Vascular Surgery Role in Vascular Trauma: 11-Year Analysis of Peripheral Vascular Trauma Management at a Level-1 Trauma Center.

BACKGROUND: Vascular surgeons are increasingly relied upon in the management of complex peripheral vascular trauma. The degree however that vascular surgery (VS) is involved in vascular trauma care is not well established. We hypothesize that VS consultation is required in a high portion of complex lower extremity vascular trauma. METHODS: A single-center retrospective review of all consecutive patients who sustained traumatic arterial injury of the lower extremity requiring open operative repair at a level-1 trauma center (from February 2009 to May 2020) was performed. Patients who underwent

Association of Geriatric Consult Services With Geriatric Processes of Care and Clinical Outcomes in US Trauma Centers.

BACKGROUND: Trauma centers are implementing geriatric-focused processes of care, including geriatric consult services, to improve outcomes for injured older adults. We hypothesized that trauma centers with a geriatric consult service would have more geriatric-focused processes of care and improved hospital-level mortality for injured older adults. METHODS: We surveyed US trauma centers regarding compliance with geriatric-focused processes of care, including presence of geriatric consult service, palliative care consult service, geriatric pharmacists, screening for frailty, screening for poten

Characteristics and Outcomes of Patients with Self-directed Violence Presenting to Trauma Centers in the United States.

INTRODUCTION: Psychiatric conditions are common presentations to the emergency department, and their prevalence has been steadily increasing. Part of this spectrum of presentations is self-directed violence. Self-directed violence involves suicidal acts and non-suicidal self-injuries that can result in serious morbidity and mortality. This study examines characteristics and outcomes of patients who presented to US trauma centers with self-inflicted injuries and identifies factors associated with survival to hospital discharge in this patient population. METHODS: We extracted data in a retrosp

Use of Andexanet Alfa for Factor Xa Inhibitor Reversal in US Verified Trauma Centers: A National Survey.

Direct oral factor Xa inhibitors are replacing vitamin K-dependent antagonists as anticoagulation treatment in many clinical scenarios. Trauma centers are noting an increase in patients presenting on these medications. The 2018 Food and Drug Administration approval of andexanet alfa provides an alternative anticoagulation reversal. Barriers may limit utilization of new medications including a lack of grade 1A evidence supporting the use of prothrombin complex concentrate (PCC) versus andexanet alfa and cost. To evaluate barriers of andexanet alfa utilization by trauma surgeons, a 15-question s

Role of preoperative CT imaging in penetrating thoraco-abdominal injuries: A multicenter study of urban trauma centers.

INTRODUCTION: Management of penetrating thoracoabdominal (PTA) injuries with signs of hemorrhage have warranted operative intervention but improved imaging capabilities have redefined interventions required. We examined outcomes of hemodynamically stable patients undergoing preoperative CT imaging with the hypothesis that CT imaging would decrease OR time without delaying OR arrival. METHODS: A retrospective multicenter study was performed amongst four urban trauma centers examining hemodynamically stable patients with PTA injuries requiring operative intervention from January 2017-December 2

Golf cart injuries in children: the experience of trauma centers in a single state in the USA.

Golf carts remain a source of morbidity in children. We aimed to establish the incidence and injury characteristics of pediatric patients injured due to golf cart usage over an 11-year period. This is a retrospective study using a single state trauma database of patients ages 0-17 years admitted to a trauma center and who sustained injures while a golf cart was in use. Thirteen Pediatric and Adult Trauma Centers within the state of Pennsylvania were evaluated from 1 January 2004 to 31 December 2014. The inclusion criteria were met by 108 patients. The mean annual incidence of injuries was 0.35

Factors associated with orofacial injury and willingness to participate in interventions among adolescents treated in trauma centers.

PURPOSE: Assault is the most common cause of facial injuries in adolescents treated at inner-city trauma centers, yet little is known about the behavioral and environmental antecedents of these injuries or the willingness of such at-risk adolescents to participate in behavioral interventions to minimize reinjury. The purpose of this study was to identify possible risk and protective factors among adolescents with assault-related facial injury and to assess their willingness to participate in prospective observational research and behavioral interventions. PATIENTS AND METHODS: Interviews were

Predicting the Trauma and Injury Severity Score (TRISS) in patients admitted to trauma centers after trauma: A modeling study based on data from the French National Programme for Medicalization of Information.

BACKGROUND: Reliable mortality prediction is central to trauma research, quality assessment, and resource allocation. While the French national hospital discharge database (PMSI) provides comprehensive nationwide data, it does not capture clinical parameters required for established severity scores such as TRISS, which is used for predicting in-hospital survival after trauma and necessary for implementing national-level evaluations of health system performance. This study tested whether TRISS could nonetheless be accurately derived from PMSI data. METHODS: We conducted a retrospective study o

Statewide assessment of injury and death rates among riders of off-road vehicles treated at trauma centers.

BACKGROUND: Injuries and deaths among riders of off-road motorized all-terrain vehicles are increasing in the US. We hypothesized that serious injuries in Oregon have increased among riders of both four-wheel and two-wheel vehicles. STUDY DESIGN: We analyzed the Oregon Trauma Registry. Seriously injured patients treated in the state's designated urban and rural trauma centers were identified using E-codes (821.0 to 821.9), which indicate whether patients were riding either an off-road all-terrain four-wheel vehicle (ATV) or off-road two-wheeled motorcycle (ORMC). Second, we performed a supple

Patterns of injury among drivers hospitalized in Level-I Trauma Centers: have frontal airbags made a difference?

