Trigeminal cervical complex: A neural network affecting the head and neck.

OBJECTIVES: To introduce the trigeminal cervical complex (TCC) as a comprehensive framework for understanding the anatomical and functional scope of the afferences and efferences of the trigeminal nerve and explaining common otolaryngologic symptoms, including head and neck myofascial pain syndrome. Additionally, it explores how the bidirectional transmission of neurotransmitters contributes to the sensitization of the TCC and motor nuclei. METHODS: The study was conducted as a narrative review. The authors performed a comprehensive search of multiple databases, including Medline/OVID, Embase

Role of heat shock protein Hsp25 in the response of the orofacial nuclei motor system to physiological stress.

Although expression of the small heat shock protein family member Hsp25 has been previously observed in the central nervous system (CNS), both constitutively and upon induction, its function in the CNS remains far from clear. In the present study we have characterized the spatial pattern of expression of Hsp25 in the normal adult mouse brain as well as the changes in expression patterns induced by subjecting mice to experimental hyperthermia or hypoxia. Immunohistochemical analysis revealed a surprisingly restricted pattern of constitutive expression of Hsp25 in the brain, limited to the facia

Neuroscience of the human thalamus related to acute pain and chronic "thalamic" pain.

The association of posterior thalamic strokes with the presence of chronic "thalamic" pain was described in the early 1900s and revisited in a recent review of these patients. Acute pain in corporal structures is associated with the spinothalamic tract (STT), which originates in the dorsal horn of the spinal cord, whereas that associated with cranial structures is associated with the spinal division of the trigeminal nucleus. These pathways terminate in the ventral posterior nucleus (VP), including its posterior and inferior subnuclei and its core, which is classically associated with tactile

Regional brain structural changes during the transition from acute to chronic neuropathic pain: a longitudinal study in a rat model of orofacial pain.

Brain imaging studies have identified structural alterations in individuals with chronic neuropathic pain, particularly in regions involved in sensory, emotional, and cognitive processing. However, most studies remain cross-sectional, leaving the temporal progression of these changes during pain chronification unclear. This gap is critical, as both neural and non-neural cells undergo dynamic changes during pain chronification. The primary aim of this study was to track longitudinally, regional brain volume and density changes before and after induction of orofacial neuropathic pain in male rat

Smelling of odorous sex hormone-like compounds causes sex-differentiated hypothalamic activations in humans.

The anatomical pathways for processing of odorous stimuli include the olfactory nerve projection to the olfactory bulb, the trigeminal nerve projection to somatosensory and insular cortex, and the projection from the accessory olfactory bulb to the hypothalamus. In the majority of tetrapods, the sex-specific effects of pheromones on reproductive behavior is mediated via the hypothalamic projection. However, the existence of this projection in humans has been regarded as improbable because humans lack a discernable accessory olfactory bulb. Here, we show that women smelling an androgen-like com

Functional brainstem representations of the human trigeminal cervical complex.

BACKGROUND: The human in-vivo functional somatotopy of the three branches of the trigeminal (V1, V2, V3) and greater occipital nerve in brainstem and also in thalamus and insula is still not well understood. METHODS: After preregistration (clinicaltrials.gov: NCT03999060), we mapped the functional representations of this trigemino-cervical complex non-invasively in 87 humans using high-resolution protocols for functional magnetic resonance imaging during painful electrical stimulation in two separate experiments. The imaging protocol and analysis was optimized for the lower brainstem and uppe

The trigeminal nerve. Part I: An over-view.

The trigeminal nerve is the largest and most complex of twelve cranial nerves. Its vast size and influence are greatly appreciated when one attempts to diagnose and treat patients suffering from orofacial pain and temporomandibular joint disorders. Without a thorough knowledge of the trigeminal nerve, the efficacy of diagnostic and therapeutic procedures will be very disappointing. This is the first of a four-part series of articles about the trigeminal nerve, a basic over-view of both the gross and neuroanatomical structures is presented.

Evoked potentials in trigeminal neuralgia.

Trigeminal evoked potentials and sensory thresholds in response to maxillary gum stimulation were obtained in patients with a complaint of unilateral face pain. The patients and the volunteer, normal control groups had undergone no prior surgical or other procedures involving cranial or cervical structures. For statistical purposes, patient data were analyzed with respect to the diagnostic classification of classical trigeminal neuralgia, atypical trigeminal neuralgia, or other face pain states in which the pain extended beyond the trigeminal nerve distribution. Latencies of trigeminal evoked

Low-Grade Glioneuronal Tumor Adjacent to the Fifth Cranial Nerve Root With KIAA1549::BRAF Fusion Presenting With Trigeminal Neuralgia.

Low-grade glioneuronal tumors of the cranial nerves are rare, with only a few case reports describing their association with trigeminal neuralgia and no prior reports including genetic analysis. We present a case of a low-grade glioneuronal tumor located adjacent to the trigeminal nerve root in a 70-year-old man who had been experiencing severe left-sided trigeminal neuralgia for several months. He had initially presented with cranial polyneuropathy, for which oral steroid therapy was initiated. Magnetic resonance imaging of the head revealed the left superior cerebellar artery running near th

Increased expression of interleukin-18 in the trigeminal spinal subnucleus caudalis after inferior alveolar nerve injury in the rat.

