Improved detection of Mycobacterium tuberculosis in lymph node aspirates through GeneXpert MTB/RIF assay in Bangladesh.

BACKGROUND: Despite tuberculous lymphadenitis (TBL) being the most prevalent type of extrapulmonary tuberculosis, there are limitations in laboratory diagnosis of TBL due to high cost, inadequate diagnostic efficacy and feasibility. Xpert MTB/RIF (Xpert) assay is a design-locked, molecular diagnostic technique that detects Mycobacterium tuberculosis (MTB) genome and rifampicin resistance by targeting rpoB gene and mutations within the gene. Currently, there is limited evidence validating Xpert assay usage for diagnosing TBL in Bangladesh. Therefore, in this study, we evaluated diagnostic effic

Serum metabolic disparity between patients with lymph node tuberculosis and patients with sarcoidosis: towards differential diagnosis.

BACKGROUND AND HYPOTHESIS: Sarcoidosis (SAR) and lymph-node tuberculosis (LNTB) are granulomatous diseases that present diagnostic challenges, especially in TB-endemic regions. We hypothesized that serum-metabolic profiles would help in differentiating SARs from LNTBs. OBJECTIVE: This study aimed to identify serum metabolic biomarkers to distinguish SAR from LNTB using NMR-based metabolomics analysis. METHODS: Serum samples were collected from 26 SAR and 22 LNTB patients. The serum metabolic profiles were measured using 800 MHz NMR spectroscopy and quantified using the commercial software CH

Mandibular lymph node tuberculosis in an 11-year-old child managed without surgery: a case report.

INTRODUCTION AND IMPORTANCE: Tuberculosis (TB) is a chronic infectious disease that can affect multiple anatomical sites. Although involvement of the maxillofacial region is rare, mandibular lymph node TB is particularly uncommon and may pose diagnostic challenges. CASE PRESENTATION: An 11-year-old male patient presented with a persistent swelling on the right side of the mandible for 8 months. Initial clinical assessment suggested a benign condition, and multiple courses of antibiotics failed to produce improvement. Histopathological examination following surgical intervention revealed granu

Monocytic niches escape T cell surveillance and promote Mycobacterium tuberculosis persistence in lymph nodes.

Lung-draining mediastinal lymph nodes (medLNs) are critical for Mycobacterium tuberculosis (Mtb) pathogenesis, serving both as sites of T cell priming and, paradoxically, reservoirs for long-term bacterial persistence. To understand this dichotomy, we examined myeloid and CD4+ T cell dynamics in medLNs after aerosol Mtb infection. Early bacterial dissemination occurred via monocytes and interleukin (IL)-12-producing conventional dendritic cells (cDCs), which initiated T helper 1 (Th1) cell priming within the T cell zone. Over time, cDC migration and T cell activation declined, and medLNs becam

The role of microbiota in bovine tuberculosis: Characterizing bacterial communities across lungs, gut, and lymph nodes.

Animals with bovine tuberculosis (bTB) develop granulomatous lesions as part of a complex immune response to chronic antigenic stimulus involving innate and adaptive mechanisms; however, the association of the microbiota with the development of and susceptibility to bTB remains poorly understood. This study characterizes the bacterial communities at different body sites involved in the pathophysiology of bTB, highlighting the gut-lung axis and their association with disease. Lungs, gut, thoracic, and mesenteric lymph nodes samples were collected from 31 bovines from a single herd (n = 19 bTB;

Coexistence of rectal adenocarcinoma and rectal and lymph node tuberculosis: a case report with a limited narrative review.

BACKGROUND: Rectal cancer concurrent with regional tuberculosis is a rare clinical entity. Due to its similar clinical symptoms and imaging features, it is frequently misdiagnosed, which profoundly complicates patient management. CASE PRESENTATION: A 72-year-old man with a history of pulmonary tuberculosis presented with three months of intermittent hematochezia and tenesmus. Because the patient self-reported the tuberculosis as cured and initial imaging showed no active pulmonary or evident extrapulmonary lesions, preoperative molecular tuberculosis screening was not initiated. Preoperative

Development and validation of a nomogram to predict atelectasis in adult lymph node fistula tracheobronchial tuberculosis patients.