This study describes the injuries of drivers discharged from Level-I Trauma Centers between 1995-1997. Differences in the drivers' injuries and outcomes by airbag deployment status and gender were evaluated using Chi 2 and T-tests. Data on 1,065 drivers (66 with airbags, 423 females) were obtained from the Massachusetts Registry of Motor Vehicle and Trauma Registries. Once admitted to trauma centers, drivers with airbags did not differ from drivers without airbags regarding the number, type, clustering, severity or outcome of their injuries. The only exception was that female drivers sustained

Knowledge, attitude, and practice toward hypothermia in severe trauma patients among healthcare professionals in emergency departments and trauma centers in Shanghai: a cross-sectional study.

BACKGROUND: Hypothermia is a critical component of the trauma triad of death and has been shown to increase morbidity and mortality among severely injured patients. Early recognition and effective temperature management are essential to improving outcomes, yet clinical knowledge and implementation remain inconsistent. Understanding healthcare professionals' preparedness for hypothermia management is therefore of great importance. To examine the knowledge, attitudes, and practices (KAP) of medical staff in emergency departments and trauma centers in Shanghai regarding the management of hypother

Cell Phone Activity and Trauma Patient Volume in New England Through the COVID-19 Pandemic: A Research Consortium of New England Centers for Trauma Study.

BACKGROUND: We sought to understand whether cell phone activity in commercial spaces as a marker for population mobility would be associated with trauma admission (TA) volumes, taking advantage of cell phone data made available during the COVID-19 pandemic and large swings in population activity. STUDY DESIGN: Trauma registry data from 6 Level I trauma centers (TCs) in New England were used to identify the number of daily TAs from January 20, 2020, to July 31, 2021. The device exposure index (DEX) is a standardized measure of daily cell phone interactions with other cell phones within a count

Does time to angiography affect the survival of trauma patients with embolization to the pelvis? A retrospective study across trauma centers in the United States.

INTRODUCTION: Traumatic pelvic injuries can result in rapid exsanguination. Bleeding control interventions include stabilization, angiography, and possible embolization. Previous studies yielded conflicting results regarding the benefit of a shorter time to embolization. OBJECTIVES: The aim of this study is to examine the impact of the time to angioembolization on the survival of patients presenting with pelvic injuries using a national database. MATERIALS AND METHODS: This was an observational retrospective study that used the National Trauma Data Bank 2017 dataset. Adult patients with pelv

Ultramassive Transfusion for Trauma in the Age of Hemostatic Resuscitation: A Retrospective Single-Center Cohort From a Large US Level-1 Trauma Center, 2011-2021.

BACKGROUND: Uncontrolled bleeding is a leading cause of death in trauma. In the last 40 years, ultramassive transfusion (UMT; ≥20 units of red blood cells [RBCs]/24 hours) for trauma has been associated with 50% to 80% mortality; the question remains as to whether the increasing number of units transfused in urgent resuscitation is a marker of futility. We asked whether the frequency and outcomes of UMT have changed in the era of hemostatic resuscitation. METHODS: We performed a retrospective cohort study of all UMTs in the first 24 hours of care over an 11-year period at a major US level-1 a

The Use of a Urinary Balloon Catheter to Control Hemorrhage From Penetrating Torso Trauma: A Single-Center Experience at a Major Inner-City Hospital Trauma Center.

INTRODUCTION: Use of a urinary catheter balloon tamponade (UCBT) in controlling traumatic hemorrhage is a frequently employed but infrequently described technique. We aim to discuss the experience of balloon tamponade as a bridge to definitive hemorrhage control in the operating room. METHODS: This is retrospective review at a single institution from January 2008 to December 2018. We identified patients with active bleeding from penetrating torso trauma in whom UCBT was used to tamponade bleeding. We used revised trauma score (RTS), injury severity score (ISS), and new trauma and injury sever

Evaluating Blunt Cerebrovascular Injury Screening in High-Energy Trauma Patients: A Rural Trauma Center Study.

INTRODUCTION: Blunt cerebrovascular injury (BCVI) is an uncommon but potentially devastating complication of blunt trauma. Current screening guidelines, such as the expanded Denver Criteria (eDC), rely on clinical and radiological indicators but may miss up to 20% of cases. In August 2023, our rural Level 1 trauma center implemented a new protocol that screens all blunt trauma patients with high-energy mechanisms (HEM) for BCVI. We hypothesized that the HEM protocol would identify more BCVI cases than the eDC. METHODS: This two-phase cohort study at a rural Level I trauma center evaluated BCV

The burden of blast injuries on a level i trauma center: a comparative analysis of blast versus blunt trauma.

PURPOSE: We hypothesized that blast trauma results in a greater institutional resource burden than blunt trauma. METHODS: This retrospective study compared blast trauma patients (N = 321) to blunt trauma patients (N = 9,172). A Propensity Score Matching (PSM) on Age, Sex and Injury Severity Score (ISS) was executed, yielding 1,481 matched blunt controls. The primary composite endpoint was Institutional Resource Burden, defined as requiring one or more of the following high-intensity resource metrics: prolonged hospital length of stay (LOS >7 days), ICU admission, or need for urgent surgical i