Interleukin-18 (IL-18) is an important regulator of innate and immune responses, and is known to be expressed in various types of cells and upregulated in pathological conditions including tissue injury and inflammation, suggesting it has both proinflammatory and compensatory roles. Here we show that IL-18 was increased in microglia in the trigeminal spinal subnucleus caudalis (Vc) after peripheral nerve injury. We used a trigeminal neuropathic pain model in which the withdrawal threshold of maxillary whisker pad skin was significantly decreased after inferior alveolar nerve transection, and o

SPARCL1-containing neurons in the human brainstem and sensory ganglion.

Secreted protein, acidic and rich in cysteine-like 1 (SPARCL1) is a member of the osteonectin family of proteins. In this study, immunohistochemistry for SPARCL1 was performed to obtain its distribution in the human brainstem, cervical spinal cord, and sensory ganglion. SPARCL1-immunoreactivity was detected in neuronal cell bodies including perikarya and proximal dendrites, and the neuropil. The motor nuclei of the IIIrd, Vth, VIth, VIIth, IXth, Xth, XIth, and XIIth cranial nerves and spinal nerves contained many SPARCL1-immunoreactive (-IR) neurons with medium-sized to large cell bodies. A vibrissa pathway that activates the limbic system.

Vibrissa sensory inputs play a central role in driving rodent behavior. These inputs transit through the sensory trigeminal nuclei, which give rise to the ascending lemniscal and paralemniscal pathways. While lemniscal projections are somatotopically mapped from brainstem to cortex, those of the paralemniscal pathway are more widely distributed. Yet the extent and topography of paralemniscal projections are unknown, along with the potential role of these projections in controlling behavior. Here, we used viral tracers to map paralemniscal projections. We find that this pathway broadcasts vibri

The periaqueductal gray is the site of the antinociceptive action of carbamazepine as related to bradykinin-induced trigeminal pain.

Using freely moving and conscious rats, the antinociceptive effects of microinjections of carbamazepine, into the periaqueductal gray (PAG), nucleus reticularis paragigantocellularis (NRPG) and nucleus raphé magnus (NRM) on the biting-like responses induced by bradykinin applied to the tooth pulp, were investigated to determine the primary site of action of this drug. Microinjections of carbamazepine into the PAG ipsi- and contralateral to the stimulated tooth pulp produced dose-dependent suppressive effects on the biting-like responses within 1 min. The ED50 was 1.57 micrograms per rat, that

Prednisolone induces microglial activation in the subnucleus caudalis of the rat trigeminal sensory complex.

Prednisolone is a member of synthetic glucocorticoids which are widely used to treat chronic inflammatory diseases. In this study, neuronal degeneration and cell death, and glial reaction were investigated in the rat trigeminal ganglion (TG) and brainstem after subcutaneous injection of prednisolone for 7 days. Expression of c-Jun activating transcription factor 3 and caspase-3 was absent or infrequent in the TG, and cranial sensory and motor nuclei of saline- and prednisolone-treated animals. In these animals, distribution of calcitonin gene-related peptide-immunoreactive (-IR) neurons and ne

Functional localization of brainstem and cervical spinal cord nuclei in humans with fMRI.

BACKGROUND AND PURPOSE: To our knowledge, no published functional map of the human lower brainstem exists. Our purpose was to use 1.5-T functional MR imaging (fMRI) to visualize the location of cranial nerve (CN) nuclei and other pontine, bulbar, and cervical spinal cord nuclei by using specific sensory stimulation or motor performance. METHODS: We localized nuclei by using cross-correlation analysis of regional blood oxygen level-dependent (BOLD) signal intensity during specific motor and sensory procedures based on known functions of specific nuclei. Statistical parametric mapping (SPM) ana

Distribution of hyaluronan in the central nervous system of the frog.

The qualitative and quantitative distribution pattern of hyaluronan (HA), a component of the extracellular matrix (ECM), was studied in the frog central nervous system by using a highly specific HA probe and digital image analysis. HA reaction was observed in both the white and the gray matter, showing a very intense staining around the perikarya and dendrites in the perineuronal net (PN). In the telencephalon, strong reaction was found in different parts of the olfactory system, in the pallium, and in the amygdala. In the diencephalon, intensive staining was found in the nucleus of Bellonci,

Chronic dry eye induced corneal hypersensitivity, neuroinflammatory responses, and synaptic plasticity in the mouse trigeminal brainstem.

BACKGROUND: Dry eye disease (DED) is a multifactorial disease associated with ocular surface inflammation, pain, and nerve abnormalities. We studied the peripheral and central neuroinflammatory responses that occur during persistent DED using molecular, cellular, behavioral, and electrophysiological approaches. METHODS: A mouse model of DED was obtained by unilateral excision of the extraorbital lachrymal gland (ELG) and Harderian gland (HG) of adult female C57BL/6 mice. In vivo tests were conducted at 7, 14, and 21 days (d) after surgery. Tear production was measured by a phenol red test and