BACKGROUND: Lymph node fistula tracheobronchial tuberculosis (TBTB) is a severe respiratory condition that can result in complications such as airway stenosis and atelectasis, posing significant clinical challenges, particularly in adults. Currently, no standardized assessment tools are available to predict the risk of atelectasis in these patients, highlighting the need to develop an effective predictive model to guide early clinical intervention and personalized treatment. METHODS: A retrospective study was conducted involving 547 adult patients diagnosed with lymph node fistula TBTB at our

The Effect of Add-On Corticosteroids on Treatment Outcomes in Lymph Node Tuberculosis: A Systematic Review and Meta-Analysis.

OBJECTIVE: To assess the effect of add-on corticosteroids on treatment outcomes in lymph node tuberculosis. METHODS: After registering the protocol on PROSPERO (CRD420251104064), we systematically searched Cochrane, MEDLINE, Embase and Web of Science for randomised controlled trials on adjunct corticosteroids for lymph node tuberculosis up to 20 October 2025. A random-effects model was applied, with heterogeneity quantified using I2 and between-study variance (τ 2) estimated by the Paule-Mandel method. Pooled effects were obtained using inverse-variance weighting, and confidence intervals wer

Pediatric Abdominal Tuberculosis With Calcified Intra-abdominal Lymph Nodes Identified by Point-of-Care Ultrasound.

BACKGROUND: Tuberculosis of the abdomen is one of the most common extrapulmonary manifestations of tuberculosis. Even in areas where tuberculosis is endemic, intra-abdominal tuberculous can pose a diagnostic and management challenge because of the lack of presence of overt clinical signs and availability of expertise for point of care diagnostics. Point-of-care ultrasound (POCUS) of the abdomen performed by emergency physicians is increasingly being used for a variety of clinical presentations to facilitate accurate diagnoses in the emergency department. CASE REPORT: We describe the case of a

Coexistence of metastatic breast carcinoma and primary tuberculosis in axillary lymph nodes: a report of a rare case.

Concomitant breast cancer metastasis and tubercular lymphadenitis in axillary lymph node is an extremely rare occurrence. Axillary lymph node metastasis is the most important factor in the staging of breast carcinoma and the number of axillary nodes showing metastases alters the stage. As tuberculosis also produces nodal enlargement, this can mimic or complicate the staging of malignant disease. Dual pathology in an organ can lead to difficulties in interpretation and inappropriate treatment of tuberculosis as well as carcinoma breast. Moreover, fine needle aspiration cytology (FNAC) from such

[CAUSES OF CHRONIC TUBERCULOSIS OF INTRATHORACIC LYMPH NODES IN CHILDREN, THE SPECIFIC FEATURES OF CLINICAL MANIFESTATIONS AND THERAPY].

Studying the results of examination in 59 children with tuberculosis of intrathoracic lymph nodes over time could establish the main causes of the chronic course of the disease, reveal the specific features of clinical manifestations, and define the optimal regimens of specific therapy. The chronic primary tuberculosis is characterized by a multiple intrathoracic lymph node lesion that corresponds to different phases of specific inflammation and is accompanied by complications in a third of cases. The signs of process activity are observed in 79.7%. Complex therapy lasted more than 10 months i

Gastric T-cell lymphoma presenting with epithelioid granulomas mimicking tuberculosis in regional lymph nodes.

In patients with malignant lymphomas, a sarcoid reaction is occasionally observed. However, lymphoma-related granulomas with caseous necrosis are rare. We describe such a case of T-cell gastric lymphoma that was difficult to diagnose. A 50-year-old man was referred to our hospital because of abnormal gastric endoscopic findings: hypertrophic folds with narrowing of the gastric lumen and multiple ulcers in the body. Gastric biopsy specimens showed non-specific inflammation. An open biopsy of the enlarged gastric regional lymph nodes was performed. The sections revealed effacement of the normal

Coexistence of breast cancer metastases and tuberculosis in axillary lymph nodes--a rare association and review of the literature.

The coexistence of metastatic breast cancer and tuberculosis in axillary lymph nodes is very rare. We present the case of a 57-year-old woman with multifocal invasive ductal breast carcinoma in whom the resected axillary nodes were found to harbor both metastatic cancer and tuberculous lymphadenitis. Thorough investigation revealed no evidence of primary tuberculosis elsewhere. A quantiFERON TB-Gold test was positive, indicating latent tuberculosis. The patient was treated with adjuvant chemotherapy antituberculous therapy, radiation and hormonal therapy with aromatase inhibitors. We conclude
