids, phospholipids are the most abundant in all cell membranes, including neurons. Not only immature axons, but also severed axons in the adult require large amounts of lipids for axon regeneration to occur. Lipids also serve as energy storage, signaling molecules and they contribute to tissue physiology, as demonstrated by a variety of metabolic

Biofeedback with heart sounds following a traumatic event.

BACKGROUND: Some countries have experienced and may continue to experience severe devastation due to wars, epidemics, earthquakes, and other natural catastrophes. When individuals are exposed to specific psychological or physical traumas, they can develop stress-related disorders, which can lead to irreversible changes in the autonomic nervous system and the development of cardiovascular disorders. When subjected to a new stress test, traumatized participants or those previously exposed to trauma may show more pronounced behavioral changes, intolerance, and greater fatigue responses than befor

Post-traumatic Pott's puffy tumour: A case report.

INTRODUCTION: Osteomyelitis of the frontal sinus is a rare clinical entity and generally occurs as a complication of trauma to the forehead or frontal sinusitis. It can be responsible for life-threatening complications, as the first symptoms may appear to be minor. Early diagnosis and appropriate management to prevent central nervous system complications significantly reduce the morbidity and mortality. CASE REPORT: The authors report the case of a 34-year-old man with Pott's puffy tumour following trauma to the frontal sinus. DISCUSSION: The diagnosis was suggested clinically and confirmed

Complications of head injury.

Management of head injury is based on two concepts, proper treatment of the acute insult and the prevention and treatment of secondary insults. The head injured patient is subject to both intracranial and extracranial secondary insults. This paper will review complications related to the central nervous system as well as the pulmonary, infectious, gastrointestinal, and psychiatric complications frequently seen following traumatic brain injury. Complications following head trauma lead to significant acute and chronic morbidity and mortality. It is essential that clinicians be able to recognize

[How to do Things with Metaphors: Reflections on the Role of Metaphors and Metaphor Theory for the History of Science Using the Example of Shock Metaphors in Medicine].

In recent decades, metaphors have attracted a great deal of interest within the history, philosophy, and sociology of science. The article takes the growing interest in epistemic metaphors as the starting point of a discussion of two conflicting motives prevalent in theories of metaphor and metaphoricity: On the one hand, metaphors are associated with the indeterminacy of scientific discovery and the emergence of new epistemic objects; and on the other hand, metaphors are said to provide a filter of possible meanings and vantage-points. It is argued that an approach, which aims to do justice t

LDL receptor-related protein-1: a regulator of inflammation in atherosclerosis, cancer, and injury to the nervous system.

Low-density lipoprotein receptor-related protein-1 (LRP1) is an endocytic receptor for numerous proteins that are both structurally and functionally diverse. In some cell types, LRP1-mediated endocytosis is coupled to activation of cell signaling. LRP1 also regulates the composition of the plasma membrane and may, thereby, indirectly regulate the activity of other cell-signaling receptors. Given the scope of LRP1 ligands and its multifunctional nature, it is not surprising that numerous biological activities have been attributed to this receptor. LRP1 gene deletion is embryonic-lethal in mice.

Separated interface nerve electrode prevents direct current induced nerve damage.

Direct current, DC, can be used to quickly and reversibly block activity in excitable tissue, or to quickly and reversibly increase or decrease the natural excitability of a neuronal population. However, the practical use of DC to control neuronal activity has been extremely limited due to the rapid tissue damage caused by its use. We show that a separated interface nerve electrode, SINE, is a much safer method to deliver DC to excitable tissue and may be valuable as a laboratory research tool or potentially for clinical treatment of disease.

Traumatic musculocutaneous neuropathy: a case report.

Isolated injury of the musculocutaneous nerve is a rare disorder. Reported cases are claimed to present with loss of biceps and brachialis power without a disturbing pain. The injury generally occurs after strenuous exercise and could be demonstrated by electrophysiological examination. We report a case of musculocutaneous nerve injury which occurred after a vigorous push and which presented with unusual symptoms and findings. The patient complained of episodic severe pain attacks which started from the axilla and radiated over the musculocutaneous nerve distribution including the lateral ante

The jury is still in: Florida's Birth-Related Neurological Injury Compensation Plan after a decade.

Florida's Birth-Related Neurological Injury Compensation Plan (NICA) is the most significant experiment with compensation for medical injury yet undertaken in the United States. As NICA enters its second decade of operation, maintaining the scheme's jurisdictional integrity has emerged as a key challenge for policy makers in Florida. We explore the relationship that has emerged between NICA and the tort system as competing avenues for families to obtain compensation for severe birth-related neurological injury. By linking NICA claims data with data on malpractice claims filed in Florida, we fo
